    Background: Periodontal disease during pregnancy has been associated with an increased risk of preterm birth (PTB) and low birth weight (LBW). However, most previous interventional studies have reported that scaling and root planing (SRP), a form of basic periodontal treatment, is insufficient to effectively prevent PTB and LBW. Recent systematic reviews and meta-analyses suggest that combining SRP with routine use of chlorhexidine gluconate (CHG) mouthwash may reduce the risk of these adverse birth outcomes by approximately half. This study will be among the first randomized controlled trials (RCTs) to evaluate the potential synergistic effects of this combined approach on improving birth outcomes. Methods: This multi-center, three-arm RCT will be conducted at two hospitals in Dhaka and Bogura, Bangladesh, between April 2025 and September 2026. A total of 480 pregnant women less than 20 weeks’ gestation attending antenatal checkups will be recruited. Participants diagnosed with periodontal disease will be randomly assigned to either an intervention group or a positive control group. Participants without periodontal disease will be assigned to a negative control group. The intervention consists of SRP combined with continued use of CHG mouthwash throughout pregnancy. The primary outcome will be the rate of PTB. Secondary outcomes include the birth weight of the newborn baby, maternal periodontal status, and oral health-related quality of life (OHRQoL). Effectiveness will be assessed by comparing outcomes across the study groups. Discussion: The oral health intervention is expected to improve both birth outcomes and maternal oral health. Demonstrating its effectiveness in a real-world setting may support the integration of oral health care into broader maternal, neonatal, and child health strategies, particularly in resource-limited settings.     The prevalence of obesity, hypertension (HTN), and type 2 diabetes mellitus (T2DM) is increasing worldwide, with significant public health implications, particularly in low- and middle-income regions. However, epidemiological data for remote rural districts in Sabah, Malaysia remains limited. This report aimed to assess the epidemiology of noncommunicable diseases (NCDs) such as obesity, HTN and T2DM among voluntary health checkup participants across selected districts in Sabah, Malaysia. A total of 1,247 adults aged 18 years or above underwent health checkups as part of Sabah state’s rural outreach activities, which included anthropometric measurements, blood pressure assessment, and blood glucose tests. Data analysis revealed that the proportion of obesity, HTN, and suspected T2DM was 21.7%, 40.3%, 28.1%, respectively. The percentage of participants who had both HTN and suspected T2DM was 13.7%. Older age was significantly associated with HTN and suspected T2DM. Higher rates of HTN were observed among males. Significantly higher odds ratios for having obesity, HTN, suspected T2DM, were found in rural districts including Beaufort, Tawau, and Lahad Datu districts, compared to the urban district of Kota Kinabalu. Participants in rural districts had a significantly higher likelihood of having two or more of these NCDs (comorbidity) than participants in the Kota Kinabalu district after controlling for sex and age. The findings highlight the urgent need for targeted community health interventions and policies addressing lifestyle modifications, with a focus on at-risk demographics in rural areas. This study provides a foundation for future research on NCDs in Sabah and emphasizes the importance of preventative measures in reducing the growing burden of obesity, HTN, and T2DM in Malaysia.

    This study examined the association of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) intake during the second trimester with low birth weight (LBW) in pregnant Japanese women and was conducted in conjunction with the Japan Pregnancy Eating and Activity Cohort (J-PEACH) study. The study included 504 pregnant women from four Japanese sites. During the second trimester (14–27 weeks), the participants filled out a self-administered questionnaire assessing the frequency of DHA and EPA supplement intake in the past month, as well as a brief-type self-administered diet history questionnaire (BDHQ). The analysis involved data from two time points: responses to the BDHQ and infant data at birth. In total, 471 and 33 participants were classified into the normal birth weight and LBW groups, respectively. The participants were divided into high-, medium-, and low-intake groups based on their total dietary and EPA and DHA supplementary intakes. The Cochran–Armitage trend test was used to analyze the data; the prevalence of LBW was higher in the low-intake group (p = 0.04). There was no significant sex-based trend (p = 0.27 and p = 0.35). In Japanese women, low dietary and supplementary EPA and DHA intake until the second trimester were risk factors for LBW.

    Nausea and vomiting in pregnancy (NVP) is a common symptom. Although the influence of NVP during the first trimester on dietary intake and birth outcomes has been revealed, no study has focused on NVP during the second trimester. This study aimed to reveal whether NVP severity during the second trimester is associated with dietary intake, gestational weight gain (GWG), birth weight, and delivery week. Participants completed a questionnaire at 18–27 gestational weeks. NVP severity was assessed using the modified Pregnancy-Unique Quantification of Emesis and Nausea scale in the questionnaire. Dietary habits were assessed using a brief-type diet history questionnaire. In total, 825 responses were analyzed: 202 (24.5%), 135 (16.4%), and 8 (1.0%) women reported mild, moderate, and severe NVP, respectively; 480 (58.2%) women did not have NVP during the second trimester. No significant association was observed between energy and nutrient intake and no/mild and moderate/severe NVP. Women with moderate/severe NVP had lower total GWG than those with no/mild NVP (p = 0.007). There was no significant difference in low birth weight and preterm birth rates (p = 0.246 and p = 0.604). This is the first study to investigate whether NVP severity during the second trimester is associated with dietary intake and birth outcomes.

    Background: Ensuring an appropriate continuum of care in maternal, newborn, and child health, as well as providing nutrition care, is challenging in remote areas. To make care accessible for mothers and infants, we developed a telehealth care system called Portable Health Clinic for Maternal, Newborn, and Child Health. / Objective: Our study will examine the telehealth care system’s effectiveness in improving women’s and infants’ care uptake and detecting their health problems. / Methods: A quasi-experimental study will be conducted in rural Bangladesh. Villages will be allocated to the intervention and control areas. Pregnant women (≥16 gestational weeks) will participate together with their infants and will be followed up 1 year after delivery or birth. The intervention will include regular health checkups via the Portable Health Clinic telehealth care system, which is equipped with a series of sensors and an information system that can triage participants’ health levels based on the results of their checkups. Women and infants will receive care 4 times during the antenatal period, thrice during the postnatal period, and twice during the motherhood and childhood periods. The outcomes will be participants’ health checkup coverage, gestational and neonatal complication rates, complementary feeding rates, and health-seeking behaviors. We will use a multilevel logistic regression and a generalized estimating equation to evaluate the intervention’s effectiveness. / Results: Recruitment began in June 2020. As of June 2022, we have consented 295 mothers in the study. Data collection is expected to conclude in June 2024. / Conclusions: Our new trial will show the effectiveness and extent of using a telehealth care system to ensure an appropriate continuum of care in maternal, newborn, and child health (from the antenatal period to the motherhood and childhood periods) and improve women’s and infants’ health status.

    Pregnancy and postpartum are periods in which women develop psychosocially. However, becoming a mother is stressful, and mood disorders related to anxiety and depression often develop. In recent years, research on interoception—sensations related to the body’s internal physiological state—has attracted attention. Interoception has multifaceted characteristics. It involves directly perceiving information in the body while also inferring and evaluating it. In this study, we examined interoception, anxiety, and depression in Japanese pregnant women. Empirical examinations and questionnaire surveys were used to measure interoception in 32 pregnant women not at high risk of pregnancy. A Japanese adaption of the Multidimensional Assessment of Interoceptive Awareness was used to measure interoceptive sensibility, and a heartbeat counting task performance was used to measure interoceptive accuracy. Anxiety and depression were measured using the Japanese versions of the State-Trait Anxiety Inventory and the Edinburgh Postnatal Depression Scale, respectively. A correlation analysis was performed between interoception, anxiety and depression and between differences between sensibility and accuracy of interoception, anxiety and depression. We revealed that interoceptive sensibility and differences between sensibility and accuracy of interoception were associated with anxiety. Based on results of this pilot study, it is necessary to investigate using longitudinal studies whether interoception might be an effective predictor tool for early detection of anxiety during pregnancy and postpartum.

    モンゴル助産師会と日本助産師会はICM Twinning Projectの一環で、モンゴルにおける妊婦肥満に関する共同研究を行った。2017年にモンゴルで初めての助産師と褥婦を対象とした全国的質問紙調査を行うために、2016年8月にWorkshop&Seminarに参加したモンゴル助産師会会員96名を対象にプレ質問紙調査を行った。モンゴル助産師の多くは、自国の妊婦肥満とそれに伴う大きな児の出産を認識していた。妊婦健診にてBMIを測定すると回答した助産師が半数のみであり、モンゴルにおける助産業務の検討を行う必要性が示唆された。

    The Japanese Midwives Association (JMA) and Mongolian Midwives Association (MMA) began a collaborative research (ICM Twinning Project) with active research starting in 2016, to clarify the Mongolian midwives’ & mothers’ recognition of obesity during pregnancy, and to consider methods for supporting their healthcare. In 2018 we conducted the 2nd National Consciousness Survey.
    The proportion of mothers receiving nutritional guidance increased, with the pregnant women's obesity prevention pamphlet & the App used at high rates. However, the proportion of those unconcerned about weight gain has not changed, and it is necessary to examine specific midwifery support methods to promote maternal behavioral change in the future.

    モンゴル助産師の妊婦肥満に対する認識を明らかにするために、モンゴルで初めてのモンゴル助産師を対象とした全国的質問紙調査を行った。結果、モンゴル助産師の多くは自国の妊婦肥満とそれに伴う大きな児の出産を認識していた。また、ウランバートル市内の助産師と、他21県の助産師の認識の差を調査し、市内の方が日常的な運動不足により妊婦肥満が増加している可能性と、妊婦肥満に関する情報を受ける機会が多いため問題意識が高い可能性が示唆された。

    本研究ではバングラデシュの実証サイトにおいて周産期・母性期・乳児期まで対応する改良母子遠隔健診システムを構築・実装化する。そして、母子に継続ケアが提供されることで、バングラデシュにおける母子の疾病罹患率や死亡率が改善するかを明らかにすることを目的とする。
    クラスターランダム化比較試験により、妊婦とその児を対象とし、遠隔での継続的な健診を実施する。研究のアウトカムとして、産前産後ケアの受診者率、健康状態の改善率、産後うつ率、妊娠・新生児合併症罹患率、児の栄養不良率とする。

    妊娠糖尿病は、器質的因子と生活因子の両者の影響により発症し、様々な周産期合併症を引き起こす。しかしながら、妊娠糖尿病を発症した妊婦に対し、食事や運動といった生活因子の自己管理を支援することで、周産期合併症が減少することが知られている。糖尿病に関する一般的な生活行動の支援は無論重要だが、妊娠糖尿病妊婦により効果的な生活支援を行うためには、個々人の生活を起点に考える必要がある。そこで、本研究では1)妊娠糖尿病発症の要因となる生活因子を明らかにすること、2)妊娠糖尿病の周産期合併症発症に関連する生活因子を明らかにすることを目的とする。
    2022年度にリクルート終了し分析・論文執筆予定であったが、新型コロナウイルス感染症感染拡大第7波の影響で再度リクルートを中断した。2023年度は分析・論文執筆を進める。
