    OBJECTIVE: This study aims to comprehensively evaluate oral health-related quality of life (OHRQoL) and associated factors from multiple perspectives in patients with head and neck cancer (HNC) with maxillary prostheses after maxillectomy. STUDY DESIGN: After obtaining informed consent, OHRQoL, HNC-specific QoL, and 7 oral functions based on the concept of oral hypofunction were investigated in study participants. Participants were classified into 2 groups according to OHRQoL scores: G1, better OHRQoL and G2, poorer OHRQoL. HNC-specific QoL and oral functions were compared between the 2 groups. Additionally, associations of OHRQoL with oral functions and cancer-related therapies were comprehensively investigated using 3 logistic regression analyses. RESULTS: Fifty patients with HNC were enrolled. In HNC-specific QoL, significant differences between G1 and G2 were found in swallowing (P < 0.01) and social eating (P = 0.01). Significant moderate correlations with the OHRQoL score were detected in social eating and social contact (P < 0.01). Swallowing function showed a significant difference between G1 and G2 (P < 0.01). Logistic regression analyses revealed the significant associations of OHRQoL with swallowing function (odds ratio: 8.75, P < 0.01) and surgical reconstruction (odds ratio: 0.22, P = 0.04). CONCLUSION: Swallowing function, surgical reconstruction, and social activities were significantly associated with OHRQoL in patients with HNC with maxillary prostheses after maxillectomy.

    BACKGROUND: Although masticatory function (MF) in patients with prosthetic rehabilitation after mandibulectomy for oral cancer has been reported to be influenced by multiple factors, comprehensive evaluations of MF from clinical perspectives remain insufficient. PURPOSE: To comprehensively evaluate MF and its associated factors in patients with prosthetic rehabilitation after mandibulectomy for oral cancer. METHODS: The potential study participants were patients who had undergone mandibulectomy for oral cancer and received mandibular prosthetic rehabilitation. Age, sex, the number of remaining teeth and the number of total and posterior functioning occlusal supports were extracted. Information on cancer-related therapies (surgical procedure, radiotherapy, chemotherapy, glossectomy and neck dissection) and measurement values of oral functions (MF, oral moisture, maximum occlusal force (MOF), tongue-lip motor functions and maximum tongue pressure) were also extracted. Differences in MF were statistically evaluated according to cancer-related therapies. The Spearman correlation coefficients and logistic regression analysis were performed to evaluate associations between MF and these factors. RESULTS: A total of 41 patients were included in this study. MF was moderately and significantly associated with the number of remaining teeth, total and posterior functioning occlusal supports, MOF, and tongue-lip motor functions. More invasive cancer-related therapies significantly declined MF. Logistic regression analysis identified the presence of posterior functioning occlusal supports as a significant explanatory variable for MF. CONCLUSIONS: MF in patients with prosthetic rehabilitation after mandibulectomy was influenced by multiple factors. Notably, a crucial role of the presence of posterior functioning occlusal supports in MF was identified.

    上顎顎義歯装着患者の最大咬合力と残存歯数や咬合支持数などの患者プロファイルや口腔機能、さらには筋肉の切除、放射線療法、薬物療法などの腫瘍に対する治療歴との関連を後ろ向きに検証した。2018年4月~2022年9月に当科を受診し、良性または悪性腫瘍の切除により上顎欠損を有し、顎欠損部に顎義歯を装着している患者44名(男性23名、女性21名、年齢中央値70.5歳)を対象とした。最大咬合力と各因子とのSpearmanの相関係数について、残存歯数と機能的咬合支持数はそれぞれ0.57、0.50と有意な中程度の正の相関を認め、ODK/ta/と最大舌圧はそれぞれ0.36、0.35と有意な弱い正の相関を認めた。腫瘍治療歴と最大咬合力との間に有意な関連はみられず、ステップワイズ法により、咀嚼筋の切除、ODK/ka/、最大舌圧の3項目が説明変数として同定され、このなかで最大舌圧の調整オッズ比が有意であった。

    Masticatory function and maximum occlusal force (MOF) are prosthetic treatment outcomes. There have been a number of reports on factors related to masticatory function in maxillectomy patients with maxillofacial prostheses, but few of these describe factors related to maximum occlusal force. This study aimed to examine the association between MOF and other factors in maxillectomy patients with maxillofacial prostheses. Median values were calculated for patients’ data including oral functions. Correlations between MOF and factors of continuous variables were calculated. A logistic regression analysis was performed using MOF as an objective variable. The present study included 44 patients (male: 23 and female: 21). The median value of maximum occlusal force (388.4N) was higher than the threshold (500N) based on the concept of oral hypofunction. The present study indicated moderate correlations of MOF with the number of remaining teeth and the number of functioning occlusal supports. A comprehensive analysis of factors related to maximum occlusal force showed an association with maximum tongue pressure.

    年齢が20~49歳でEichner分類がAの患者を対象に、口腔機能を表す3種の指標を解析した。2020年4~11月に福岡市にある単一の開業歯科医院を受診した上記患者183名(女性133名、平均34.0±7.7歳)のデータを後ろ向きに抽出した。口腔機能の指標としては、最大咬合力(MOF)、最大舌圧(MTP)、咀嚼機能を測定した。咀嚼機能はグミゼリー(グルコラム)からのグルコース溶出濃度として計測した。口腔機能低下症の診断基準となる閾値として、MOFでは500N、MTPでは30.0kPa、咀嚼機能では100mg/dLという値を設定した。全集団における3指標の平均値はいずれも閾値より高くなった。個々の患者でみると閾値を下回った患者はMTPで最も多く30名に上り、MOFでは19名、咀嚼機能ではわずかに3名のみであった。20、30、40歳代の年齢層別で比較したが、どの口腔機能指標にも有意な差はみられなかった。また男女比較ではどの口腔機能指標も女性より男性の方が有意に高かった。2種の口腔機能指標が共に閾値を下回っていた患者は9名いた。いずれも女性患者であり、うち8名は歯列不正を伴っていた。以上のように、一部の患者では、不便を自覚していないにもかかわらず口腔機能、特にMOFとMTPの値が低下していた。歯列不正は口腔機能低下に関連していると考えられた。

    <p><b>Purpose:</b> The purpose of this study was to analyze oral functions (MOF: maximum occlusal force; MTP: maximum tongue pressure; and MF: masticatory function) in Eichner A subjects aged from 20 to 49 years.</p><p><b>Methods:</b> The data of three oral functions in Eichner A patients aged from 20 to 49 years were extracted. The mean values, subjects with lower values than the thresholds, and the differences among ages, and between sexes were investigated retrospectively.</p><p><b>Results:</b> Although the mean values of these oral functions in 183 subjects were higher than the thresholds, the numbers of the subjects with lower values than the thresholds were 30 in MTP, 19 in MOF, and only three in MF. No significant effects of ages on all oral functions were identified and all functions in males were significantly higher than those in females. The subjects whose two functions were lower than the thresholds were nine females, eight of whom were associated with malalignment.</p><p><b>Conclusion:</b> Although the mean values of oral functions were higher than the thresholds, some patients showed lower values, especially in MOF and MTP, even though they were unaware of any inconvenience. Patients with malalignment might be associated with declined oral functions.</p>

    The aim of this retrospective cross-sectional study was to comprehensively assess masticatory function in maxillectomy patients with functioning removable prostheses. Their general and oral profiles, the measurement values of their oral functions, including masticatory function, and the history of tumor therapy were extracted from medical charts. The correlations of masticatory function with numerical data and the effects of tumor therapy-related factors on masticatory function were evaluated. In addition, a stepwise conditional logistic regression analysis was performed to identify the potential predictive factors comprehensively. The data from 55 maxillectomy patients revealed that the median value of masticatory function (138.0 mg/dL) was higher than the threshold (100.0 mg/dL) based on the concept of oral hypofunction. Moderate correlations of masticatory function with the number of remaining teeth, the number of functioning occlusal supports, and maximum occlusal force were found, as well as a weak correlation with maximum tongue pressure. These variables also showed statistically significant coefficients (p < 0.01). No significant effect of each tumor therapy-related factor on masticatory function was detected. A logistic regression analysis identified the number of functioning occlusal supports as a significant predictive factor. These results implied the crucial interactions of masticatory function with various factors and the specificities of maxillectomy patients.

    口腔がんの外科的切除による歯や顎骨などの周囲組織の欠損は、審美性や咀嚼、嚥下、発音などの口腔機能を低下させ精神的苦痛を与える。顎欠損患者の口腔機能に関する報告は多くあるが、精神的苦痛などの心理状態の評価やそれらに影響を与える因子についての報告は少ない。そこで、顎欠損患者のストレスや不安を抱えた心理状態を精神・心理的フレイルととらえ評価を行うことで、これらに影響を与える因子、および口腔機能との関連を明らかにする。
