    小児病棟看護師が行う小児がん患者の復学に向けた支援の実施状況と課題を明らかにすることを目的とし、15歳以下の小児がん患者への看護経験がある小児病棟看護師136名から得られた無記名式質問紙の回答を分析した。実施状況は設問項目ごとに記述統計量を求め、自由回答で求めた支援の課題は意味の類似性によって分類した。結果、晩期合併症の説明を子どもへ毎回実施している看護師は32.4%で全項目中最も少なかった。課題では、【小児がん患者や家族に復学を前向きにとらえてもらうことが難しい】、【学校側のスムーズな復学のための情報・体制・対応が不足している】、【医療従事者としての入院から退院後の継続した復学支援が難しい】の3カテゴリーが抽出された。看護師は必要な復学支援をしていたが、課題を感じていることも明らかとなった。今後は、復学支援にかかわるすべての人がそれぞれの役割を認識し、スムーズな支援につながるツールの作成が必要である。(著者抄録)

    Background/Objectives: This study aimed to identify the P-bottle feeding techniques systematically organized by Ms. Kumagai, an expert in nursing care for children with a cleft lip and/or palate (CLP), which were developed as she gained expertise in feeding affected children. Methods: We recruited three nurses who had mastered the Kumagai method for feeding with a P-bottle. Through analysis of participants’ voices and videos during interviews, we focused on aspects such as dealing with a closed mouth, inserting the nipple in cases of unilateral and bilateral CLP, dealing with the child’s movements after insertion, and key considerations when squeezing the bottle. Results: The interview analyses revealed numerous techniques used by nurses to manage the difficulties encountered while feeding children, ensuring successful provision of nourishment. Specifically, the nurses employed techniques such as placing the nipple along the midline of the child’s tongue and varying the application of force on the nipple depending on the cleft type. The nurses reported that the objectives of these techniques were to prevent ulcer formation and encourage the use of the tongue, simulating original feeding movements. Conclusions: We explored feeding techniques and the management of associated challenges. Our results suggest that the “Kumagai Method” could be valuable in improving feeding practices.

  • Nurses' Techniques for Bottle-Feeding of Infants with Feeding Difficulties: A Qualitative Descriptive Study

    Background/Objectives: This study identified bottle-feeding techniques for infants with feeding difficulties. Methods: Between December 2021 and April 2022, a survey was distributed to nurses with at least 5 years of experience in caring for infants at 1109 hospitals in Japan. The questionnaire included open-ended questions regarding preparation before bottle-feeding, methods of nipple insertion, methods of assisting with sucking, and criteria for continuing bottle-feeding. The responses were classified according to semantic similarity. Results: In total, 514 valid responses were received. The respondents had an average of 19.16 years of experience as a nurse or midwife. The most frequently used nipples for infants with feeding difficulties were the Combi Breastfeeding Model, Pigeon Weak Sucking Nipple, and Bean Stalk Nipple. Preparation before bottle-feeding consisted of six categories, including determining the timing of feeding and stimulation to promote wakeup. Nipple insertion methods consisted of four categories, such as assisted opening of the mouth, nipple insertion, and nipple insertion depth. Methods of assisting with sucking were divided into four categories, including encouraging sucking ability from around the mouth and matching the infant’s sucking pace. The criteria for continuing bottle-feeding spanned three categories, including willingness to suck and ensuring necessary nutrition. Conclusions: The variety of survey responses describes the current state of bottle-feeding technologies. A combination of several techniques indicated that feeding strategies may lead to effective and appropriate feeding. The nurses continued feeding based on an assessment of the infant’s acceptance. Future studies regarding bottle-feeding techniques must consider the individuality of each infant.

  • Effectiveness of Watching a Kumagai Method Video for Long-Nipple Bottle-Feeding for Children with Cleft Lip and Palate: A Pilot Experimental Before-After Trial Study

    Aim: This study aimed to determine whether the Kumagai method could be followed by watching an instructional video and to compare the feeding actions of specialists and the general population. Materials and Methods: Eleven adults from diverse backgrounds were recruited and asked to feed a baby doll using a bottle feeder with a long nipple, and their actions were recorded. Then, they watched a 2.5-min instructional video of the Kumagai method and were asked to bottle-feed again. Video recordings of the participants were used to evaluate whether their actions followed the Kumagai method. Differences in the feeding actions of the specialists and participants were determined from still images in the videos. The McNemar test was used to analyze the differences between the participants’ actions before and after. The Kruskal–Wallis test was also used to determine differences between the actions of participants and specialists. Results: Feeding movements significantly improved after watching the video. Participants’ actions, which differed significantly from those of the specialists, were evident in “the angle of the nipple at the moment the nipple was brought close to the lips” and “the angle change of the nipple from the lip to the mouth”. Conclusions: Watching our short instructional video was effective in improving the technique of the Kumagai method; however, it is insufficient for feeding a real child with cleft lip and palate. Including the rationale of each action in the video and adding direct instruction as a supplement is required.

  • Association between organizational justice and serious psychological distress among hospital nursing staff during the COVID-19 pandemic: A cross-sectional study

    Aim: Amidst the COVID-19 pandemic, the association between organizational justice and psychological distress among hospital nursing staff is underexplored. Thus, this cross-sectional study, conducted in Fukuoka Prefecture, Japan, examined the relationship between organizational justice and serious psychological distress (SPD) among hospital nursing staff during COVID-19. Methods: The study surveyed 783 hospital nursing staff using the Organizational Justice Questionnaire and Effort-Reward Imbalance Questionnaire. The Kessler K6 scale was used to measure SPD. Sociodemographic and occupational characteristics were controlled for as potential confounders. Results: The prevalence of SPD was 14.4%, with a mean K6 score of 6.5. Moderate procedural justice (odds ratio [OR] = 2.38, 95% confidence interval [CI] = 1.14–4.94, p =.021) and low distributive justice (effort-reward imbalance) (OR = 3.66, 95% CI = 2.01–6.67, p <.001) were associated with SPD, even after adjustment for confounders. Interactional justice showed significance only in the crude model. Effort-reward imbalance had the strongest association with SPD. Conclusions: The findings showed that moderate procedural justice and low distributive justice were associated with SPD, highlighting the need for organizational interventions to address these factors. Imbalances in effort/reward had the greatest impact, highlighting the critical role of distributive justice in mental health. Thus, in the context of a pandemic, extreme procedural justice is not necessarily associated with mental health, and efforts to ensure distributive justice are critical to improving the mental health of hospital nursing staff. Moreover, organizational stressors should be addressed during disruptive conditions such as infectious disease outbreaks.

    COVID-19パンデミック期間中の病院看護スタッフにおいて、組織的公正と重度の精神的苦痛(SPD)との関連性について横断研究で調査した。2021年8月~10月までの期間内に、福岡県内で病床数300床以上の医療施設に勤務する看護スタッフのうち8施設から約5000例の看護スタッフを対象に、Organizational Justice Questionnaire and Effort-Reward Imbalance Questionnaireを用いて調査した。有効回答が得られた783例(男性44例、女性739例)のデータを用いてKessler K6スケールからSPDを調べ、社会人口学的および職業的特徴を潜在的交絡因子として調整した。その結果、SPDの有病率は14.4%で、K6スコアの平均は6.5スコアであり、中等度の手続き公正性と低い分配公正は交絡因子調節後もSPDと関連することが明らかにされた。また、相互作用公正性はcrudeモデルにのみ有意で、努力-報酬不均衡がSPDと最も強く関連していることが確認された。以上の調査結果から、中等度の手続き公正と低い分配的公正がSPDと関連することから、これらの因子に対処するためには組織的介入が必要と考えられた。

    We aimed to identify the steps involved in the Kumagai method—an experimental nursing procedure to feed children with cleft lip and/or palate, using a feeder with a long nipple. We conducted a descriptive study, enrolling five specialist nurses who have mastered the Kumagai method. Their approaches were examined using structured interviews. Moreover, the participants were asked to perform the sequence of actions involved in this method while describing each step. Therefore, we were able to explore the Kumagai method in depth and step-by-step, including the following aspects: correct infant posture; correct feeding bottle holding position; nipple insertion into the child’s mouth; and feeding process initiation, maintenance, and termination. Each step comprises several clinically relevant aspects aimed at encouraging the infant to suck with a closed mouth and stimulating chokubo-zui, i.e., simulation of the natural tongue movement during breastfeeding in children without a cleft palate. In conclusion, when performed correctly, the Kumagai method improves feeding efficiency in children with cleft lip and/or palate. Feeders with long nipples are rarely used in clinical practice; the Kumagai method might popularize their use, thereby improving the management of feeding practices for children with cleft lip and/or palate.

    <p> This study aimed to understand the level of nursing support available to pediatric patients with cancer, and their families, when returning to school following discharge from a pediatric ward. An anonymous questionnaire survey was conducted. Participants included nurses who worked in pediatric wards and had experience caring for pediatric patients with cancer aged <15 years. Qualitative data were categorized into similar groups based on the inferred meaning obtained from the survey response of participants discussing problems in the support available to pediatric patients returning to school. A total of 136 participants were included in this study. Participant responses revealed that 32.4% of the nurses explained late complications to the children every time they were treated, which was the lowest among all items in the practice status. In addition, the following challenges were found regarding supporting pediatric patients with cancer returning to school: difficulty in enabling affected children and their families to view return to school positively; a paucity of information, system, and support from schools to ensure a smooth transition for these patients returning to school; and difficulty for medical professionals in providing continued support during the return-to-school period following patient hospitalization and their subsequent discharge. Although nurses provided the necessary supports for returning to school to the patient and family, they encountered some challenges. It is necessary to create a tool that can be used by all medical professionals involved in supporting pediatric patients with cancer returning to school to achieve seamless support.</p>

    目的:小児病棟看護師が検査・処置時の看護において,子どもの権利擁護のために実践している看護実践プロセスを明らかにする.方法:小児病棟経験5年目以上の看護師20名に対し,子どもの権利擁護の場面について半構造化面接調査を行い,グラウンデッド・セオリー・アプローチを用いて分析した.結果:《子どもが検査・処置に取り組めそうかの見極め》をしながら【子どもが検査・処置になじめるケアの働きかけ】を行っていた.《恐怖心を減らす関わり》や《頑張ろうと揺れる気持ちを待つ》ようにし,子どもが納得して処置に取り組めるようになり[権利擁護できた関わり]となった.業務の都合や看護師の捉え方により[権利擁護できなかった関わり]となる場合もあった.結論:看護師は検査・処置時の状況を多角的に捉えて子どもができそうか見極め,子どもが主体的に検査・処置に取り組みなじめるように看護実践しているプロセスが明らかとなった.(著者抄録)

    <p><b>Purpose:</b> To clarify how pediatric nurses recognize the protection of children’s rights during medical examinations and procedures and how they practice nursing for the protection of children’s rights.</p><p><b>Methods:</b> A semi-structured interview was conducted with 20 nurses with five years of experience regarding the practice of protecting children’s rights. Data were analyzed using the grounded theory approach.</p><p><b>Results:</b> The nurses “assessed whether the child was able to undergo the procedure”, and “encouraged care that the child would be comfortable with.” The nurses tried to “reduce the child’s fear” and “wait for the child’s wavering feelings of doing their best,” which led to “involvement that was able to protect the child’s rights.” In some cases, in which the nurses were not able to protect the child’s rights, depending on how the nurses view the situation.</p><p><b>Conclusion:</b> During nursing care, the nurses view the child and the situation from various perspectives and adjust to the environment; they respect the child’s independence while also pursuing their best interests and adjusting to the situation.</p>

    看護師は子どもの予防接種時に抑制すべきか否かの判断に迫られるが、その明確な基準はない。本研究では、①:文献レビューし、看護師が抑制を判断するための影響要因について明らかにする、②:面接調査・行動観察調査で子どもの行動パターンと看護師が行う抑制方法のパターン、その看護師の抑制の判断の意図を明らかにする、③:子どものパターンに応じた看護師の抑制する基準を明らかにする、④子どもの予防接種時の抑制経験に対する否定的・好意的感情と医療者への要望を明らかにし、子どもの最善の利益を最大限配慮しつつ行動や環境による抑制の必要性を数的な臨床推論のプロセスを構築することを目的とする。
