脊髄損傷に対する神経幹細胞移植療法の臨床応用 一度損傷を受けた哺乳類の中枢神経は再生しないと信じられてきたが、近年中枢神経にも可塑性が存在することが明らかとなり、中枢神経疾患に対する再生医療が脚光を浴びている。現在、脊髄損傷に対する効果的な治療法は存在しないが、我々はヒトの随意・巧緻運動において最も重要な回路である皮質脊髄路が脊髄の神経幹細胞を移植した際に再生することに成功した。さらにヒトのES細胞から脊髄の神経幹細胞を作り出すことに世界で初めて成功しており、この細胞を用いて脊髄損傷後の運動麻痺の原因である断裂した皮質脊髄路の再生を誘導し、損傷した電気回路を再構築するという新しい治療法を臨床応用するために日々研究を行っている。 脊椎・脊髄疾患 脊椎・脊髄疾患に対する診療を行う。

    ヒトの中枢神経は一度損傷を受けると再生しないと約100年もの間信じられてきたが、ここ数年の神経科学の急速な発展によって中枢神経であっても環境が整えば再生することが明らかとなった。そのため、これまで効果的な治療法がなかった脊髄損に対する新しい治療法としてiPS細胞や間葉系細胞をはじめとした幹細胞移植療法による再生医療に期待が寄せられており、既に本邦でも臨床治験が開始されている。
    脊髄損傷によって運動麻痺が生じる直接的な原因はヒトの随意運動を直接支配する最も重要な神経回路とされる皮質脊髄路(錐体路)が断裂し脳からの電気シグナルが損傷部で途絶することにあるが、我々はこれまでに幹細胞移植により脊髄損傷によって断裂した錐体路が旺盛に再生し、断裂した神経回路が再構築することを証明してきた。しかしながら、その運動機能改善効果に関しては限定的であり、確実な治療効果を認めるためにはさまざまなハードルが残されている。本講演では脊髄損傷に対する再生医療においてどのように錐体路が再生し、断裂した神経回路が再構築されていくのかを概説し、治療効果をあげるためのハードルとなっているさまざまな問題、今後どのようにしてこれらのハードルを乗りこえていくのかを解説したい。
