Oral function (OF) decline in older people is associated with nutritional deficiencies, which increases frailty risk and the need for nursing care. We investigated whether the delivery of an oral function improvement program on a tablet device was as effective as delivery through a paper-based program. We also investigated the association between tongue pressure (TP) improvement and nutritional status at the baseline. The participants involved in the study were 26 community-dwelling older people with low TP, <30 kPa, aged ≥65 years, who were enrolled in a randomized controlled trial for a month in Itoshima City, Fukuoka, Japan. Oral and physical functions and body composition were measured at the baseline and at follow-up. Two-way analysis of variance revealed that body mass index (p = 0.004) increased, and maximum masticatory performance (p = 0.010), maximum TP (p = 0.035), and oral diadochokinesis /pa/ and /ka/ (p = 0.009 and 0.017, respectively) improved in a month. Participants with higher TP improvement showed an increased intake of animal proteins at the baseline: fish (p = 0.022), meat (p = 0.029), and egg (p = 0.009). OF exercises for improving TP were associated with higher animal protein intake at the baseline. This study has been registered with the UMIN Clinical Trials Registry (UMIN 000050292).

Introduction: It is known that oral frailty is one of the risk factors for physical frailty. Therefore, early detection, appropriate treatment, and prevention of oral frailty are really important. Tongue lifting exercise has been identified as a well-known method for improving decreased tongue pressure, one of the factors for oral frailty. However, few reports have investigated how tongue-strengthening exercises affect physical function and body composition. The aim of this study was to investigate the effects of isometric tongue lifting exercises on oral function, physical function, and body composition. Methods: Participants were 49 elderly people aged 68-79 years, who had previously participated in the "Itoshima Frail Study."Participants performed isometric tongue lift exercises for 3 months. Oral function (tongue pressure and oral diadocokinesis), physical function (grip strength, open-eyed one-leg standing, sit-to-stand motion time, 5-m gait speed, and 3-m Timed up and go [TUG]), and body composition were measured at baseline and post-intervention, and the extent of changes in each item was statistically analyzed. Furthermore, participants were divided into physical frailty/pre-frailty and robust groups based on the Japanese version of the frail scale proposed by [BMC Geriatr. 2015 Apr;15:36] and were compared in terms of the extent of changes in each item baseline and the post-intervention. Results: After the intervention, oral function increased significantly together with a significant improvement in physical function, open-eyed one-leg standing time, sit-to-stand motion, and 3-m TUG. For body composition, visceral fat level and basal metabolic rate decreased significantly. Although no significant change in body composition was observed in the physical frailty/pre-frailty group after the intervention, significant improvements in several items were observed in the robust group. Conclusion: Isometric tongue lifting exercise can effectively improve oral function. Furthermore, it might affect physical function and body composition.

Background/Objectives: Undernutrition increases the mortality risk in older persons requiring long-term care; further, it is associated with oral functions such as swallowing and chewing. Moreover, occlusion affects oral function and is crucially involved in nutritional intake. The present study aimed to examine the association between the number of molar occlusions and undernutrition according to body mass index (BMI) in older persons requiring long-term care. Methods: Japanese older persons requiring long-term care were categorized based on BMI (<20 kg/m<sup>2</sup> vs. 20 kg/m<sup>2</sup>). We examined the association between undernutrition and the number of molar occlusions (one in each of the left and right premolars and molars, for a total of four). Results: Among 893 included participants, 440 (49.3%) had BMI < 20 kg/m<sup>2</sup> and 453 (50.7%) had BMI > 20 kg/m<sup>2</sup>. Binomial logistic regression analysis revealed that BMI < 20 kg/m<sup>2</sup> was significantly associated with increased number of molar occlusions (odds ratio: 0.52–0.70, 95% CI: 0.28–1.00). This indicated that a decrease in the number of molar occlusions was associated with malnutrition as determined through BMI in older persons requiring long-term care and residing in long-term care insurance facilities in Japan. Conclusions: Our findings suggest that maintaining occlusal support may help maintain nutritional status in older persons requiring long-term care.

<p> Japan has one of the worldʼs highest rates of population aging. Therefore, it is necessary for dentists to acquire a broad understanding of older adults. They are also required to learn to interact with older adults through gerodontology clinical practice and homebound dentistry practice. However, several students could not receive such training due to the COVID-19 pandemic. Therefore, this study aimed to investigate changes in attitudes toward older adults and the self-assessment of knowledge in dentistry among older adults before and after clinical practice in gerodontology from 2019 to 2021. The study used a questionnaire on required competencies, consisting of behavioral and achievement goals for older adults and the Japanese version of the Attitudes Towards the Elderly（ATE）, which represents emotional attitudes toward older adults. The results demonstrate that students self-rated their abilities more positively after clinical practice and homebound dentistry practice than before（p<0.01）. Attitudes toward older adults also changed more favorably（p=0.012）. However, when comparing the amount of change in each survey item based on experience of home-based dentistry practice and the duration of interruption of clinical practice, no statistically significant differences were observed between the groups. This study showed changes in attitudes toward older adults and self-evaluation of required competencies through clinical practice. The evaluation of changes in attitudes toward older adults and self-evaluation of knowledge in gerodontology may help to confirm the effectiveness of training in gerodontology and to improve the content of future education.</p>

我が国は超高齢社会であり,歯科医師は高齢者に対する幅広い知識と患者に合わせた対応が求められており,臨床実習生も,高齢者歯科における外来臨床実習や歯科訪問診療実習を通して,高齢者とのかかわり方を学んでいる。現在,歯科臨床実習が臨床実習生に与える影響を調査した研究はほとんどなく,新型コロナウイルス感染症流行による外来臨床実習の中断などの影響に関する報告も少ない。そこで本研究では,2019年度から2021年度において歯学部臨床実習を受けた学生91名を対象に,実習前後における高齢者に対する意識および高齢者歯科医学における知識についての自己評価の変化を調査することを目的とした。調査には,高齢者に対する行動目標や到達目標の指標となる項目が示された,要求される能力に関する質問紙と高齢者に対する感情的な態度を測定する日本語版Kogan's Scale of Attitudes Towards the Elderlyを用いた。対象学生はすべての実習修了時,みずからの能力の上昇に肯定的な評価を認めた(p<0.01)。高齢者に対する態度についても,肯定的に変化していた(p=0.012)。実習前後の各調査項目における変化量について,歯科訪問診療実習の経験の有無および外来臨床実習の中断期間別に比較したところ,群間に統計学的な有意差は認められなかった。本研究により,臨床実習を通じて,高齢者に対する態度や要求される能力に対する自己評価に変化が認められた。高齢者に対する意識および高齢者歯科医学における知識についての自己評価の変化を評価することは,高齢者歯科医学の学修効果の確認や,今後の教育内容を向上するための一助となる可能性があると思われる。(著者抄録)

Background: Isaacs’ syndrome, also known as neuromyotonia or peripheral nerve hyperexcitability, is a rare disorder that affects the peripheral nervous system. Clinical findings include cramps, fasciculations, and myokymia; however, there are few reports of dental treatment for trismus. Case presentation: A patient with trismus due to Isaacs’ syndrome experienced swelling and pain in the gingiva surrounding his right lower first molar. He was diagnosed with chronic apical periodontitis by a dentist near his home. However, the patient was informed that dental treatment and medication could not be administered because of the presence of Isaacs’ syndrome, and he visited the Geriatric Dentistry and Perioperative Oral Care Center at Kyushu University Hospital 2 weeks later. The patient’s painless mouth-opening distance (between incisors) was 20 mm at that time, and medication, including amoxicillin capsules and acetaminophen, was administered because the dental extraction forceps or endodontic instruments were difficult to insert into the oral cavity for treatment. Two months after his initial visit, the patient visited us complaining of pain in the same area. However, he had recently undergone plasmapheresis treatment in neurology to alleviate limited mouth opening and systemic myalgia, resulting in a pain-free mouth-opening distance of approximately 35 mm. During this temporary period in which he had no restriction in mouth opening, we performed tooth extraction and bridge restoration on the mandibular right first molar and created an oral appliance for sleep bruxism. Conclusions: Plasmapheresis therapy transiently reduced trismus, rendering dental interventions feasible, albeit temporarily. This case report underscores the importance of close collaboration between neurologists and dentists who encounter similar cases while furnishing valuable insights to inform dental treatment planning.

Neurotoxic microglia-provoked neuroinflammation is implicated in cognitive decline in Alzheimer's disease (AD). Supplementation with Ginkgo biloba, phosphatidylserine, Curcuma longa, and propolis is reported to improve the cognitive functions of elderly people; however, the underlying mechanisms of this combination of natural ingredients are unknown. We investigated the effects of a mixture of extracts from propolis, Coffea arabica, Gotu kola, phosphatidylserine, Ginkgo biloba, and Curcuma longa (mixture) on microglia polarization after exposure to amyloid β1-42 (Aβ1-42, 1 μM) and lipopolysaccharide from Porphyromonas gingivalis (PgLPS, 1 μg/mL), using MG6 and BV2 microglial cells. Exposure to Aβ1-42 and PgLPS (AL) raised the mRNA expression of IL-1β, TNF-α, and IL-6, nuclear translocation of p65 NF-κB in MG6 cells and BV2 cells, and mitochondrial reactive oxygen species (ROS) production in MG6 cells. The mixture dramatically suppressed the mRNA expression of IL-1β, TNF-α, and IL-6, but significantly promoted that of IL-10, TGFβ1, and BDNF in AL-exposed MG6 and BV2 cells. Furthermore, the mixture significantly suppressed the nuclear translocation of p65 NF-κB but significantly promoted that of NF-E2-related factor 2 (Nrf2) in AL-exposed MG6 and BV2 cells. Furthermore, the mixture significantly ameliorated mitochondrial ROS production but increased mitochondrial membrane potential in MG6 cells. These observations strongly suggest that the mixture demotes the neuropathic polarization of microglia by modulating NF-κB/Nrf2 activation and improving mitochondrial functions. This study supplies the potential mechanisms of the efficacy of a combination of natural ingredients that can be applied in the prevention of cognitive decline in AD and aging by targeting microglia-mediated neuroinflammation.

Purpose: Oral cryotherapy is an effective method to prevent oral mucositis (OM) induced by chemotherapeutic agents, such as melphalan (Mel). However, there is limited data about cryotherapy in allogeneic hematopoietic stem cell transplantation (allo-HSCT) recipients; thus, the current study aimed to examine the efficacy of cryotherapy among allo-HSCT recipients treated with Mel-containing regimens. Methods: Medical records of 78 consecutive allo-HSCT recipients were retrospectively analyzed. Baseline characteristics and clinical courses between the patients who received cryotherapy (cryotherapy group, n = 42) and those who did not (control group, n = 36) were compared, especially focusing on methotrexate (MTX) use as a part of graft-versus-host disease (GVHD) prophylaxis. Results: Binary logistic regression analysis revealed that a higher dose of Mel (OR, 3.82; 95%CI, 1.085–13.46; P = 0.037) or MTX use (OR, 7.61; 95% CI, 2.41–23.97; P < 0.001) was associated with the incidence of OM. MTX use was also significantly associated with the duration of OM (β = 0.515; 95% CI, 9.712–21.636; P < 0.001). Among 31 patients without MTX use, cryotherapy was associated with a significant reduction of OM development (0% in the cryotherapy group vs 35% in the control group, P = 0.021). We did not find such an association in 47 patients with MTX use. Conclusion: Cryotherapy was useful to prevent the incidence of OM in allo-HSCT recipients in the cases without MTX for GVHD prophylaxis.

Many older people have restricted activities or movements because of the coronavirus disease 2019 (COVID-19) pandemic, which causes concerns about secondary health problems. This study aimed to investigate how frailty-prevention activities implemented by local governments have changed the health of community-dwelling older people during the COVID-19 pandemic. In this observational study, the participants were 23 older Japanese people who took part in keyboard harmonica or exercise classes in 2021. Oral function examination and physical function tests were conducted at baseline and after 10 months of follow-up. In each class, the participants met 15 times and worked on assignments at home. The results showed that oral diadochokinesis/pa/, which represents lip dexterity, improved during 10 months (from 6.6 to 6.8 times/s, p < 0.046); however, grip strength (p < 0.005) and total skeletal muscle mass (p < 0.017) decreased in the keyboard harmonica group. In the exercise group, a statistically significant difference was found only in grip strength, which decreased (p < 0.003). The oral and physical functions of older people who participated in frailty-prevention activities implemented by local governments characteristically changed. Moreover, activity restrictions during the COVID-19 pandemic may have caused decreased grip strength.

Background: Few studies have examined the relationship between oral functions and the physical pre-frailty status, classified using physical function tests. This cross-sectional study aimed to clarify this association among community-dwelling older people from the Itoshima Frail Study in Itoshima Fukuoka Prefecture. Methods: Of the 1,555 individuals invited to join the study, 381 (188 males and 193 females) enrolled. Their physical pre-frailty was assessed with a classification system consisting of two physical indicators (fatigue and unintentional weight loss, determined with a questionnaire), two functional components (declined walking speed and muscle weakness, determined using a body function measuring instrument), and declined physical activity (examined using a triaxial accelerometer). Subsequently, the individuals were classified into three groups: robust, pre-frailty, and frailty. Along with the number of teeth remaining, oral functions, such as masticatory performance, tongue pressure strength, and oral diadochokinesis (ODK), were examined. Data regarding social activity and exercise habits were collected, and the individuals’ body compositions were measured. Odds ratios (ORs) and 95% confidence intervals (CIs) for the physical pre-frailty were calculated using logistic regression models. Results: In this study, 126 (33%) participants presented with physical pre-frailty. The participants in the robust group were younger, had stronger maximum handgrip strength, and walked faster than those in the physical pre-frailty group (p < 0.001). The robust group presented with better oral functions (masticatory performance, p = 0.015; oral ODK /ta/, p = 0.004). The physical pre-frailty status was significantly associated with age (OR, 1.111; 95% CI, 1.048–1.178; p < 0.001), masticatory performance (OR, 0.819; 95% CI, 0.680–0.986; p = 0.035), low ODK/ta/ (OR, 1.864; 95% CI, 1.069–3.250; p = 0.028), and low social activity (OR, 2.273; 95% CI, 1.308–3.951; p = 0.004). Conclusion: This study indicated that older people with higher age, lower anterior tongue movement, lower masticatory performance, and lower social activity are positively associated with physical pre-frailty.

地域在住高齢者42名(男性23名、女性19名、平均年齢73.1±3.1歳)を対象に、1日3回、1週間に3日以上の実施を指示した舌挙上運動訓練を3ヵ月間実施し、その身体機能や体組成に与える影響を、介入前後の比較から検討した。フレイル/プレフレイル群(11名)と非フレイル群(31名)に分け比較検討した結果、両群で舌圧、オーラルディアドコキネシスといった口腔機能に加え、椅子立ち座り時間、5m歩行速度、3mアップ・アンド・ゴー時間(3mTUG)の身体機能で有意な改善がみられた。一方、非フレイル群では、皮下脂肪率、骨代謝率、基礎代謝量といった体組成においても有意な改善が認められたのに対し、フレイル/プレフレイル群では改善を認めなかった。

An extended method to discover exceptional association rule sets from incomplete databases is proposed. In an exceptional rule set, each itemset X, Y has a weak or no statistical relation to class C; however, the join of X and Y has a strong relation to C. An exceptional rule set can be used to infer long rules for the join of X and Y and to discover rare rules. The proposed method calculates the rule evaluation odds ratio directly. In this study, the method is applied to rule discovery using a database of community-dwelling elderly. Experimental results demonstrate that the proposed method can help discover interesting rare rules and exceptional association rule sets. The results show the effectiveness of the proposed method in the fields of medicine and health care.

Some elderly patients who receive home-visit dental treatment do not use their newly fabricated dentures. The aim of this study was to investigate factors associated with non-use of newly fabricated dentures among elderly Japanese patients during home-visit dental treatment.</p><p> In total, 108 patients received newly fabricated dentures in a private practice in Kumamoto prefecture from August 2012 to July 2013. The usage situation of the new dentures was recorded. Additional information obtained from medical records included age, sex, medical history, care need certification, oral state by Eichner's classification, and experience of wearing dentures prior to the manufacture of new dentures, which were compared between the denture use and denture non-use groups after the newly fabricated dentures were divided into upper jaw（n＝88）or lower jaw（n＝83）categories. Logistic regression analysis was performed with non-use of the newly fabricated dentures as a dependent variable and age, sex, experience of wearing dentures and care need certification as independent variables.</p><p> Overall, 7 subjects（8.0％）did not use the newly fabricated dentures in the upper jaw and 7 subjects（8.4％）did not use them in the lower jaw. Among those who did not use the newly fabricated dentures, the percentage of subjects with no denture-wearing experience was 71.4％ in the upper jaw and 85.7％ in the lower jaw groups. Logistic regression analysis showed that no experience of wearing dentures was associated with non-use of the newly fabricated dentures during home-visit dental treatment after adjusting for age, sex, experience of wearing dentures and care need certification.</p><p> These results suggest that no experience of wearing dentures is associated with a risk of non-use of newly fabricated dentures among elderly Japanese patients during home-visit dental treatment.

Objective: Many cross-sectional studies have reported a relationship between overweight/obesity and eating quickly, but there have been few longitudinal studies to address this relationship in younger populations. The purpose of this prospective longitudinal study was to investigate whether eating quickly was related to being overweight in Japanese university students. Methods: Of 1,396 students who underwent a general examination and completed questionnaires at the start of university and before graduation, 1,314 students (676 male and 638 female) of normal body composition [body mass index (BMI) <25 kg m-2] at baseline were included in the analysis. The questionnaires included speed of eating and other lifestyle factors. After a 3-year follow-up, the students whose BMIs were ≥25 kg m-2were defined as overweight. Results: In this study, 38 participants (2.9%) became overweight. In the logistic regression analysis, the risk of being overweight was increased in males [adjusted odds ratio (OR): 2.77; 95% confidence interval (CI): 1.33-5.79; P < 0.01] and in those who ate quickly at baseline (OR: 4.40; 95% CI: 2.22-8.75; P < 0.001). Conclusions: Eating quickly may predict risk of being overweight in Japanese university students.

Purpose of Review: Oral functions, which are responsible for feeding and swallowing, play a very important role in obtaining proper nutrition. Therefore, their deterioration could interfere with nutritional ingestion. This review investigated previous reports on the relationship between oral function deterioration and malnutrition. Recent Findings: Reduced occlusal force, decreased tongue–lip motor function, decreased tongue pressure, decreased masticatory function, and deterioration of swallowing function have been found to be associated with malnutrition. On the other hand, consensus does not exist on whether oral dryness is associated with malnutrition. Prosthetic rehabilitation and oral function training are effective interventions for improving oral functions. However, malnutrition could not be addressed by prosthetic rehabilitation alone; it requires simultaneous management with prosthetic rehabilitation and nutritional guidance. Furthermore, it has been reported that data on whether oral function training, such as tongue strengthening training, could improve malnutrition are limited. Summary: Partial deterioration of oral functions can cause malnutrition. Prosthetic rehabilitation and oral function training could be considered interventions for improving malnutrition, but numerous unclarified points about these methods remain. In the future, detailed research should be conducted to elucidate the causal relationship between deterioration of oral functions and malnutrition and to establish oral function interventions for improving malnutrition.

健康寿命延伸の観点から，フレイルや生活習慣病を予防することはわが国における喫緊の課題である。多くの生活習慣病は免疫の乱れに起因する慢性炎症を基盤としているが，慢性炎症を制御する手法は確立されていない。本研究の目的は，データ駆動型に疾患関連因子を探索する新しい疫学手法を用いて，フレイルや生活習慣病に関連する口腔の因子をみいだし，その予防効果を検証することである。口腔からアプローチする新しいフレイル・生活習慣病予防法による健康寿命の延伸を目指す。

From the viewpoint of extending healthy life expectancy, it is an urgent issue in Japan to prevent frailty. Though the improvement of oral frailty seems to make the aid of the frailty prevention, there is no clear evidence. The purpose of this study is to develop a "Oral rail prevention and improvement program" to prevent frailty, to construct a training and guidance system using IoT, and to verify the frailty preventive effect of the new program for local elderly residents. It aims at the establishment of the evidence that the health of the oral cavity is connected with the extension of healthy life.

Oral function decline in older people is associated with nutritional deficiencies, which increases frailty risk and the need for nursing care. This study investigated the association between tongue pressure (TP) improvement and nutritional status at the baseline.
The participants involved in the study were 26 community-dwelling older people with low TP (<30 kPa), who were in Itoshima City, Fukuoka. Oral and physical functions and body composition were measured at the baseline and at follow-up. Two-way analysis of variance revealed that body mass index increased, and maximum masticatory performance, maximum TP, and oral diadochokinesis /pa/ and /ka/ improved in a month. Participants with higher TP improvement showed an increased intake of animal proteins at the baseline: fish, meat, and egg. Oral function exercises for improving TP were associated with higher animal protein intake at the baseline. -
