    Post-transplant diabetes mellitus (PTDM) affects long-term outcomes in liver transplant recipients (LTRs). While prolonged sedentary time and low physical activity (PA) increase diabetes risk, their association with PTDM among LTRs remains unclear. We investigated the relationship between sedentary behavior, PA, and PTDM while considering their interdependence. We analyzed data from 68 living donor LTRs at a single hospital. Participants wore an accelerometer on their waist for 7 consecutive days to measure sedentary time and PA. PTDM was identified from medical records, and its association with sedentary time was assessed using logistic regression. Isotemporal substitution analysis estimated the effect of substituting sedentary time with PA. The participants’ mean age was 63.4 ± 10.5 years, with a mean time since transplant of 9.0 ± 6.7 years. PTDM was present in 54% of participants, and the odds ratio for PTDM associated with ≥8 h of sedentary time was 1.60 (95% confidence interval [CI], 1.15–1.78, p = 0.015). Replacing 30 min of sedentary time with light-intensity PA lowered the odds to 0.81 (95% CI, 0.70–0.92, p = 0.001), whereas replacing it with moderate-to-vigorous PA reduced the odds to 0.28 (95% CI, 0.09–0.69, p = 0.002). Reducing sedentary time and increasing PA may contribute to the prevention of PTDM in LTRs.

    Background: Physical activity (PA) is beneficial for stroke prevention; in particular, moderate-to-vigorous physical activity (MVPA). However, few studies have investigated its relationship with recurrent ischemic stroke (RIS). Objectives: To clarify the relationship between MVPA and RIS and the burden of risk factors after a first-ever ischemic stroke. Methods: A total of 45 outpatients (mean age 67.1 ± 10.2 years) who had previously experienced a transient ischemic attack or a minor non-cardioembolic ischemic stroke at a single hospital in Japan (mean 6.4 ± 4.2 years previously), were enrolled between March and June 2016. All patients wore an accelerometer around their hips for 10 days, and their percentage body fat (%BF) and visceral fat level (VFL) were measured by bioelectrical impedance. Retrospective information about the history of RIS and risk factors (blood pressure, lipoprotein cholesterol and estimated glomerular filtration rate) were extracted from the clinical records. Binary logistic regression models were used to estimate the relevance of the RIS history to MVPA and potential risk factors such as sociodemographic and clinical variables (obesity, smoking and hypertension). Results: RIS occurred in 9 patients; they had significantly higher VFL (p = 0.007) and %BF (p = 0.007) values and lower MVPA (p = 0.011) values than patients without recurrence. A multivariate analysis of these factors indicated that age, VFL and MVPA were significant independent predictors of RIS. Conclusions: Patients with a history of mild ischemic stroke had low MVPA and high VFL values, which together may be a risk factor for RIS.

    <p>This study aimed to assess whether patients with type 2 diabetes mellitus (T2DM) meet the recommended time of moderate-to-vigorous physical activity (MVPA) and to identify clinical and behavioral factors associated with MVPA. In this cross-sectional study of 74 outpatients with T2DM, physical activity (PA) was assessed using accelerometers and self-management behaviors were measured using a validated questionnaire. Overall, 58.1% of participants achieved the recommended MVPA time (≥ 150 minutes/week) and were classified as the Active group; the remaining participants formed the Inactive group. Participants in the Active group demonstrated significantly higher daily step counts and total physical activity than the Inactive group. In addition, self-management behaviors related to commuting, activity planning, and deliberate exertion in daily routines were more prevalent among those meeting MVPA recommendations. Hierarchical multiple regression analysis identified body mass index (BMI) and habitual active behavior as significant factors associated with MVPA time. The Active group had lower BMI and triglyceride levels and higher high-density lipoprotein cholesterol than the Inactive group, supporting the metabolic benefits of MVPA. Participants employed strategies to increase PA, such as taking more steps during commuting, selecting suitable times and places for activity, and increasing exertion in routine movements. These findings underscore the importance of cultivating habitual PA behaviors to achieve and maintain recommended MVPA times. As PA was measured over a 7-day period, seasonal or temporal variations may have influenced the results. Future studies should explore long-term, sustainable strategies to integrate PA into daily life and support diabetes management.</p>

    中等度以上の身体活動の評価尺度であるShort Questionnaire to Assess Health-Enhancing Physical Activity(SQUASH)に座位時間と軽度の身体活動の項目を加えたSQUASH改訂版(LPA-SQUASH)を作成し、肝移植後の患者における信頼性と妥当性を評価した。まず、肝移植後の外来患者20例を対象としたパイロット研究においてエキスパートパネルがLPA-SQUASHの内容を検証した。本研究では肝移植後の外来患者173例がLPA-SQUASHへの回答を完了した(信頼性106例、妥当性71例)。再テスト信頼性の検討における相関係数は0.49～0.58、級内相関係数はレジャーの項目を除き0.72～0.80であった。基準関連妥当性の検討において、活動量計のデータとLPA-SQUASHの総身体活動量および軽度の身体活動時間は中等度の相関を示した。LPA-SQUASHは許容可能な妥当性と信頼性を有することが示された。

    臓器移植後患者は免疫抑制や筋力低下の影響を受け、座位中心のライフスタイルとなる傾向があり、糖尿病発症の要因となっている。しかしその問題意識は低く、臓器移植後の身体活動増進のための患者教育プログラムが不足している。近年、mHealthデバイスによる身体活動の増進効果が報告されているが、臓器移植後にmHealthデバイスを活用した身体活動介入は見当たらない。本研究では臓器移植後患者にmHealthデバイスを用いた身体活動増進プログラムを実施し、座位時間の短縮、身体活動の増加、HbA1cの改善があるかを検証する。

    肝移植後の長期生存が可能となった現在、糖尿病、高血圧、脂質異常症などの代謝性疾患（Post-Transplant Metabolic Syndrome：PTMS）が新たな健康課題となっている。PTMSの主な要因は免疫抑制剤であるが、患者が移植後に座位中心のライフスタイルとなる事もPTMSの要因と考えられている。本研究は、移植患者の座位時間と低強度活動を測定できる身体活動調査票を開発し、さらに座位中心のライフスタイルを改善するための患者教育プログラムを開発し、その有用性を明らかにする。本プログラムにより、移植後患者の主要な問題であるPTMS、不眠、QOLの改善が期待できる。

    病院看護師の倫理研修の実態について調査・検討した。病院看護師の倫理研修、道徳的感受性、倫理的行動等の質問紙調査を実施し、約1,000名の看護師から回答を得て記述統計、検定、相関、因子分析、重回帰分析、多重比較等を行った。倫理研修は必要が90％以上だが、参加希望は77％だった。希望として、時間は60分、内容は講義と事例検討が最も多かった。
    併せて倫理研修担当者7名に面接調査を行い、分析を行った結果、倫理研修の課題は、研修時間の確保、目前のテーマ優先があり、研修への要望は、身近な倫理事例の検討だった。評価は、評価者、評価項目、評価方法等について各施設で様々な取組みがあり更に検討が必要である。

    A total of 45 outpatients (mean age 67.1 ± 10.2 years) who had previously experienced a transient ischemic attack or a minor non-cardioembolic ischemic stroke at a single hospital in Japan. RIS occurred in 9 patients; they had significantly higher visceral fat level (p = 0.007) and %body fat (p = 0.007) values and lower MVPA (p = 0.011) values than patients without recurrence. A multivariate analysis of these factors indicated that age, visceral fat level and MVPA were significant independent predictors of RIS.
