Outline Activities
Dr. Tsukiyama graduated Kyushu University School of Dentistry and proceeded to postgraduate school in 1987. He engaged in the research on stomatognathic function. He went to UCLA School of Dentistry between 1995 and 1997, and engaged in the research on temporomandibular disorders and orofacial pain. After coming back from US, he expanded the research fields to basic science using animal experiments. He is now engaging in the research on chronic pain, occlusal dysesthesia, evaluation of prosthodontic treatment, etc.
He has been teaching removable prosthodontics and stomatognathic function to undergraduate students through lectures, basic and clinical training courses. He has been also teaching postgraduate students and young dentists.
He has been serving several national and international academic societies, such as Japan Prosthodontic Society, Japan Society of Orofacial Pain, Asian Acedemy of Craniomandibular Disorders as a councilor and/or committee member. Recently, he has been involved in International Association for the Study of Pain - Orofacial Pain Special Interest Group (IASP OFP SIG) and International RDC/TMD Consortium Network as a member and contributing toward the development of international clinical guidelines.
He was promoted to a Professor of the Section of Dental Education, Faculty of Dental Science, Kyushu University, as of April 1st, 2017.

Epub 2019 Sep 3, 2020.01, Distress, suffering and care‐seeking behaviour are characteristics of pain‐related disease and illness. Pain that transitions from an acute to a chronic phase carries with it the potential of further effects: these include a worsening of the disease or illness; high‐impact chronic pain; and substantial personal, societal and economic burden. The biopsychosocial model directly addresses these multiple processes, yet clinical frameworks supporting this model are not universally implemented. This paper explores barriers to clinical implementation of a full biopsychosocial framework for temporomandibular disorders (TMD) and other oro‐facial pain (OFP) conditions. In June 2016, INfORM invited OFP researchers to a workshop designed to optimize the DC/TMD Axis‐II. Workshop groups identified five sources of implementation barriers: (1) cultures and societies, (2) levels‐of‐care settings, (3) health services, (4) cross‐cultural validity of self‐report instruments and (5) provider and patient health literacy. Three core problems emerged: (A) mental health aspects are seldom fully considered, thus impairing the recognition of illness, (B) training in use of validated multi‐axial assessment protocols is under‐rated and insufficiently used, and (C) clinical assessment often fails to recognise that sensory and emotional dimensions are fundamental aspects of pain. To improve patient care, these barriers and problems require action. Most importantly, TMD/OFP educators and researchers need to coordinate globally and (i) be educated in the biopsychosocial model, (ii) implement evidence‐based biopsychosocial guidelines for assessment and management of OFP conditions at their institutions, (iii) incorporate this model in undergraduate and postgraduate dental curricula and (iv) be responsive to stakeholders, including regulatory authorities and practitioners..

Tsukiyama Y, Yatani H, Sugisaki M, Kino K, Kobayashi K, Wajima K, Arima T, Kakudo K, Kubota E, Revision of the diagnostic criteria for temporomandibular disorders in Japan., The 3rd Asian Academic Congress for Temporomandibular Joint, 2013.06, The Japanese Society for the Temporomandibular Joint has been working to establish standard procedures for the diagnosis and management of temporomandibular disorders (TMD) since 1984. The classification system for TMD was proposed in 1986, and was established after the revision in 1996. Since then, it has been predominantly used in our daily clinic, not only by TMD specialists but also by general practitioners for 17 years. It has been frequently utilized in research and educational situations, and even used in the dental national board examination in Japan.
However, there are several limitations for this classification system. First, the reliability and validity of the system has never been examined. Second, there can be the discrepancy between the clinical diagnosis and the definitive diagnosis determined by imaging such as panoramic x-rays, CT, and MRI. Since the system does not accept the comorbid status, e.g., a patient with severe masticatory muscle pain exhibited bony changes in the temporomandibular joint (TMJ) by imaging, he/she shall be diagnosed as degenerative joint diseases only. In such a case, treatment strategies do not correspond to actual needs for a patient if the comorbid situation, i.e., muscle pain, is not considered.
In United States, the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) was published in 1992. The RDC/TMD have been accepted as the standard requirement for publishing in scientific journals, and considered as one of the most widely accepted clinical criteria in the biomedical field. They have been established based on the biopsychosocial model and have dual axes system, i.e., biological evaluation (Axis I), and evaluation of psychosocial factors and disabilities related to pain (Axis II). Recently, the RDC/TMD have been re-evaluated, and after the strict validation process the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) will be published soon.
Taking these recent movements and limitations of the existing classification system for TMD in Japan into consideration together, Japanese Society for the Temporomandibular Joint decided to revise the diagnostic system.
The concepts, procedures of revision and products will be outlined in the presentation..
