    赤血球には核もミトコンドリアもなく、赤血球膜がヘモグロビンを内包する単純な構造である。赤血球が酸素の運搬供与体である以上、酸化ストレスは不可避である。酸化ストレスは変形能を低下させ赤血球の老化を促進する。下記について概説した。1)赤血球膜の特徴、2)赤血球の老化、3)赤血球の変形能、4)酸化ストレスと変形能、5)生活習慣病(高血圧、糖尿病)と赤血球膜変形能、について述べた。

    心臓リハビリテーション(心臓リハビリ)とは、"理学療法士が実施する単なる理学療法"ではなく、"医師・看護師・理学療法士・薬剤師・臨床心理士・運動療法指導士などの多職種により編成されたチームが、対象患者に対して至適運動療法・患者教育・個別カウンセリングなどの多面的介入を実施する包括的疾病管理プログラム"である。その目標は単なる筋力アップ・体力増強に止まらず、運動耐容能改善・QOL向上・心不全再入院抑制、そして究極的には長期予後改善である(図1)。左室駆出率が低下した心不全(HFrEF)患者の長期予後改善をめざして、β遮断薬やACE阻害薬などによる標準薬物療法が推奨されるが、近年増加している左室駆出率の保たれた心不全(HFpEF)には有効ではなく、また薬物療法のみでは運動耐容能改善効果が期待できない。さらに、心不全再入院の要因として管理不十分による体液管理不良・感染や腎不全などの非心臓性併存疾患・標準治療へのアドヒアランス不良が明らかとなり、再入院抑制をめざす疾病管理プログラムが注目されている。外来心臓リハビリは、患者教育・生活指導と運動療法とを組み合わせたいわば"ハイブリッド疾病管理プログラム"であり、心不全再入院防止だけでなく運動耐容能・QOLの改善が期待でき、心不全の疾病管理における有用性が期待される。(著者抄録)
