Satoshi Hiramine, Masaharu Nagata, Mosaburo Kainuma, The Impact of an Undergraduate Community‐Based Medical Education Program in a Japanese Urban City, 10.7759/cureus.54204, 2024.02, 【目的】本邦では世界的に類をみない程の高齢化の進展に伴い、地域医療を担うプライマリ・ケア医の必要性が高まってきており、その育成は大きな教育課題である。先行研究では、地域医療実習により学生の関心が高まることが示されているが、ほとんどの報告が僻地での実習に関するもので、都市部での実習の効果の検討は未だ不十分だと考えられた。本研究の目的は、都市部での学外実習が学生の地域医療に対する理解と態度に与える影響を評価することである。
【方法】九州大学医学部医学科5年生では、福岡市とその近郊に位置する診療所および病院での計1週間の地域医療実習が必修となっている。2018年から2019年までの実習履修者を対象に、Visual Analogue Scale (:VAS)を用いた自記式アンケートを実習の前後に実施した。調査項目は、かかりつけ医、訪問診療、高齢者の病態、多職種協働、コミュニケーションなどの医学的知識や能力に関する理解度と地域医療に対する印象や態度で、実習前後の変化をスチューデントのt検定を用いて評価した。
【成績】実習前後ともに回答した学生は188名（87.0%）であり、地域医療に関連する医学的知識や能力の理解度は全ての項目で有意に上昇した（P＜0.01）。地域医療に対する印象についても、「地域医療に従事している医師に魅力を感じる」、「将来、地域医療に従事したい」などのスコアが有意に上昇した（P＜0.05）。「将来、定着して働きたい場所」のスコアは、都市部から僻地に有意にシフトした（P＝0.002）。
【結論】都市部で行われた地域医療実習でも、学生の医学的知識や能力は伸び、地域医療に対する好意的な姿勢を育てることが明らかになった。また、僻地への志向性も高まる可能性が示唆された。.
