    Introduction: The aim of this study was to clarify the effectiveness of various imaging modalities and characteristic imaging features in the screening of IgG4-related dacryoadenitis and sialadenitis (IgG4-DS), and to show the differences in the imaging features between IgG4-DS and Sjögren’s syndrome (SS).
    Methods: Thirty-nine patients with IgG4-DS, 51 with SS and 36 with normal salivary glands were enrolled. Images of the parotid and submandibular glands obtained using sonography, 2-[18F]-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (FDG-PET/CT), computed tomography (CT) and magnetic resonance imaging (MRI) were retrospectively analyzed. Six oral and maxillofacial radiologists randomly reviewed the arranged image sets under blinded conditions. Each observer scored the confidence rating regarding the presence of the characteristic imaging findings using a 5-grade rating system. After scoring various findings, diagnosis was made as normal, IgG4-DS or SS, considering all findings for each case.
    Results: On sonography, multiple hypoechoic areas and hyperechoic lines and/or spots in the parotid glands and obscuration of submandibular gland configuration were detected mainly in patients with SS (median scores 4, 4 and 3, respectively). Reticular and nodal patterns were observed primarily in patients with IgG4-DS (median score 5). FDG-PET/CT revealed a tendency for abnormal 18F-FDG accumulation and swelling of both the parotid and submandibular glands in patients with IgG4-DS, particularly in the submandibular glands. On MRI, SS had a high score regarding the findings of a salt-and-pepper appearance and/or multiple cystic areas in the parotid glands (median score 4.5). Sonography showed the highest values among the four imaging modalities for sensitivity, specificity and accuracy. There were significant differences between sonography and CT (p = 0.0001) and between sonography and FDG-PET/CT (p = 0.0058) concerning accuracy.
    Conclusions: Changes in the submandibular glands affected by IgG4-DS could be easily detected using sonography (characteristic bilateral nodal/reticular change) and FDG-PET/CT (abnormal 18F-FDG accumulation). Even inexperienced observers could detect these findings. In addition, sonography could also differentiate SS. Consequently, we recommend sonography as a modality for the screening of IgG4-DS, because it is easy to use, involves no radiation exposure and is an effective imaging modality.

    症例は38歳男性。開口障害を主訴に当科を紹介受診した。患者は22歳時にも下顎左側の疼痛と腫脹のため当科を受診しており、その時は下顎骨左側の慢性骨髄炎と診断し、掻爬術と閉鎖式持続灌流療法を施行していた。それらの治療で症状は軽快し、経過観察のための受診には訪れないまま15年間が経過していた。そして今回、下顎の不動化、開口量減少、多発性の重度齲蝕がみられる状態で再び当科を受診するに至った。パノラマX線撮影では下顎骨全体が綿花様の外観を呈する所見が示された。CT検査では下顎骨全体が膨張し変形している様子に加え、両側の顎関節強直も観察された。Tc-99m骨シンチグラフィーでは下顎骨で取り込み量増加が認められた。下顎骨のPaget病と両側性顎関節強直症と臨床診断し、リセドロン酸17.5mg/日を8週間投与した後に両側顎関節間隙形成術を施行した。こうした治療によって下顎の可動化は達成されたが、最大開口量は術後3年間にわたり14mmのままであった。この3年間に顎関節の再強直症は観察されなかった。

    Sonography is an easy to apply, inexpensive, and non-invasive diagnostic modality.
    Although tissues behind bony structures and deep tissues are not delineated, sonography
    can depict superficial soft tissues very clearly. In the head and neck region, however, it
    has not yet been widely performed, as the anatomical structures are complicated, and
    considerable experience is needed both to perform an examination and to make a
    diagnosis.
    To perform examinations smoothly, operators must be familiar with the sonographic
    system in use, and take images at standard planes. To make a correct diagnosis, operators
    require knowledge of the sonographic anatomies on those planes, representative
    sonographic signs and artifacts, and common diseases and their typical sonographic
    findings.
    In this paper, we have explained the sonographic anatomies on standard planes, and
    sonographic findings of common diseases in the oral and maxillofacial regions.

    本研究の目的は、人工知能に多施設共同研究により集められた様々な唾液腺疾患の超音波画像を学習させ、それぞれの病変を識別・診断できる学習モデル(人工知能)を作成することである。
    最初の1、2年で唾液腺(耳下腺・顎下腺)に発症する代表的な疾患の超音波画像データを各共同研究施設のデータベースから収集し、それらを自動診断する学習モデルを作成する。2年目以降は、上記で作成した学習モデルを各共同研究施設に転送し、新たに撮像された病変画像を順次追加学習させ、精度の向上を試みる。さらに、海外の協力施設(オスロ大学・延世大学)からデータを提供してもらい、人種による影響を調査する。

    The ultimate goal of the project was to establish the basis for the introduction of sonography into the international diagnostic criteria for Sjogren's syndrome. We tried to achieve quantification/objectivity of the diagnosis through the application of ultrasound elastography.
    Elastography images and measurements are affected by the pressure applied. In this study, to keep the pressure applied during examinations within a certain range, we attempted to develop a pressure sensor that can be attached to the probe. Elastic springs, slide resistors, and stainless steel movable plates were connected to a support surface. When the movable plate is moved, the spring extends and the resistance value increases. The amount of change was measured as a voltage by applying a constant voltage to the entire area. The force applied and the display on the digital panel were in a linear relationship. Therefore, the prototype of pressure sensor could be successfully developed.

    We aimed to determine the image quality level to diagnose the mandibular canal on dental cone-beam CT images using the specially developed phantom. Our final goal was to establish the reference dose level based on the image quality index.
    we compared this new quantitative image quality evaluation method with the result of the ROC analysis, in the detectability of the mandibular canals in CBCT. The number of extracted holes reflected an influence different scanning conditions, and it almost corresponded to the Az values calculated by ROC analysis. These results suggested the we can predict the detectability of the mandibular canals using our image quality evaluation method.
    This quantitative method of evaluating CBCT image quality considered to be more useful and less time-consuming than evaluation by observers. Reference dose level was established based on the image quality index by our analyses.

    To introduce sonography to the international diagnostic criteria of Sjogren's syndrome, more basic issues, differences in diagnostic units, center frequency, frequency of examinations, etc., were examined at multiple affiliations.
    Comparing the diagnostic accuracy of images taken of the same patient with two types of diagnostic units at four center frequencies, the diagnostic unit by company A was significantly higher, but there was no significant difference in center frequency. The frequency of examination needs to be about 1 case per month, and accurate feedback seems to be required.
    Next, each observer was asked to diagnose sonographic images of 107 cases without final diagnosis (evaluation 1). After learning with final diagnosis, each observer was asked to diagnose another sonographic images of 31 cases (evaluation 2), results of both were compared. Diagnostic accuracy was improved in all observers, and we confirmed the effectiveness of the diagnostic training.

    We performed an experimental study for establishing non-invasive image diagnosis of Sjogren’s syndrome (SS). The tendency of an elevation of oxy-hemoglobin after acid stimulation was observed using near-infrared spectrometer in the parotid glands. However, it did not correlate with the degree of gland damage. Moreover, we could not obtain similar results even in the same patients. Motion sensitized driven equilibrium (MSDE) method on MRI could not offer better images. Dynamic curve analysis on the conventional salivary gland scintigraphy showed significant differences between SS and non-SS in every calculated value in the submandibular glands. Therefore, diagnostic criteria on salivary gland scintigraphy should be established based on the values in the submandibular glands.

    We performed an experimental study for“sonographic sialography", sonography after injecting sonographic contrast medium through ducts. Sonazoid was acceptable in manipulation capability and osmotic pressure, and caused no inflammatory reaction to rat parotid glands. We could obtain enough signals of contrast medium in our phantom experiments. Supposed problems when it is applied in a clinical setting are difficulty in measuring duct diameter by enhancing effect, and inferior depiction of deeper parts when the imposed parts above are enhanced.

    Digital system is widely used in medical imaging and this trend is found in general dental practitioners, too. However, no consensus was obtained about the optimum display of the digital image and information in the digital images are not fully used for diagnosis and treatment planning.
    In this study, low contrast detectability in the digital systems was compared with that in the film using specially developed image quality phantom for quantitative evaluation. Then diagnostic accuracy for proximal caries of the extracted teeth was evaluated using the same systems and the same observers.
    Regression analysis showed high correlation between low contrast detectability and diagnostic accuracy. There was no statistically significant difference between diagnostic accuracy of the digital systems and that of the film. Effect of exposure on diagnostic accuracy or low contrast detectability, however, was larger in the digital systems than in the film. This effect was smaller in the digital system with automatic exposure compensation than in that without it.
    Further investigation clarified that image contrast of the digital system was completely different from that of the film. Image contrast of the digital system in low exposure was far inferior to that of the film. We developed the image display algorithm to improve contrast in low exposure. The result showed that overall image contrast of the processed image was better than that of the original and it was similar to that of the film.
    This image display algorithm may have general applicability to all digital intraoral imaging systems.

    Ultrasonography is an useful imaging tool in the assessment of cervical lymph nodes. Gray-scale ultrasonography is widely used in the evaluation of the number, size, site, shape, borders, matting and internal architectures of cervical lymph nodes. Size is the most frequently used criterion for diagnosis ; sensitivity and specificity vary widely, depending on the cutoff value that is used. So, we consider that the internal architecture with the ultrasonogram is the most reliable sign of a metastatic lymph nodes. It is necessary for us to establish a technical method for the quantitative analysis of ultrasonographic internal architecture.
    Purpose : The purpose of this study is to evaluate the ultrasonographic internal architecture of enlarged cervical lymphnodes from the oral carcinoma with the modified complexity value (MCV), and to determine whether the MCV is a valid diagnostic parameter in the differentiation between metastatic and non-metastatic lymphnodes.
    Subjects and Methods : The ellipsoid ultrasonographic phantom and 50 enlarged cervical lymphnodes (metastasis ; 27, non-metastasis ; 23) were examined with a linear ultrasound scanner. Two hundred fifty six gray scale images were reconstructed in the 16 gray scale stages. MCV was calculated to divide the total length squared of pixels by the total area of the images in the same density level in the region of interest (ROI). Definite diagnosis of the nodes was obtained by histological examination following neck dissection.
    Results and Discussion : MCVs in the different conditions of the phantom were not differed each other. the same one. The sum of MCV values of the metastatic lymphnodes was higher than that of the non-metastatic lymphnodes.
    Conclusion : The MCV of lymphnode ultrasonogram is an useful parameter for differentiation between metastatic and non-metastatic enlargement in the cervical lymphadenopathy in the oral carcinoma.

    Many studies have reported that diagnostic accuracy of the digital dental imaging system is comparable to films. It may be due to the inadequate image display of the digital data The purpose of this study was to obtain maximum image information from digital dental imaging system by applying optimum image display of the digital data. Further aim was to develop a computer-assisted diagnosis system using caries diagnosis algorithm and optimized images. The results obtained are as follows.
    1.Collection of digital image data
    Twenty-seven extracted teeth including caries of various grades were prepared. Pair of two teeth were radiographed with digital dental imaging system. Exposure time was varied in 8 steps including optimum exposure. Teeth were cut into halves and histopathology of proximal surfaces was assessed under the stereomicroscope. The final diagnoses were used as the "gold standards".
    2.Algorithm of automated outline extraction of proximal tooth surface
    To detect proximal tooth surface automatically, an algorithm was developed using Wavelet transforms. It was possible to extract exact proximal tooth surface automatically by using this method.
    3.Algorithm of automated caries diagnosis
    A region of interest (ROI) was set at the interproximal position. An algorithm was developed to measure the pixel value changes vertical to the tooth surface. By moving along the proximal tooth surface, a profile of the pixel value changes. along the proximal tooth surface with certain thickness was obtained. Several representative values of the pixel value change have been determined.
    4.Evaluation of the diagnostic accuracy of this system
    Automated caries diagnosis was performed using the representative values extracted above. The diagnostic accuracy of the present system was comparable to that of the observers and better over some of the observers. We consider this system may be useful as a computer-assisted diagnosis system for proximal caries diagnosis.

    The purpose of this study was to clarify the value of quantitative Doppler waveform analysis of blood-flow as an objective indicator of pain in the region of the temporomandibular joint (TMJ).
    Power Doppler sonography was, performed using a Sequoia 512 unit (Acuson, Mountain View, USA) with a linear transducer (15L8) to thirty healthy volunteers who had no pain and six patients who, had pain in the region of the TMJ. An axial power Doppler sonography image parallel to the Frankfurt horizontal plane and,perpendicular to the surface of the skin at the TMJ was obtained to detect the artery running close to the TMJ. With reference to the power Doppler image, a sample gate was placed at the center of the target artery, and then the velocity waveform was traced and a spectral analysis was performed with the built-in soft-ware for the automatic Doppler tracing to obtain the maximum velocity (MAX), the minimum velocity (MIN), the pulsatiity index (PI), and resistive index (RI). The pulse Doppler parameters were as follows : pulsed wave, central frequency, 8.0 MHz ; gate, 1mm ; filter, 1; Doppler angle, less than 60 degrees ; sweep speed, 50mm/s. Furthermore ; the Fourier transform of the waveform was performed to analyze the frequency pattern of the waveform. The mean value of each parameter of the velocity waveform was calculated, and the mean values and of volunteer group were compared to these of patient group. The distribution of the frequency of the velocity waveform of each subject was also compared to each other.
    As a result, differences between the volunteer group and patient group in MAX and MIN were not significant, but differences between the two groups in P1 and RI were significant. Moreover, the frequency pattern of the waveform of the volunteer group was different from that of the patient group. Furthermore, the vascularity of the TMJ region in patient group was rich rather than that in volunteer group.
    In conclusion, the quantitative Doppler waveform analysis of blood-flow, especially the frequency analysis has the potential of an objective indicator, of pain in the region of the TMJ.

    VX2 tumor cells were transplanted in oral floor of the white rabbits to develop the cervical lymph node metastasis. The thermotherapy was performed by using THERMOTRON-RF8.
    The thermo therapy failed to control the primary site in all 5 cases, and histopathologic assessment at the end of the follow-up period revealed local relapses due to incomplete thermo therapy. On Power Doppler US image obtained after thermotherapy, the internal blood flow disappeared within the primary site ; however, the residual perfusion was detected at periphery. Therefore, the residual blood flow at periphery might be attributed to the failure of the control of the primary site.
    In regard to the prevention of lymph node metastasis, the effectiveness of thermo therapy was still questionable. In the process of the lymph node metastasis, the vascularity increased at first, and later, the avasular area appeared and it became larger to be depicted as no signal within lymph node.
    If the metastasis was large enough to be detected as avascular area, the thermotherapy could not control the metastasis.
    If the tumor deposit was not so large to be detected as avascular area, viable tumor cells were completely disappeared in some nodes. In such nodes, the transient rich vascularity decreased after thermotherapy. The other nodes retained rich vascularity after thermotherapy and such nodes could not be controlled by thermotherapy. Therefore, in the microscopic metastasis, the disappearance of the depicted vessels obtained by power Doppler images was indicative of success of the thermotherapy.
    We think that the fixation of model animals and the evaluation of the three-dimensional thermal distribution are essential for success of the thermotherapy.

    Aims To analyze potential contrast media for a new diagnostic method having merits of both sialography and sonography.
    Methods Saline and various contrast media (Urografin 76% ; 100%, 90%, and 67% Lipiodol Ultra-Fluide ; 67% Lipiodol Ultra-Fluide with emulsifier ; 5% and 1% barium sulfate ; and Levovist) were injected into thin tubes (0.5mm and 0.25mm diameter) at about 0.001 - 0.1 ml/sec. Ultrasonography was performed with a diagnostic ultrasound imaging system (Squoia 512, Acuson Co, USA) and the pulsed Doppler imaging was obtained, All images were transferred to a personal computer and analyzed with an image processing software, NIH Image. A rectangular region of interest (ROI) was selected in a pulsed image and the sum of the gray-levels of all pixels in the ROI was calculated for representing Doppler signal intensities. The relationship between Doppler signal intensity and the kind, concentration and velocity of the fluid were analyzed.
    Results Levovist, 90% and 67% Lipiodol Ultra-Fluide, and the barium sulfate solutions produced Doppler signals. The mixture of Lipiodol Ultra-Fluide and saline showed high signals at any concentration in contrast to the barium sulfate solutions. Signals could be observed at any speed, from the speed of normal sialography down to 0.001 ml/sec, and there was a proportional relationship between signal intensity and velocity for all fluids producing signals.
    Conclusions As we could obtain high signals with several fluids, the diagnostic usefulness of sialographic sonography should be assessed clinically.

  • Experimental study for detecting cervical lymph node micro-metastasis using Doppler ultrasonography

    Results of this study were as follows :
    1. Power Doppler ultrasonography (US) of cervical metastatic lymph nodes in rabbit with oral carcinoma (VX2 carcinoma implanted) using high frequency transducers (14MHz) was compared with intranodal angiography. Power Doppler Us could present intranodal vascularization almost compatible to intranodal angiography. Therefore, it was considered that power Doppler Us was useful to assess the intranodal angioarchitecture.
    2. Histopathologic finding and angiography showed that intranodal vasculization increased transiently in early stage of the tumor growth in lymph node, and thereafter decreased as the tumor growth advanced. In some case, peripheral vascularization of metastatic lymph node was found in advanced stage of intranodal tumor growth. Power Doppler Us could also reveal these change of intranodal vascularization in the tumor growth process.
    3. Pulse Doppler Us showed that velocity of blood flow at hilar region increased transiently in early stage of the tumor growth in lymph node, thereafter decreased as the tumor growth advanced. The phase of the maximum velocity of blood flow at hilar region associated with the phase then focal area of no perfusion was shown.
    4. Pulse Doppler Us showed that the vessel at hilar region had beats as the velocity of the blood flow increased.
    5. Peripheral blood flow was found in some cases of the metastatic lymph nodes when area of no perfusion within lymph node was presented.
    6. Above results suggested that when tumor implants in lymph node, firstly the velocity blood flow at hilar increase, and then intranodal vascularization increase transiently, and thereafter intranodal vascularization decrease as the tumor growth advanced, accompanying with the presence of peripheral vascularization in some cases.
    7. It was considered that we could detect cervical lymph node metastasis in early stage (micro-metastasis) using Doppler US.

  • フッ素核(19F)磁気共鳴画像による悪性腫瘍浸潤範囲の評価法に関する実験的研究

    悪性腫瘍の浸潤範囲を正確に評価するためには、腫瘍と腫瘍周囲の炎症性変化とを画像上で区別することが必要である。前年度は炎症性変化のモデルとして、閉塞性顎下腺炎の磁気共鳴画像について検討したが、今年度は閉塞性顎下腺炎の磁気共鳴画像と超音波画像との対比を行った。
    閉塞性炎を誘発した顎下腺の湿重量は、結紮後1-2日では対照側より増加し、3日で対照側と同程度になり、4日後から減少した。病理組織標本では湿重量の増加時には腺房の浮腫が認められ、リンパ球の浸潤は認められなかった。減少時には腺房の萎縮・消失、導管様構造及び結合組織の増生、リンパ球の浸潤が主たる変化として認められた。以上より、結紮後1-2日を急性期、4-7日を慢性移行期とした。
    結紮側顎下腺の磁気共鳴画像は、SET1強調画像では対照側よりやや低い信号強度を示し、高速SE並びにGRET2強調画像では対照側よりも高信号を呈した。急性期と慢性移行期との経時的変化は、高速SE並びにGRET2強調画像で認められたが、SET1強調画像での経時的変化の検出は不可能であった。
    結紮側顎下腺の超音波画像では、急性期には内部エコーレベルの低下や分布の不均一化は認められず、血流の増加が検出された。慢性移行期には内部エコーレベルの低下・分布の不均一化が認められ、血流は対照側と同程度まで減少した。

    The new measure for texture analysis is based on the intuition that the important texture information for the human visual system is contained in the relative frequency of local extremas in intensity. The principal measurement in this process is the determination of the number of local gray level maximum and minimum along a one-dimensional scan direction. The gray level values are first sent through a smoothing process which eliminates reversals of small amplitude, thereby retaining only the principal extrema. The smoothing algorithm is the digital equivalent of the familiar analog mechanical process known as gear backlash and it was originally described as a preprocessing method for character recognition.
    The ultrasonographic images of the tissue-mimicking phantom, normal parotid and submandibular glands, sialadenitis and salivary gland tumors were used. The images were scanned, digitized. In the digitized images, the selected ROI was analysed with the Max-Mm Measure in one dimension. The two-dimensional extension measured the max-mm features in the ROI.
    As the results, the threshold was increased, fewer extrema were detected and when the threshold exceeded the pixel density range of the image, no extrema were detected. Each image produced a characteristic curve which can be used to identify the texture. Each different density image of the same ROI in the base-material of the phantom produced a slightly different curve. Obviously, a larger size region of the ROT would give less variance in the features and more accurate classification results.

    ラット耳下腺の炎症程度を動物実験用磁気共鳴画像(MRI)上で定量的に把握するに先立ち、実験的耳下腺炎誘発方法の精度について検討した。ラットステンセン管を脳外科用クリップで結紮し、一定期間置く際に、クリップ先端に挟むアルミ箔の厚みを変化させることで、3段階のを設定した。それぞれの結紮圧に対し、1週間の2通りの結紮期間を設定し、計6グループの実験群を設定した。各実験群において、クリップ除去直後、3日後、7日後、14日後、28日後、84日後に、微小唾液腺造影と組織標本の作成を行い、耳下腺の回復過程を観察した。各ステージにおける耳下腺の回復状態が結紮圧と結紮期間に依存していたことより、この耳下腺炎誘発方法では、惹起させる炎症の程度を段階的にコントロールすることが可能であることが示唆された。
    次いで、動物実験用磁気共鳴画像(MRI)装置を用いて、ラット耳下腺のプロトン顕微MRI(MRマイクロイメージング)の方法を確立した。すなわち、磁場強度4.7テスラMRI装置(Varian社製 UNITY plus-SIS 200/330)を用いて、Wister系ラット雄(16週齢)の耳下腺MRマイクロイメージングの方法について検討した。使用したコイルは、プロトン用表面コイル並びにクアドラチャコイルであり、撮像シーケンスはスピンエコー法並びにグラディエントエコー法とした。撮像パラメータを変化させながら、得られるMRマイクロイメージングの画像を比較した。その結果、良好な信号/ノイズ比を保ちながら空間分解能を向上させるためには、加算回数を可能な限り多くするだけでなく、バンド幅の狭いシーケンスの選択が有効であることが示唆された。
