    Compression on the median nerve can lead to carpal tunnel syndrome (CTS), and median nerve indicators measured from ultrasound images can be used for CTS diagnosis. The aim of this study was to investigate the relationship between grip force modulation and dynamic morphological changes of the median nerve. We used a digital grip dynamometer to measure grip force while simultaneously conducting ultrasound examinations. Ultrasound images were sampled for both the dominant and nondominant hands of all participants (n = 20) during a baseline condition at approximately 0% maximum voluntary force (MVF), during sustained grip force conditions at 25%, 50%, 75%, and 100% MVF, and during the return to a relaxed state (≈0% MVF) directly following each grip force condition. Regardless of hand dominance, grip force level, and grip force modulation, median nerve cross-sectional area (MNCSA) during the grip tasks was smaller relative to the initial baseline condition without grip force. With respect to shape change, the median nerve became more flattened, including increased longitudinal diameter (D1) and decreased vertical diameter (D2), when grip force was relaxed compared to the preceding sustained grip force condition for the dominant hand; however, there were no significant shape changes for the nondominant hand. As morphological changes to tissue result in strain, our results indicate that median nerve injury development may be associated with more hand usage (dominant hand, grip exertion, and grip force modulation), and further suggests the evaluative potential for median nerve dynamics within the carpal tunnel.

    Physical inactivity is a major global public health concern, and table tennis offers a low-impact, engaging way to promote physical activity across various age groups. However, many beginners struggle to maintain effective participation due to their lower skill levels. Therefore, the development and reinforcement of stable grip techniques is crucial because it will help beginners achieve sustainable improvements in performance. This will provide additional opportunities to increase physical activity, and therefore overall health, across all age groups. Thus, in this study, we investigate the effects of a prototype table tennis handle on the racket angle and performance of beginners. The prototype handle features a 20° tilt to assist the player in maintaining a stable topspin forehand grip during play. The participants were randomized into three groups, Groups A, B, and C, which used the prototype handle, standard handle, and practiced with the prototype but performed tests with the standard handle, respectively. The participants executed topspin forehand strokes in approximately 30 min of practice, and data on racket angles, swing mechanics, success rates, and ball landing positions were collected. The results showed that Group A exhibited a larger racket open angle and a smaller racket face Angle than the other groups. However, the groups showed no significant differences in hit positions or overall success rates. Our results suggest that although the prototype handle can influence racket angles and some performance aspects, individual differences and swing mechanics should be considered.

    Cycling is a beneficial physical activity for rehabilitating individuals with lower-limb amputations and serves as a feasible leisure sport. However, the optimal bicycle configuration for cycling with a unilateral transtibial prosthesis at leisure levels has not been investigated. For saddle height at professional cycling levels, existing literature suggests utilizing the same configuration as that used by intact cyclists, where the knee reaches 25–35° at maximum extension. However, leisure cyclists tend to select lower saddle heights, and cycling with a unilateral transtibial prosthesis infers altered biomechanics during cycling practice. This study aimed to investigate the effects of cycling at different saddle heights with a simulated unilateral prosthesis. Ten able-bodied participants wore orthoses to simulate prosthetic conditions. The experimental task was performed on an ergometer at 40 W resistance, 60 rpm to simulate leisure cycling. Standard saddle height was defined as maximum knee extension of 45°. This height was used as the control condition and its trials were performed without orthoses. The variable heights were set as height percentage variations (-7%, -3.5%, 0, +3.5%, and +7%). Muscle activity, joint movement, force application to the pedals, perceived exertion, and comfort were evaluated. The -3.5% and -7% saddle heights resulted in joint movement and muscle activity levels closer to those in the control conditions, which also showed improved power symmetry between the affected and non-affected legs. In addition, the -3.5% height increased comfort level in participants. In conclusion, selecting lower saddle heights may be beneficial for unilateral transtibial amputees during leisure cycling. The optimal saddle height for this population may maintain maximum knee extension within the 37–45° range, dynamically measured on the affected side.

    Background: Although several signals have been proposed to control the activation timing of wearable robots, there is no consensus on the suitability of these signals in terms of their ability to promote effective cooperation and smooth device activation. Objective: This study investigated the effects of biomechanical (grip forces and voice commands), bioelectrical (electromyography [EMG]), and external sound (EX) signals on elbow flexion to trigger a wearable robot arm. Methods: Constant torque control was applied to a single-degree-of-freedom (DOF) wearable robot arm, which operated within a range of 30°–130° of elbow flexion. Activation of the wearable robot arm was initiated using a grip force applied to a handle from either hand, the verbal command “go,” EMG signals, and an EX cue, while lifting a load equivalent to 15% of maximum voluntary contraction (MVC). Results: Prior to initiating the wearable robot arm, the grip force used to activate the movement using the hand on the side of the assisted arm significantly induced activities of the biceps brachii (BB) (%MVCBB) by approximately 6%–10%MVCBB compared to bioelectrical and biomechanical signals. Nonetheless, after assistive forces were generated, the %MVCBB elicited during the initial phase of elbow flexion was similar to other activation signals. In contrast, EMG-induced activation increased the triceps brachii (TB) activity (%MVCTB) by approximately 7.2%MVCTB during elbow flexion, resulting in the highest joint angular acceleration during the initial phase among all signals. Conclusion: These findings suggest that the grip force applied from the assistive hand was the most effective at triggering the wearable robot arm and may enhance muscle preparation and cooperation with the device more effectively.

    Pentosidine is representative of the cross-linked structure of advanced glycation end products (AGEs) and has been suggested as a biomarker to assess bone and muscle quality. As studies on pentosidine in young adult men remain limited, we aimed to clarify the associations of urinary pentosidine with musculoskeletal status and physical performance in young men. Participants in this study comprised 32 men (age range: 19–39 years). Anthropometric measurements (body composition by InBody 430; stiffness index by ultrasound), muscle performance (grip strength by dynamometer, thigh muscle thickness by ultrasound), physical performance (functional reach test, 30-s chair stand test, and timed up and go test), and urinary biomarkers (pentosidine, N-telopeptide of type I collagen, and creatinine) were measured. In partial correlation analysis adjusted for age and height, higher urinary pentosidine levels were significantly associated with lower fat-free mass index (rho = − 0.368, p = 0.046), grip strength (rho = − 0.433, p = 0.017), rectus femoris thickness (rho = − 0.393, p = 0.032), and anterior thigh thickness (rho = − 0.416, p = 0.022), and a marginally inverse correlation was noted between urinary pentosidine levels and functional reach test (rho = − 0.327, p = 0.078). Our findings suggest that pentosidine correlates inversely with a few muscle and physical performance indicators. Pending future validations, urinary pentosidine may be a biomarker of AGEs in young men.

    One way to provide assistance in a dynamic lifting task is to pre-emptively move the exoskeleton based on a predicted reference trajectory. However, the level of aggressiveness in the prediction (i.e., how far ahead in time) and the exoskeleton's degree of adherence to the reference trajectory (stiffness) are not yet fully understood. This study investigated the effects of stiffness and pre-emptive offset parameters in an impedance-controlled robotic arm on muscle activation and perceived exertion of the user. Thirteen participants were instructed to lift a load equivalent to 15% of their maximal voluntary contracted force in collaboration with a robotic arm with 40°–135° of elbow flexion in 1.12 s. Three levels of stiffness (lower: 0.1 N m deg−1, medium: 0.2 N m deg−1, and higher: 0.31 N m deg−1) and two levels of pre-emptive offsets (shorter: 0.1 s and longer: 0.4 s) were investigated. We found that (1) during 0–0.5 s (acceleration stage) of elbow flexion, a higher stiffness level and a longer pre-emptive offset decreased muscle activity; (2) during 0.5–1 s (deceleration stage) of elbow flexion, medium and higher stiffness with a shorter pre-emptive offset decreased muscle activity; (3) the perceived exertion and assistance of participants were improved with a higher stiffness and a longer pre-emptive offset, whereas cooperation was rated higher at a shorter pre-emptive offset under higher stiffness. This study reveals that the optimal parameters for stiffness and pre-emptive offsets for predictive impedance controls are different for different stages of elbow flexion.

    Background: This scoping review summarizes the tasks and outcomes in dynamic and functional balance assessments of individuals with chronic ankle instability, focusing on the physiological and biomechanical characteristics. Method: A comprehensive literature search was conducted in PubMed, Scopus, Web of Science, and MEDLINE databases in September 2023 and revised in April 2024. Studies evaluating dynamic and functional balance in chronic ankle instability using clinical tests, as well as biomechanical and physiological outcomes, were included. Results: Out of 536 publications, 31 met the screening criteria. A history of ankle sprain was the main focus of the inclusion criteria (28 articles, 90%). The star excursion balance test, emphasizing maximum reach distance, was the most common quantitative task (12 articles, 66%). Physiological data mainly came from electromyography studies (7 articles, 23%), while biomechanical variables were often assessed through center of pressure studies using force plates (17 articles, 55%). Conclusions: The preferred quantitative clinical assessment was the star excursion balance test, focusing on normalized reach outcomes. Qualitative functional balance assessments emphasize landing activities and center of pressure displacement. Electromyography is commonly used to analyze the tibialis anterior and peroneus longus muscles. However, there is a lack of qualitative data on dynamic balance control, including morphological characteristics and the center of mass adaptation.

    Studies investigating the walking patterns of elderly men are well documented, however, there has been no investigation of the gait parameters that specifically reflect the walking motion of elderly men. This study aimed to identify reliable gait parameters to represent the walking motion of elderly Japanese men. 33 elderly men (65 to 74.9 years old) and 20 very elderly men (≥ 75 years old) participated. A 3-dimensional motion analysis system was used to collect kinematic data and 52 gait parameters were analyzed namely; spatiotemporal gait, joint angle, and angular velocity parameters (peak value and peak timing at the hip, knee, and ankle joints. Our results showed that walking speed, cadence, walk ratio, gait cycle, peak joint angle timing, and angular velocity parameters significantly differ between elderly men and very elderly men. Delayed peak timing in the joint angle occurred during the terminal stance phase and pre-swing phase. The accurate parameters for walking motion in old Japanese men as they age may result from the peak timing of joint angle parameters and the peak value of angular velocity parameters.

    ロービジョン者による転落・接触事故の原因には、視覚情報の減衰とそれを起因とする身体動揺等がある。そのため、ロービジョン者が移動する場合、モノやヒトに触れ、得られた触覚情報をフィードバックすることで歩行速度を調節し安全な移動を企図する。一方で、ロービジョン者が実環境下を移動する際、歩行環境や同行援護者の有無等によって感覚情報の重みづけは変化する。本研究では、複数の感覚情報を組み合わせたマルチモーダルな移動支援の方法を検証する。
    ロービジョン者が実環境下を移動する際、歩行環境や同行援護者の有無、眼疾患の病態等によって視覚や聴覚、触覚等の感覚情報の重みづけは変化する。そのため、ロービジョンケア分野では近年、画一的な移動支援から複数の感覚情報を組み合わせたマルチモーダルな移動支援への転換が目指されている。
    当該年度は、2種類の実験により、時間的・空間的制約下のリスク回避動作と視覚情報、心理的不安感の関係性を検証した。方法として、健常者16名を対象に求心性視野狭窄5°、10°、オクルージョン0.04の視覚状態を模擬的に再現し、視覚状態と段差またぎ動作の関係性を検証した。また、動作実行に伴い3秒間の時間的制約を設定し、その際のSTAI尺度等を用いて特性-状態不安を測定した。次に、同様の実験を視覚障がい者を対象に実施した。
    一般的に、視野障害により視覚入力が減少すると、物体の知覚や姿勢制御が困難になる.上記実験結果では、保有視野が狭まり環境の情報収集が困難な視覚条件で、被験者は段差でのつまずきや空隙への転落リスクを回避するために、股関節を屈曲し足部を高く挙上させて空隙またぎ動作を企図した。一方で、オクルージョン条件により視野だけでなく保有視力も制限された条件では、保有視覚による環境情報の収集が困難となり、また移動に対する心理的不安感も一層高まった。そのため、股関節の屈曲伸展動作は抑制的になり、段差に対して探索的な行動が示された。
    当該年度に予定していた実験・解析を実施することができたため。
    本申請研究は視覚障害の多様な見え方と生活動作との関連性に焦点をあてるため、視覚障害者を対象とした追加実験を実施する予定である。なお、昨年度中に得られた実験データは、眼科医学や視覚リハビリテーション、健康科学系の学会において成果発表をおこなう。

  • My research focuses on understanding human functional performance and physiological adaptability in everyday life, from the perspectives of ergonomics, physiological anthropology, and occupational therapy. I pursue applied studies that integrate biomechanical, physiological, and behavioral assessments to improve health, usability, and comfort.
