研究:シェーグレン症候群やIgG4関連疾患といった難治性唾液腺疾患の病態・発症のメカニズムについての研究や、多施設共同での「IgG4関連涙腺・唾液腺炎における顎下腺超音波検査の有用性」や「シェーグレン症候群の診断における口腔水分計の有用性」の検証を行っている。また、近年では口唇裂・口蓋裂の形態評価にも着目し、検討を行っている。 教育:大学院生の研究、研修医の診療、および歯学部学生の臨床実習指導を行っている。特に歯学部学生に関しては、臨床実習ライター長として、臨床予備実習・オスキー・臨床実習・アドバンストオスキーの分野内での統括を担当している。 臨床:九州大学病院顎口腔外科にて口腔外科全般の臨床を行っている。特に昨今は口唇口蓋裂や顎変形症に関して専門的に診療を行っている。2023年度は副病棟医長、2024年度は副外来医長、2025年度は病棟医長として病棟・外来マネージメントも行っている。

    This study was performed to investigate the utility of U-shaped osseous release (USOR) for Le Fort 1 osteotomy as a novel surgical technique for superior repositioning. Thirty-six patients with jaw deformities were divided into 2 groups based on whether or not USOR was adopted (18 in the non-Adoption group and 18 in the adoption group). Maxillary surgical time, blood loss, and discrepancy from the planned amount of movement (anterior-posterior and superior-inferior directions) were compared between the 2 groups. Correlations between these items and the planned amount of superior-inferior movement were also examined. There were no significant differences in mean age, preset displacement, surgical time, blood loss, body mass index, or preset displacement error. However, a significant positive correlation was observed between the planned and actual amount of vertical movement only in the adoption group (P=0.0018). In addition, there was a tendency for the error (downward) to increase as the amount of upward movement increased in the non-Adoption group, but not the adoption group. These findings suggest that USOR may be a useful technique because it can safely and conveniently remove bony interference and can enable guidance to a more precise position, especially in cases with substantial superior movement.

    Objectives: For the diagnosis of immunoglobulin G4 (IgG4)-related dacryoadenitis and sialadenitis, either revised comprehensive diagnostic criteria or organ-specific diagnostic criteria for IgG4-related dacryoadenitis and sialadenitis in 2008 were applied; however, the collected knowledge for IgG4-related dacryoadenitis and sialadenitis required us to revise the criteria for IgG4-related dacryoadenitis and sialadenitis. Methods: The board member of Japanese Study Group for IgG4-related Dacryoadenitis and Sialadenitis revised the diagnostic criteria for IgG4-related dacryoadenitis and sialadenitis. We collected the clinical questions to be revised and performed a review of the literature. When the data were insufficient, additional data collection was performed. After the revision, public comments were collected. Results: The three major points were revised. (1) Asymmetric or under two pairs of dacryoadenitis and sialadenitis were included as IgG4-related dacryoadenitis and sialadenitis. (2) The thresholds of IgG4-positive cell infiltration were adjusted to an IgG4+/IgG+ ratio >0.4 and IgG4+ cells >10 per high power field. (3) The labial salivary gland biopsy was allowed to diagnose IgG4-related dacryoadenitis and sialadenitis Conclusions: The revised diagnostic criteria for IgG4-related dacryoadenitis and sialadenitis solved several issues with the previous criteria. It will improve the early diagnosis of IgG4-related dacryoadenitis and sialadenitis, especially in situations without enough resources for a biopsy.

    Background: Sjögren’s disease (SjD) is an autoimmune disease marked by lymphocytic infiltration of salivary and lacrimal glands, leading to glandular dysfunction, where CD4-positive helper T (Th) cells and their cytokines are crucial in the pathogenesis. Recent studies have demonstrated that Toll-like receptors (TLRs), particularly those recognizing immune complexes containing DNA and RNA, contribute to Th cell activation in various autoimmune diseases. This study explores the expression and function of these TLRs in SjD. Methods: DNA microarray analysis of salivary gland tissue from six SjD patients and real-time PCR (n = 32) was used to identify overexpressed TLRs. Single-cell RNA sequencing (scRNA-seq) was performed using tissue lesions and integrated with published scRNA-seq data from tissues and peripheral blood mononuclear cells to examine gene expression in macrophages and monocytes. Finally, multi-color immunofluorescence staining was conducted to confirm TLR8 expression and function in SjD lesions (n = 19). Results: DNA microarray analysis revealed the up-regulation of TLR8, along with other TLRs and innate immune response genes in SjD. Real-time PCR showed significant up-regulation of TLR7 and TLR8. TLR8 up-regulated in both analyses. In scRNA-seq analysis, the TLR8-expressing cluster comprised macrophages and monocytes, which also produced T cell activation genes like CD86. TLR8-positive macrophages infiltrated inflammatory sites and frequently expressed CD86 in quantitative imaging approaches. Conclusions: These results suggest that infiltrating monocytes and macrophages may produce cytokines and chemokines through TLR8 stimulation, potentially enhancing B7 molecule expression, promoting the adaptive immune response, and contributing to SjD pathogenesis.

    口腔水分計の製品であるムーカスIIIがシェーグレン症候群(SS)診断の助けになるか、唾液分泌量と同時測定することで比較検討した。2019~2021年に当院に紹介されたSS患者40名(女性38名、平均62.3±15.1歳)(罹患群)と健康対照者29名(女性28名、平均43.2±7.9歳)(対照群)を前向きに組み入れた。ムーカスIIIを使い舌背および頬粘膜で計測された湿潤値は、いずれも対照者に対し罹患群で有意に低下していた。唾液分泌量、特に吐唾法で測定された唾液分泌量は、舌背・頬粘膜の両湿潤値と有意に正相関していた。舌背湿潤値が示した診断能の感度は92.5%、特異度は68.9%、正確度は82.6%であり、頬粘膜湿潤値では同順で70.0%、82.8%、73.9%となった。舌背湿潤値は、口腔乾燥症の二次的な自覚症状(口腔疼痛、嚥下障害、味覚障害)の視覚的アナログスケールスコアと中等度に逆相関し、口腔乾燥症に直接関係する自覚症状(口腔乾燥および唾液生成量の減少)とは弱く逆相関していた。口腔水分計のムーカスIIIによる測定は、唾液分泌量の検査結果、および口腔の慢性自覚症状を正確に反映していた。本製品による測定法はSSの診断基準になると考えられた。

    Odontogenic tumors (OGTs), which originate from cells of odontogenic apparatus and their remnants, are rare entities. Primary intraosseous carcinoma NOS (PIOC), is one of the OGTs, but it is even rarer and has a worse prognosis. The precise characteristics of PIOC, especially in immunohistochemical features and its pathogenesis, remain unclear. We characterized a case of PIOC arising from the left mandible, in which histopathological findings showed a transition from the odontogenic keratocyst to the carcinoma. Remarkably, the tumor lesion of this PIOC prominently exhibits malignant attributes, including invasive growth of carcinoma cell infiltration into the bone tissue, an elevated Ki-67 index, and lower signal for CK13 and higher signal for CK17 compared with the non-tumor region, histopathologically and immunohistopathologically. Further immunohistochemical analyses demonstrated increased expression of ADP-ribosylation factor (ARF)-like 4c (ARL4C) (accompanying expression of β-catenin in the nucleus) and yes-associated protein (YAP) in the tumor lesion. On the other hand, YAP was expressed and the expression of ARL4C was hardly detected in the non-tumor region. In addition, quantitative RT-PCR analysis using RNAs and dot blot analysis using genomic DNA showed the activation of Wnt/β-catenin signaling and epigenetic alterations, such as an increase of 5mC levels and a decrease of 5hmC levels, in the tumor lesion. A DNA microarray and a gene set enrichment analysis demonstrated that various types of intracellular signaling would be activated and several kinds of cellular functions would be altered in the pathogenesis of PIOC. Experiments with the GSK-3 inhibitor revealed that β-catenin pathway increased not only mRNA levels of ankyrin repeat domain1 (ANKRD1) but also protein levels of YAP and transcriptional co-activator with PDZ-binding motif (TAZ) in oral squamous cell carcinoma cell lines. These results suggested that further activation of YAP signaling by Wnt/β-catenin signaling may be associated with the pathogenesis of PIOC deriving from odontogenic keratocyst in which YAP signaling is activated.

    Objectives: To investigate the use of an oral moisture meter to aid in the diagnosis of Sjögren's syndrome (SS) by measuring the moisture values and salivary flow rate (SFR) simultaneously. Methods: The SFR and moisture values of the dorsum of the tongue and buccal mucosa were measured in 40 patients with SS and 29 healthy controls using an oral moisture meter. In addition, a visual analogue scale (VAS) was used to determine the degree of subjective oral symptoms (xerostomia, decreased saliva production, drinking during eating, oral pain, dysphagia, and dysgeusia). Results: The moisture values of the dorsum of the tongue and buccal mucosa were significantly lower in SS patients than in healthy subjects (both p < 0.01). SFR, especially when measured by the spitting test, correlated positively with the moisture values of the dorsum of the tongue (p < 0.0001) and buccal mucosa (p < 0.0003). Furthermore, the sensitivity, specificity, and accuracy of the oral moisture meter measurements of the dorsum of the tongue and buccal mucosa were 92.5%, 68.9%, and 82.6%, and 70.0%, 82.8%, and 73.9%, respectively. The moisture values of the dorsum of the tongue had a moderate negative correlation with the VAS scores of secondary subjective oral symptoms of xerostomia (oral pain, dysphagia, and dysgeusia), whereas a weak negative correlation was observed for subjective oral symptoms directly related to xerostomia (xerostomia and decreased saliva production). Conclusions: The oral moisture meter accurately reflected the results of the SFR tests and chronic subjective oral symptoms, and therefore might be a diagnostic criterion for SS.

    Objectives: Consensus has not yet been reached on the therapy for medication-related osteonecrosis of the jaw (MRONJ). We assessed the effectiveness of conservative surgical management, which involves removing the necrotic bone and performing a gingivectomy, followed by localized cleaning to facilitate the separation of the sequestrum. Additionally, we revalidated the treatment algorithm utilized by our department. Methods: We examined 160 patients diagnosed with MRONJ at three hospitals. The data for this study were collected retrospectively. These patients were followed up for a minimum of 2 years. Results: The outcome 2 years after starting conservative surgical management was: cure: 117 cases; improvement: 22 cases; unchanged: 12 cases; and deterioration: 9 cases. The cure rate was 72.3% and 83.0% at 2 and 5 years after therapy initiation, respectively, and significantly lower at age ≥ 74 years compared to age < 74 years (P < 0.001), with malignant tumour as the primary disease when compared to osteoporosis (P < 0.001), and with ≥ 4 years of administration compared to < 4 years (P = 0.0301). On multivariate analysis, the trigger (P < 0.05), primary disease (P = 0.001), and administration period (P < 0.01) were independent prognostic factors. Conclusions: Conservative surgical management is effective for MRONJ regardless of stage, especially in patients with age < 74 years, osteoporosis, and short drug-administration periods, and could become a first-line therapy for all stages.

    Oral lichen planus (OLP) is a chronic inflammatory disease associated with T cell infiltration. The crosstalk between oral epithelium and mucosal T cells was considered to be crucial in the pathogenesis of OLP. Here, we selectively extracted the normal epithelium (NE) and lesional epithelium (LE) of buccal mucosa specimens from three patients with OLP by laser capture microdissection due to identify the pathogenic factors. Cathepsin K (CTSK) was identified as one of common upregulated genes in the LE by DNA microarray. Immunohistochemically, CTSK was distinctly detected in and around the LE, while it was rarely seen in the NE. Recent studies showed that CTSK enhanced Toll-like receptor 9 (TLR9) signaling in antigen-presenting cells, leading to Th17 cell differentiation. TLR9 expression mainly co-localized with CD123+ plasmacytoid dendritic cells (pDCs). The number of RORγt-positive cells correlated with that of CTSK-positive cells in OLP tissues. CD123+ pDCs induced the production of Th17-related cytokines (IL-6, IL-23, and TGF-β) upon stimulation with TLR9 agonist CpG DNA. Moreover, single cell RNA-sequencing analysis revealed that TLR9-positive pDCs enhanced in genes associated with Th17 cell differentiation in comparison with TLR9-negative pDCs. CTSK could induce Th17-related production of CD123+ pDCs via TLR9 signaling to promote the pathogenesis of OLP.

    シェーグレン症候群(SS)は、発症・病態形成にはTh細胞が重要な役割を果たしていることが示唆されている。近年、ステロイドに代わる新規治療薬が炎症性サイトカインを標的に開発されているが、SSに対する効果は限定的である。それはSSの病態形成にTh細胞から産生される多彩なサイトカインが関与しているためだと推察される。最近の報告では免疫システムの発生や恒常性の維持に重要な役割を果たすオートファジーの機能不全が炎症性サイトカインの過剰産生を促すことが報告されている。本研究では、SSにおけるオートファジー関連分子の発現と機能を解析し、サイトカイン産生を制御する上流分子を標的とした新規治療法の確立を目指す。

    IgG4 関連疾患(IgG4-RD)は、高 IgG4 血症や多臓器に IgG4 陽性形質細胞の浸潤と著明な線維化を伴う腫脹を特徴とする本邦から提唱された新しい疾患概念である。 IgG4 関連涙腺・唾液腺炎(IgG4-DS)の診断においては、罹患臓器を全摘出することも多く、唾液分泌機能の低下や顔面神経障害などの合併症が生じるリスクを伴う。
    そこで本研究では、非侵襲性で繰り返し使用できる顎下腺超音波検査に着目し、IgG4-DS の診断および予後判定法の確立を目的に、希少疾患としては国内最大規模の患者レジストリデータを利用した検証を行い、診断への実用化を目指す。

    SSは涙腺・唾液腺が障害される臓器特異的自己免疫疾患であり、病態にはTh細胞が重要な役割を果たしている。炎症性サイトカインを標的とした生物学的製剤が開発され、一部の自己免疫疾患では治療効果が得られているがSSに対する効果は限定的である。それはSSの病態形成にTh細胞由来の多種のサイトカインが関与し、単一のサイトカインを阻害しても病態進展を阻止できないためだと推察される。またオートファジーの異常が炎症性サイトカインの過剰産生を促すことも報告されている。本研究ではSSにおけるオートファジー関連分子の発現・機能を解析し、サイトカイン産生を制御する上流分子を標的とした新規治療薬・診断法の確立を目指す。

  • 九州大学歯学部学生に対して、臨床実習ライター長として5-6年次の臨床予備実習・臨床実習を通して口腔外科領域の知識・手技に関して教育・指導を行っている。講座の大学院生には研究方法を教育し、主に難治性唾液腺疾患(シェーグレン症候群・IgG4関連涙腺・唾液腺炎)や口唇裂・口蓋裂についての研究を指導している。また、2023年度に九州大学病院歯科医師臨床研修指導歯科医資格を取得し、研修医教育にも従事し、学外では2023年度より福岡医健専門学校(歯科衛生士科)、2024年度より九州医療専門学校柔道整復師科の非常勤講師を務めている。
