    Objectives: To investigate whether warming gadoxetic acid affects the frequency and degree of artifacts in the arterial phase of MRI. Methods: This prospective study included patients who underwent gadoxetic acid-enhanced MRI (April 2016−November 2017 and June−November 2018) at a single center. Either warmed (37 °C, n = 134) or non-warmed (24 °C, n = 137) gadoxetic acid was intravenously injected at a dose of 0.025 mmol/kg for 5 s. Breath-hold time for each phase was fixed at 20 s. Artifact severity was graded using a four-point scale. Patient background and artifact scores in precontrast, arterial, and portal phases were compared between groups. Results: Two hundred and seventy-one patients (mean age 64 ± 14 years, 149 male) were included. Patient background including the presence of pleural effusion, ascites, cirrhosis, and pulmonary disease, did not differ significantly between the two groups. The mean artifact score in the arterial phase was significantly lower in the 37 °C group than in the 24 °C group (1.4 ± 0.8 vs. 1.6 ± 0.9, p = 0.017), whereas no significant difference was observed in the precontrast and portal phases. The rate of substantial artifact (score = 3 or 4) in the arterial phase was significantly lower in the 37 °C group than in the 24 °C group (11.2 % vs. 21.1 %, p = 0.031). The number of patients with high artifact scores in the arterial phase compared to those in the precontrast image was also lower in the 37 °C group than in the 24 °C group (21.6 % vs. 36.0 %, p < 0.01). Conclusions: Warmed gadoxetic acid could reduce artifacts in the arterial phase of dynamic MRI.

    <p>Purpose: To compare diagnostic reliability between an intravoxel incoherent motion (IVIM) imaging and an arterial spin labeling (ASL) in assessment of renal blood flow in rodents.</p><p>Methods: We first evaluated 3 different fitting methods on 5 datasets of diffusion-weighted imaging (DWI) with 10 b-values (0–1000 s/mm<sup>2</sup>) for bi-exponential analysis in IVIM imaging to calculate pseudo-diffusion parameters. Coefficient of variation (CV) for each parameter and correlation among the parameters was assessed to test the robustness of the 3 fitting methods. Subsequently, DWI and ASL methods were performed before and 14 days after onset of acute kidney injury (AKI) in a rat model. Temporal change before and after AKI onset in the pseudo-diffusion parameters in 3 fitting methods was compared with that in the renal blood flow (RBF) derived in the ASL method.</p><p>Results: The CVs in all IVIM parameters were the lowest in the fitting method that estimated pseudo-diffusion parameters after a fixed true-diffusion was determined where the pseudo- and true-diffusion coefficients had no correlation. The RBF substantially reduced (~50%, <i>P</i> < 0.001) due to the AKI onset; however, no pseudo-diffusion parameters in any of 3 fitting methods could not detect the change. Further, any pseudo-diffusion parameters showed no correlation with the RBF.</p><p>Conclusion: Pseudo-diffusion parameters in the IVIM concept were not reliable to estimate RBF in the study. Since the kidney has a unique profile in the “tissue flow”, our data indicate that study design and interpretation of results needs to be carefully considered when IVIM imaging is used for evaluation of blood flow in tissue, especially in the kidney.</p>

    Fontan関連肝疾患(Fontan-associated liver disease:FALD)の原因は、高い中心静脈圧により肝にうっ血を生じることである。肝うっ血は肝の線維化をきたし、進行すると肝硬変の原因となる。また、特に線維化の強いFALDには肝細胞癌が生じることがある。MRIには肝うっ血や線維化の程度を評価できる種々のパラメーターがあり、FALDに伴う肝実質の変化を包括的に評価することが可能である。本研究ではFALD患者を対象に、種々のMRIパラメーターを評価し、FALDの新たな評価法を確立する。また、MRIによる肝細胞癌の発癌リスクの予測モデルを確立する。

    慢性肝疾患は未治療のまま経過すると肝の線維化が進行し、やがて肝硬変に至る。肝硬変は肝機能の低下や肝細胞癌のリスクを高めるため、線維化を早期に発見し、進行を防ぐことが重要である。そのためには、初期段階の線維化を高精度かつ非侵襲的に診断できる技術が求められる。
    本研究では、多核種MRIの一つである31 P-MRSという撮像法を用い、肝実質のリン酸代謝や細胞内pHを測定し、線維化の程度やそれに関与しうる微小環境の定量的評価を実現し、肝線維化の高精度かつ再現性の高い新たな診断法の確立を目指す。

    深部臓器である膵の硬さを体外から測定することは困難とされ、膵の硬さを体外から計測する手法は確立されていない。慢性膵炎は膵実質の脱落・線維化が生じる不可逆的な病態であるが、早期慢性膵炎の段階で膵の硬化・線維化がすでに生じているとされる。
    本研究では、臨床で広く普及しているCTを用いて膵の硬さ(線維化)を測定し病態解析を行う。膵の線維化は細胞外スペースの増加に反映されるので、dual energy CTで測定される細胞外液量から膵線維化を推定し、早期慢性膵炎の低侵襲的な診断法を確立する。この研究は、膵内分泌機能・外分泌機能のバイオマーカーや癌の悪性度診断、術後合併症の予測などへの応用が期待できる。

    Liver resection is currently considered the most effective treatment for patients with liver cancer. To the best of our knowledge, no study has investigated the association between gadoxetic acid-enhanced magnetic resonance imaging (MRI) findings and liver regeneration in patients who underwent hemihepatectomy. We aimed to clarify the relationship between the signal intensity of the liver parenchyma on gadoxetic acid-enhanced MRI and the degree of liver regeneration in patients who underwent hemihepatectomy. Forty-one patients who underwent gadoxetic acid-enhanced MRI before hemihepatectomy were enrolled. As a result, uptake of gadoxetic acid showed a positive correlation with liver regeneration. Uptake of gadoxetic acid may serve as a preoperative predictor of liver regeneration in patients undergoing hemihepatectomy.

    The aim was to establish a method to reflect the three-dimensional images of the kidney obtained on diagnostic imaging in the operative field (body surface).First, contrast-enhanced CT was performed in a case of cryotherapy for renal cell carcinoma to obtain the original data necessary to create three-dimensional images.Based on the data obtained, 2-3 different images were created with different characteristics, such as emphasising the lesion and surrounding organs.The next step was to reflect and display these images on the AR device.The process of transferring and displaying the acquired 3D images and data to the device did not go smoothly, and it was necessary to take measures to address this.The research period ended while various countermeasures, such as searching for and developing applications and changing devices, were being considered.

    In diffuse liver diseases, MRI enables non-invasive evaluation without the risk of radiation exposure. It allows assessment of liver morphology (such as shape and volume), detection and characterization of hepatic tumors, and evaluation of changes in the liver parenchyma (including fat and iron deposition, and fibrosis). Furthermore, the use of hepatocyte-specific contrast agents can improve tumor detection rates and facilitate tumor characterization. In this study, we focused on non-contrast-enhanced MRI using T1ρ and T2 mapping as quantitative assessment methods. The results demonstrated that T1ρ and T2 values can quantitatively evaluate liver fibrosis, necroinflammatory activity, and hepatic injury. In particular, T1ρ values were found to be more sensitive than T2 values in distinguishing between advanced fibrosis and non-fibrotic liver conditions.

    When we summarize the study results, the following three conclusions can be made: (1) the optimal presaturation pulse time is 250 msec, (2) the APT signal of focal nodular hyperplasias may show a high value (however, it is necessary to accumulate more cases in the future), (3) The APT signal in the liver parenchyma may reflect liver function, fibrosis and necro-inflammation from the viewpoint of protein synthesis. As we proceeded with the development of the sequence, we were concerned about the accuracy of analysis for small lesions due to the stability of the image quality of APT imaging. Therefore, we decided to evaluate only large liver tumors. This limited the enrollment of cases with hepatocellular carcinoma.

    First, we investigated the optimal reconstruction method of CT cholangiography for the evaluation of biliary tract cancer in ultra-high resolution CT. The reconstruction methods used were AiCE (Advanced intelligent Clear-IQ Engine), FIRST (Forward projected model-based Iterative Reconstruction SoluTion), and AIDR (Adaptive Iterative Dose Reduction)-3D, of which AiCE was the best. Next, we examined the optimal reconstruction method for CT using low-iodine contrast media in patients with poor renal function. The AiCE reconstruction method was superior to the conventional method.
