    症例は70歳女性.労作時息切れの増悪のため近医を受診したところ,心房中隔欠損症を指摘され当院へ紹介受診となった.2次元経胸壁心エコー図検査で心房中隔に複数の欠損孔と左右短絡血流が認められたことから多孔性心房中隔欠損症(Multiple-ASD)と診断されたが,欠損孔の位置,個数,形態など解剖学的位置関係の把握には限界があり,3次元経胸壁心エコー図検査の機能も併用することで3ヶ所の欠損孔を同定することができた.術中経食道心エコー図検査では3次元経胸壁心エコー図検査で指摘した所見と同様の所見が得られ,経皮的心房中隔欠損閉鎖術において最も大きい欠損孔を含む2ヶ所を閉鎖し手技終了となった.2次元経胸壁心エコー図検査と併用した3次元経胸壁心エコー図検査の様々な機能を活用することで,multiple-ASDの欠損孔の位置や形態評価に有用であった症例を経験したので報告する.(著者抄録)
