    This study aimed to examine the changes that took place between 2015–2019 and 2020 and reveal how the COVID-19 pandemic affected financial contributions from donors. We used the Creditor Reporting System database of the Organization for Economic Cooperation and Development to investigate donor disbursement. Focusing on the Group of Seven (G7) countries and the Bill and Melinda Gates Foundation (BMGF), we analyzed their development assistance for health (DAH) in 2020 and the change in their average disbursement between 2015 and 2019.
    As a result, total disbursements for all sectors increased by 14% for the G7 and the BMGF. In addition, the year 2020 saw an increase in DAH for the BMGF and the G7 except for the United States. The total disbursement amount for the “COVID-19” category by G7 countries and the BMGF was approximately USD 3 billion in 2020, which was 3 times larger than for “Malaria,” 8.5 times larger for “Tuberculosis,” and 60% smaller for “STDs including HIV/AIDS” for the same In 2020 as well, the United States, the United Kingdom, Japan, Italy, and Canada saw their disbursements decline for more than half of 26 sectors. In conclusion, the impact of COVID-19 was observed in the changes in DAH disbursement for three major infectious diseases and other sectors. To consistently address the health needs of LMICs, it is important to perform a follow-up analysis of their COVID-19 disbursements and the influence of other DAH areas.

  • Association between Health Indifference and Problem Drinking using a Nationwide Internet Survey Reviewed International journal

    Background: Little is known about the vulnerable populations and problem drinking in terms of health inequality. This study aimed to investigate the relationship between health indifference estimated by Health Interest Scale (HIS) and problem drinking identified by the Alcohol Use Disorder Identification Test (AUDIT).
    Methods: A cross-sectional study was conducted utilizing data from a nationwide internet survey in Japan in 2022. The number of total participants was 29,377, with 49% of them being male, and the mean age was 47.9 (±17.9) years old. The participants were categorized into the following groups based on the quintiles of HIS score: health indifferent (0–16), low health interest (17–20), middle health interest (21–22), middle-high interest (23–26) and high health interest (27–36) groups. Problem drinking was identified as AUDIT score of ≥8 points, and controlled drinking was defined as ≤3 times per week as the frequency of drinking, ≤2 drinks (20 g ethanol) per one occasion as the volume of drinking, and no alcohol-related problem, such as blackout after drinking, based on AUDID.
    Results: The association between health indifference and problem drinking was explored through logistic regression with adjustment for various socioeconomic status such as education, income level, and occupation; the adjusted odds ratio (aOR) was 1.72 [95% confidence interval (CI): 1.51–1.95]. Health interest group were likely to have controlled drinking compared with the health indifferent group; the aORs were 1.34 (95% CI:1.20–1.51) for high health interest groups.
    Conclusion: No or lower health interest groups were a vulnerable population for problem drinking, regardless of their socioeconomic status. It could be useful to identify the health indifferent group through HIS and to monitor the impact of health intervention for this group for the reduction of health inequality.

    The COVID-19 Vaccines Global Access Facility (COVAX Facility) is a global risk-sharing framework established to ensure the equitable distribution of COVID-19 vaccines. This paper explains the mechanism of the COVAX Facility and discusses the following aspects: (1) issues related to vaccine supply through the COVAX Facility, and vaccine donation or provision as a means of bilateral assistance; (2) case studies of vaccine supply and the number of vaccines administered in the Pacific Islands; and (3) the state of vaccine support in terms of the island nations’ vulnerability to COVID-19.

    The letter “Global extension of Japanese medical products related to COVID-19: A survey of WHO Emergency Use Listing” by Dr. Wakabayashi et al. has been published in GHM Open.
    The study focused on the World Health Organization’s Emergency Using Listing (EUL) for COVID-19. EUL is a procedure for the emergency use of vaccines and therapeutic and in vitro diagnostic products based on clinical data available at the time under the Public Health Emergencies of International Concern. As of January 2022, 28 in-vitro diagnostic products from China and three vaccines from India were on EUL. However, the only vaccine manufactured in Japan was listed on EUL. For Japanese medical products to be used globally in emergencies, this study discussed the importance of taking preparation and initiative for the global public procurement of medical products.

    保健分野の開発援助(developmental assistance for health;DAH)は、低中所得国において保健サービスを維持するための重要な財政的支援である。本研究では、2015~2019年のDAH支出額と2020年の支出額にみられる変化を検証し、COVID-19パンデミックが援助供与国からの支出額にどのような影響を与えたのか検討した。経済協力開発機構のデータベースを用いて、DAHの主要供与国であるG7とビルゲイツ財団に焦点を当ててデータを分析した。2020年には、G7(米国を除く)とビルゲイツ財団は全開発援助の総支出額が14%増加していた。ただし、5か国では26分野の内半分以上の分野で過去5年平均の支出額と比較して2020年は支出額が減少していた。保健分野への支出額は相対的に増加していたが、感染症分野のなかでの三大感染症等への支出が5年間平均と比べ2020年には減額している国もあった。COVID-19の影響は、3大感染症や水供給・衛生などの他のセクターに対する支出額の変化からも観察された。

    健康関心尺度(HIS)から推定される健康無関心層と問題飲酒との関係を調査した。インターネットを利用した自記式質問票調査を通して29377名を登録した。HISスコアによって被験者を健康無関心層、低関心層、中関心層、中~高関心層、高関心層に分類した。アルコール使用障害同定テスト(AUDIT)スコアに基づいて被験者を非問題飲酒者と問題飲酒者に分類した。被験者の49%は男性で、平均年齢は47.9±17.9歳であった。問題飲酒者は4352名(15%)で、平均年齢は48.1±16.4歳であった。問題飲酒割合は男性が21.8%、女性は6.6%であった。非問題飲酒と比べて、問題飲酒で回答率が高かった社会人口統計学的特徴は年齢40~59歳、男性、高い教育レベル、既婚、一人暮らし、幹部/管理職、無期雇用従業員、自営業、収入600~1000万円、収入1000万円以上であった。健康無関心層の割合は問題飲酒者の方が非問題飲酒者より高かった(26%対20%)。平均HISスコアは非飲酒者と低リスク飲酒者で高く、中リスク飲酒者と高リスク飲酒者では相対的に低かった。潜在的交絡因子で調整後、高関心層と比べて健康無関心層または低関心層は問題飲酒と関連していた。社会経済的要因にかかわらず、健康無関心層では問題飲酒のリスクが高いことが示唆された。

    BACKGROUND: A new ready-to-drink premixed high-alcohol-content beverage, called strong chū-hai, was launched in Japan, and more recently, in Taiwan and Australia. We aimed to examine the popularity and association of strong chū-hai with individual alcohol use, both of which remained unclear. METHODS: We conducted a cross-sectional study using data from the Japan "Society and New Tobacco" Internet Survey conducted from February 1-28, 2022 in Japan. We enrolled 27,993 respondents (aged 15-81 years; male 48.5%), including 15,083 current alcohol users. Using inverse probability weighting of data from the 2016 Comprehensive Survey of Living Conditions on Health and Welfare, we estimated the weighted proportions of strong chū-hai users among all respondents and constructed multivariable logistic regression models to estimate the weighted odds ratios (OR) and 95% confidence intervals (CI) of strong chū-hai use for hazardous and harmful alcohol use, defined as a score ≥8 of the Alcohol Use Disorder Identification Test, among current alcohol users. RESULTS: Among all respondents, 56.2% (weighted proportions: past, 35.9%; and current, 20.3%) drank strong chū-hai. Among drinkers, both past and current strong chū-hai use, compared to never use, were associated with hazardous and harmful alcohol use (past, OR 1.73, 95% CI 1.42-2.12; current, OR 2.19, 95% CI 1.79-2.69). CONCLUSIONS: Our study found that more than half of the respondents experienced strong chū-hai consumption, suggesting that it is widely used in Japan. In addition, both past and current strong chū-hai use were associated with hazardous and harmful alcohol use among current alcohol users.

    201の国と地域のCOVID-19ワクチン政策の状況を在外公館が調査した。2021年3月末までに159ヶ国から回答があり、日本のデータを加えて分析した。質問項目は自由回答で、使用ワクチンとともにワクチン接種の資格のような五つの政策を含んでいた。2021年2月24日までは81ヶ国がワクチン接種を開始しなかったが、3月末までにその数は37ヶ国に減少し、特にアフリカのワクチン無接種国が40から16に減少した。43ヶ国は妊婦へのワクチン接種を推奨していなかった。ワクチンは116ヶ国では無料で、43ヶ国は無過失補償を提供していた。124ヶ国ではワクチン接種は任意であった。2021年3月までに多くの国でワクチン接種が始まり、特にアフリカで著しかったが、アフリカではmRNAワクチンを利用できないようであった。

    In response to the coronavirus disease 2019(COVID-19)pandemic, vaccines developed at an unprecedented speed, and the vaccination were highly promoted mainly in developed countries. On the other hand, vaccination rates in many developing countries, especially in Africa, were still low, highlighting the global vaccine disparity.

    The COVID-19 Vaccine Global Access Facility (COVAX Facility) is a global risk-sharing framework established to ensure the equitable distribution of COVID-19 vaccines with guaranteed safety, efficacy, and quality to the entire world, regardless of their economic power. This paper describes how to achieve equitable access to the COVID-19 vaccines through COVAX Facility and discusses (1) their mechanisms and issues related to the global vaccine disparity, (2) guidelines for prioritizing subpopulation for the vaccinations under limited supply recommended by the World Health Organization (WHO), and (3) communication with the participating countries of the COVAX Facility.

    It is commendable that the COVAX Facility was quickly established through the collaboration of WHO and other global health organizations. Moreover, COVAX Facility plays a significant role of platformer for vaccine donation through developing countries that had excessive stockpiling of COVID-19 in early 2020. However, it remains to be seen whether the strengthening of the health system focused on COVID-19 vaccination would effectively improve vaccination rates in developing countries. We expect that COVAX Facility could continue to have an impact globally and achieve its new goal of increasing the vaccination rate to more than 60% worldwide. Global cooperation and solidarity are important to reduce the vaccine disparity and to conquer the COVID-19 pandemic.

    Background: In order for Japan, as a member of the international community, to contribute to the achievement of health-related goals in the Sustainable Development Goals (SDGs), it is necessary to propose effective and strategic ways to contribute to and engage international organizations and institutions related to the health sector.
    Objective: This study examines the impact of contributions to COVID-19 on contributions to other sectors and other health areas.
    Methods: Using the database of Official Development Aid (ODA) at the Organization for Economic Cooperation and Development (OECD), we compared the contributions of major donor countries in the health sector (G7) and the Bill & Melinda Gates Foundation, the leading private philanthropic organization in the health sector, between 2020 and the average for 2015-2019.
    Results: In 2020, total contributions of all development assistance from the G7 and the Bill & Melinda Gates Foundation increased by 14% compared to the average of the previous five years. However, contributions to more than half of the 26 sectors in five countries (U.S., U.K., Japan, Italy, and Canada) decreased compared to the previous five-year average. For example, in the water supply and sanitation sector, the combined G7 and Bill & Melinda Gates Foundation contributions fell 14% from the five-year average, including a 43% decline in Japan, which was the largest donor country in the five-year average. On the other hand, contributions to the health sector increased. In particular, contributions to COVID-19 accounted for 17% of total health contributions in 2020 (approximately US$17.57 billion). Contributions to the three major global infectious diseases (HIV/AIDS, malaria, and tuberculosis) decreased by 2% in 2020 compared to the average for the previous five years, but still accounted for 37% of total contributions to the health sector in 2020. However, with respect to contributions from donors for each of the three major global infectious diseases, compared to the average of the previous five years, contributions for malaria decreased in France (46%), the United Kingdom (72%), Germany (100%), Japan (58%), and Canada (95%); those for TB decreased in the United Kingdom (56%), Germany (4%), Japan (74%) (74%), Italy (12%), and the Bill & Melinda Gates Foundation (28%), and for sexually transmitted diseases, including HIV/AIDs, decreases were observed in six countries except France and the Bill & Melinda Gates Foundation (11%). The COVID-19 pandemic has had a small impact on contributions to the world's three major infectious diseases, as well as on contributions in other sectors outside of health.
    Conclusion: It is possible that contributions to COVID-19 were prioritized under circumstances that made it difficult for countries to significantly increase their ODA contributions. In order to consistently address the health needs of low- and middle-income countries, it is important to analyze future trends in contributions to the health sector.

    Background: We developed an Official Development Assistance (ODA) tracker for health and examined Japan’s contribution.
    Methods: We used a database of ODA projects registered in the Creditor Reporting System (CRS) of member countries of the Development Assistance Committee of the Organization for Economic Cooperation and Development. We analysed the cash flows to health sectors and the amount of assistance and compared to those of G7 countries using CRS data between 2011 and 2019. Fund from the Gates Foundation and other private philanthropic organizations were also analyzed using CRS to compare their contributions with those of the ODA from the G7.
    Results: When focusing on “Basic Health” (CRS code 12200 series), which included the management of infectious diseases, the G7’s total contribution in 2019 was approximately $3.4 billion, approximately 51.6% of which was from the United States (~$1.76 billion). Japan contributed 5.5%, ranking fifth among G7 countries. The Gates Foundation’s contribution was about $1.85 billion, which is larger than those of each G7 country. Regarding “Water and Sanitation” (CRS code 14000 series); Japan remained the top contributor among G7 countries from 2011 to 2019.
    Discussion: One target of Sustainable Development Goal 3 (SDG3) is the response to infectious diseases. While Japan had a relatively small contribution to “Basic Health” including infectious disease control, Japan led the sector “Water and Sanitation,” which is under SDG6. Japan needs to continue its contributions to this sector and increase its aid to “Basic Health” which contribute to public health crisis such as COVID-19.

    This describes discusses whether loneliness developed hazardous alcohol use with cohort data from a national Internet survey in Japan conducted in February 2021 and 2022.

    The result showed loneliness was associated with increased severity of hazardous alcohol drinking patterns, independent of objective social isolation, psychological distress, and other potential confounders. We found that nondrinkers scored slightly higher on loneliness than low- and medium-risk drinkers. Nondrinkers had much lower loneliness scores than high-risk drinkers and those with probable alcohol dependence. Providing appropriate support to people experiencing high degrees of high loneliness will have a positive impact on the prevention of potential alcohol dependence.

Based on the current situation in Brazil's health sector, I will report on UNICEF's health projects in Brazil based on my experience working as a health specialist at the UNICEF Brazil office.     <文献概要>はじめに ブラジル連邦共和国(以下、ブラジル)は、金融・世界経済に関する首脳会合(G20)の2024年の議長国になり、2025年国連気候変動枠組条約締約国会議(COP30)をアマゾン地域で初開催するなど、今年度来年度と国際的な議論をリードする国の一つである。そのようなブラジルの保健分野の現状を踏まえ、UNICEFブラジル事務所で保健専門官として働いた経験を基に、UNICEFブラジルでの保健プロジェクトを報告する。なお、執筆内容は所属する組織等とは関係なく、個人の見解である。ブラジルは世界第11位の国内総生産(gross domestic product:GDP)で高位中所得国に分類される新興国であり、国際機関等からの支援卒業国である。そのため、他国からの金銭的な支援を受ける機会は限られている。UNICEFにおいて支援プログラムに用いる通常予算は、最も支援を必要としている子どもたちに最優先に支援が届けられるよう、以下の三つの指標を基に優先順位を判断した上で各国・地域に配分される。三つの指標とは、(1)5歳未満乳幼児死亡率が高い国、(2)1人当たり国民総所得が低い国、(3)子どもの人口が多い国である。世界全体でみると5歳未満の乳幼児死亡率がかなり改善され(出生1,000人当たり14人)、GDPが高いブラジルにはUNICEF本部からの援助金はほとんど分配されない。しかし、経済格差(2022年ジニ係数0.518)を背景とする貧困層(0歳から14歳人口の49%が貧困と推定される)の社会課題が多く残っている。そのような社会課題に対応するため、UNICEFブラジルでは、主にブラジル国内の個人からの寄付や企業等からの寄付によって活動を行っている。UNICEFブラジルの2022年の寄付受領額は約USD16.9百万(約25億円)となっている。また、ブラジル国内に首都ブラジリアの本部と8つの地区事務所とベネズエラ難民の対策としてボア・ビスタに臨時事務所が配置され、比較的規模の大きな国事務所となっている。UNICEFブラジルは、社会経済的に脆弱な北部や北東部の州(アマゾン州を含む)などを中心にブラジル全土で、本論で述べるような保健分野の活動を含めて子どもたちへの教育や保護などさまざまな分野の取り組みを行っている。

    2013年の「国際保健外交戦略」の策定により,国際的な保健分野の取り組みが日本の外交上の重要課題と位置付けられた.本論では,その後10年のグローバルヘルス外交の動向を俯瞰するとともに,日本における官民連携による具体的な事例やグローバルヘルス外交における主要な担い手の役割を述べ,日本のグローバルヘルス外交における官民連携の課題について考察する.日本政府は,グローバルヘルスにおける官民連携パートナーシップを,企業は持続可能な開発目標(SDGs)達成に向けた取り組みを活発化させてきている.しかし,グローバルヘルス分野において,適切に官民連携パートナーシップを活用し,企業価値にまでつなげられている日本企業は多くない.政府においては,グローバルヘルス外交に精通し,国際機関等と企業を適切につなぐことを支援できる人材,企業においては,グローバルヘルス分野にかかわる国際機関等との交渉ができる人材の養成が必要である.特に,2023年の広島サミットで採択された「グローバルヘルスのためのインパクト投資」のようにグローバルヘルス分野へ民間企業からの投資を促進させるためにも,グローバルヘルス外交への理解は重要な要素であると考える.またグローバルヘルス外交をめぐる国際的な動向が複雑化してくる中で,日本政府として,限られた政府開発援助をどの国際的な組織で活用し,グローバルヘルス分野における日本の存在感をいかに示すかは大きな課題である.グローバルヘルス外交での日本の貢献やその方向性を積極的に発信し,関心を持つ人々を広げていくことで,グローバルヘルスにおける様々なステークホルダーと効果的に協働していけると考える.(著者抄録)

    新型コロナウイルスの世界的な感染拡大に対し、異例のスピードでワクチンの開発が進み、先進国を中心に高いワクチン接種が進められた。その一方で、多くの途上国、特にアフリカ諸国におけるワクチン接種率は低く、世界のワクチン格差が浮き彫りになった。「COVAXファシリティ」は、安全性、有効性及び品質が保証された新型コロナワクチンを、途上国を含めた世界全体に公平に供給することを目的として発足した国際的な枠組みである。本稿では、COVAXファシリティを通じたワクチンの公平な分配実現に向けて、(1)ワクチンを公平に分配するメカニズムと課題、(2)ワクチン接種の優先順位に関するガイドライン、(3)COVAXファシリティ参加国とのコミュニケーションのあり方、に関して論じる。WHOをはじめとする国際的な組織が協働し、迅速にCOVAXファシリティという枠組みが構築され、2021年の早期に、途上国での新型コロナワクチン接種が開始されていることや、先進国が買い占めたワクチンをCOVAXファシリティに寄与してもらう仕組みを実現したことは評価できる。一方、ワクチン接種体制への支援強化が途上国への接種率向上に効果的に結び付くか、またCOVAXファシリティが求心力を持ち続け、世界における公平なワクチン接種を2022年末までに70%以上にという目標を達成できるかが今後注目すべき点である。ワクチン格差を是正し、新型コロナウイルス感染症の終息に向けてグローバルな協力と連帯がますます重要である。(著者抄録)

    2020年、世界的な新型コロナウイルス感染拡大に対して、異例のスピードで「新型コロナウイルス感染症のワクチン(以下、新型コロナワクチン)」の開発が進んだ。そして、2021年、高所得国を中心に、高いワクチン接種率を達成した。その一方で、低/中所得国におけるワクチン接種率は、2022年1月時点でも低く、世界のワクチン格差が浮き彫りになっている。新型コロナワクチンを世界全体に公平に供給する国際的な枠組みであるCOVAXファシリティに関して、本稿ではそのワクチン供給の仕組みを説明するとともに、(1)COVAXファシリティを通じた供給とワクチン寄付・供与をめぐる課題、(2)太平洋島嶼国を事例としたワクチン供給、(3)太平洋島嶼国の脆弱性に焦点を当て、ドナー国からのワクチン支援のあり方に関して論じる。COVAXファシリティという枠組みによって、2021年2月に低/中所得国でのワクチン供給が開始され、また高所得国が買い占めたワクチンの寄付が実現できた。しかしながら、ワクチン供給の「公平性」は実現できたとは言い難い状況となっている。太平洋島嶼国14か国の間でもワクチン格差が存在する。太平洋島嶼国は、国土の拡散性・狭隘性・離散性および国際市場からの遠隔性といった共通の課題を抱える。ワクチン接種率が高い国では、2国間援助やワクチン寄付等により、必要十分なワクチンが供給され、ほぼ対象年齢全員にワクチン接種が完了している、もしくは2022年内に完了予定である。しかしながら、キリバス、ソロモン諸島、バヌアツといったワクチン供給が十分であっても、効果的にワクチン接種に結びついていない国では、ワクチン損失が多く起こっている可能性がある。さらに、パプアニューギニアのように、基本的な保健医療水準が低い国では、健康危機における短期的に大規模な保健医療支援が行われたとしても、ワクチン接種向上に結びつかない可能性もある。新型コロナウイルス感染症が流行して、2年以上が経過しようとしている。今後、ワクチン接種の向上を目指すには、供給支援から、ワクチン接種体制や基本的な保健医療サービス支援等も含めた中長期的な支援にシフトしていく必要がある。(著者抄録)

    UNICEF Brazil has dedicated itself to the issue of adolescent mental health, especially since the pandemic period. UNICEF Brazil gathered partners with expertise in mental health and designed the “Speak up” initiative (“Pode Falar” in Portuguese) in 2021. UNICEF Brazil achieved significant results. Almost 78,000 adolescents, parents, and caregivers were provided with mental health and psychosocial support services through the Speak-up initiative for adolescents and an online course for professionals and workers who provide services on adolescent mental health. Over 37,000 adolescents engaged with the AI chatbot, which is a tool that promotes self-empowerment and body confidence, that impacts mainly girls (over 70% of users) in the whole of Brazil (UNICEF, 2023).
    Fortaleza in Ceara state is one of the project sites and provides service for the Speak up initiative. Ceara state was the second state with the highest number of teenagers served on the Speak up online platform, with 4,777 accesses, behind only São Paulo as of December 2022. Ceara state proposed adolescent health including services in-person for mental health to strengthen the debate and mental health care practices for adolescents. UNICEF Brazil will support this new project and expand those experiences into other areas of Brazil.
    I conducted a field visit in Fortaleza in Ceara state in Brazil and drafted a project proposal for private companies. A company accepted my proposal, and they will support USD 300,000 to UNICEF Brazil until 2025 through UNICEF HQ. UNICEF HQ has a connection with this company.

  • ODA Tracker data used by the Global Health Strategy Promotion Council

    Data from the ODA tracker developed by iGHP was used as reference material at the 8th Global Health Strategy Promotion Council (November 28, 2023) hosted by the Cabinet Secretariat and Medical Strategy Promotion Headquarters. The document "Bilateral aid in the health sector funded by G7 countries in 2020" showed that Germany (34%) and Japan (29%) led the way in terms of the proportion of aid allocated to COVID-19 countermeasures. In addition, the document "Annual trends in bilateral aid in health-related fields funded by G7 countries (2016-2020)" showed that Japan's aid in the health-related field decreased in "water and sanitation" from 2016 to 2020, and increased in "basic health" including infectious disease countermeasures in 2020.
