    Neutrophil extracellular traps (NETs) are a host defense mechanism whereby activated neutrophils release decondensed chromatin and antimicrobial proteins into the extracellular space to trap and kill invading pathogens. While effective in clearing pathogens, NETs also pose pathological risks by exposing self-DNA, histones, granular enzymes, and reactive oxygen species (ROS), contributing to pathologies such as autoimmune diseases, inflammatory disorders, and thrombosis. Here, we identify phospholipase D1 (PLD1), a lipid-signaling enzyme that generates phosphatidic acid (PA), as a critical regulator of ROS generation and NET formation in murine neutrophils. Using both PLD1-deficient neutrophils and a selective inhibitor, we demonstrate that PLD1 is essential for NET release. Notably, exogenous PA alone is sufficient to trigger robust ROS production and NET formation. In vivo, PLD1-deficient mice fail to generate ROS in an acute lung inflammation model and are protected from venous thrombosis. These findings identify PLD1 and PA as key upstream regulators of NET formation and suggest that pharmacological inhibition of PLD1 could provide a potential avenue for early intervention in NET-related diseases such as venous thrombosis.

    Bone regeneration remains a significant challenge in regenerative medicine. In this context, fibrin hydrogels have attracted attention as a promising biomaterial that regulates the inflammatory response and promotes tissue repair by influencing macrophages. In this study, we investigated the immunomodulatory effects of fibrin hydrogels on macrophage polarization and their subsequent impact on bone regeneration. It is widely recognized that M1 macrophages produce tumor necrosis factor alpha (TNF-α), while M2 macrophages produce interleukin-10 (IL-10). When undifferentiated mouse bone marrow-derived macrophages were stimulated with lipopolysaccharides (LPS), a marked increase in the proinflammatory cytokine TNF-α was observed. However, coculture with fibrin hydrogels in the presence of LPS significantly suppressed TNF-α production while enhancing the secretion of the anti-inflammatory cytokine IL-10. Furthermore, in a rat calvarial defect model, tissue analysis 1-week postimplantation of fibrin hydrogels revealed an upregulation of M2 macrophage markers (CD163, CD204, and CD206), indicating a shift toward an anti-inflammatory phenotype. Notably, 11 weeks after implantation, the fibrin hydrogel-treated sites exhibited enhanced bone regeneration. These findings highlight the potential of fibrin hydrogels as an immunomodulatory biomaterial that facilitates bone repair by promoting M2 macrophage polarization and modulating the local inflammatory microenvironment.

    <p><b>抄 録:</b>歯内療法領域で行われる各処置においてバイオセラミックス系材料が有用であることは,歯内療法専門医のみならず,多くの歯科医師に認知されるようになった.その代表格としてMineral Trioxide Aggregate(MTA)が挙げられるが,MTAが臨床応用されるようになって以降,さまざまなバイオセラミックスを構成成分とする歯科材料が開発・製品化され,近年では「バイオセラミックス系」あるいは「バイオセラミックス配合」とされる数多くの歯内療法用材料を手に取ることができるようになっている.一般に,歯科医療で用いられる材料の利点・長所を十分に引き出すには,材料の開発目的,目的を達成するために材料に配合されている主要な成分,および主要成分が示す機能を知っておく必要がある.バイオセラミックス系あるいはバイオセラミックス配合の歯内療法用材料を使用する際も,構成要素であるバイオセラミックスを理解し,臨床においてどのように機能するのかを知ることで,材料の長所を十分に引き出すことが可能になる.本稿では,バイオマテリアルという範疇でのバイオセラミックスの位置付け,歯内療法用材料に応用されているバイオセラミックスの特徴,および生体(象牙質)との反応について概説し,歯内療法,特に根管充塡法にバイオセラミックス系材料がどのようなインパクトを与えたかについて考察する.</p>

    <p> Purpose: Bioceramics-based root canal sealers are becoming essential in modern endodontics worldwide and are currently being used in Japan; however, there have been no follow-up studies on teeth treated using these sealers. The purpose of this study was to retrospectively evaluate 3-year follow-up data pertaining to teeth treated with a bioceramics-based root canal sealer.</p><p> Methods: Patients treated at the Division of Tooth Therapy (Endodontics and Restorative Dentistry) of Kyushu Dental University Hospital and a private dental clinic were recruited, and 127 teeth that were obturated with a bioactive glass-based root canal sealer were followed up for 3 years. The rates and incidence of functionally surviving and extracted teeth were analyzed. Additionally, 56 teeth with 3-year follow-up radiographs were assessed using the periapical index (PAI) score.</p><p> Results: One hundred and fifteen teeth (91%) survived functionally, and 12 teeth (9%) were extracted during the 3 years. The 12 extracted teeth were initially diagnosed as non-preservable by the dentist because of root cracks or fractures and extensive bone defects, but were endodontically treated in accordance with the patients’ strong requests. There were significant associations between functional surviving and extracted teeth and tooth type, subjective symptoms, initial treatment/re-treatment, and cracks/fractures. In contrast, there was no significant difference between surviving and extracted teeth according to the root canal obturation technique, pain immediately after root canal obturation, and overfilling of the root canal. In the comparison according to PAI scores, the rates of scores 1 and 2 increased, whereas those of scores 3 and 4 decreased.</p><p> Conclusion: This 3-year retrospective follow-up study of root canal-obturated cases revealed that the bioactive glass-based root canal sealer evaluated herein, a bioceramics-based sealer, can be applied to achieve favorable outcomes in endodontic therapy.</p>

    生体活性ガラス系根管シーラーの製品であるニシカキャナルシーラーBGで根管治療を行った歯の3年経過を後方視的に調査した。当歯科大学病院と一歯科診療所で上記の治療を行った127歯を調査対象とした。このうち89歯(70%)は女性患者の歯で、全患者の年齢範囲は10~89歳であった。3年後までの経過を調べた。その結果、12歯(9%)はこの期間中に抜歯されており(抜歯群)、残り115歯は機能的に生存していた(生存群)。抜歯群の歯はいずれも、歯根に亀裂または破折、および広範な骨欠損が存在していたため本来は歯科医によって温存不可と診断されていたが、患者の強い要望により根管治療を実施した歯であった。機能的生存か抜歯かという転帰と有意に関連していた因子は、歯の種類、主観的症状、初回治療か再治療か、亀裂か破折か、であった。生存群と抜歯群の間では、根管充填法の違い、充填直後の疼痛、過剰充填、に応じた有意差は示されなかった。放射線学的に3年経過を観察できていた56歯でperiapical index(PAI)スコアを評価した。その結果、スコアが1と2の割合は増加し、3と4の割合は減少していた。3年経過を後方視的に評価した本研究の結果から、歯内療法で良好な成績を達成するために生体活性ガラス系根管シーラーのニシカキャナルシーラーBGを適用できると結論された。

  • A Questionnaire Survey on the Effects of Experience-led Learning with Scenario-based Pre-clinical Training for Basic Education of Operative Dentistry and Endodontics on Pre- and Post-graduate Dental Education Reviewed

    <p> Purpose: At Kyushu Dental University (KDU), third-grade students of the dental school attend “Tooth Therapeutics,” which includes operative dentistry and endodontics, one of the basic clinical dentistry courses. In this course, the experience-led learning method with scenario-based pre-clinical training including some of the content of other courses is carried out. The purpose of this study, which was conducted by a questionnaire survey, was to verify the effects of the experience-led learning with scenario-based pre-clinical training on clinical clerkship and the clinical residency program as pre- and post-graduate education.</p><p> Materials and Methods : The targets of this questionnaire survey were dental residents who completed the clinical residency program at KDU Hospital during the 3 fiscal years from 2016 to 2018. On the final day of the residency program in each year, the survey was carried out after explaining an outline of the experience-led learning with scenario-based pre-clinical training.</p><p> Results: The questionnaire collection rate was 100% (168 dental residents). Of the dental residents who graduated from KDU, 129 (91%) were educated by the experience-led learning with scenario-based pre-clinical training, and 13 (9%) by conventional specialized practical training. Of the dental residents who graduated from other universities, 0 (0%) were educated by the experience-led learning with scenario-based pre-clinical training, 23 (88%) by conventional specialized practical training, and 3 (12%) by conventional specialized practical training in cooperation with other courses. To the question about looking back at the training contents, the rate who answered, “I could imagine the training as real clinical practice,” was higher in dental residents educated by the experience-led learning with scenario-based pre-clinical training than in those educated by conventional specialized practical training. To the question about the effect of basic clinical dentistry on pre- and post-graduate education, 151 (90%) answered “basic clinical dentistry classes were beneficial,” with or without the education by the experience-led learning with scenario-based pre-clinical training.</p><p> Conclusion: The results of the questionnaire survey, in which most of the survey targets (91%) were educated by the experience-led learning with scenario-based pre-clinical training, suggest that the experience-led learning with scenario-based pre-clinical training is beneficial for acquiring knowledge and clinical techniques during clinical clerkship and the clinical residency program as pre- and post-graduate education.</p>

    組織再生において欠損部での炎症制御は必要不可欠なことである。近年、M2マクロファージ誘導能が明らかにされたフィブリンゲルに組織再生に適した環境を構築する役割が期待されているが、フィブリンゲルによるM2マクロファージ誘導メカニズムや硬組織への影響は明らかにされていない。一方、ハイドロキシアパタイト形成を誘導するバイオセラミックスの生体応用が実現しつつある。本研究の最終目標はフィブリンゲルを用いた根尖歯周組織再生療法の確立であり、マクロファージや根尖歯周組織に及ぼすフィブリンゲルの影響を明らかにした上で、バイオセラミックスをフィブリンゲルに配合した新規バイオマテリアルを作製することである。

    感染・炎症によって拡大した根尖部骨欠損は、その直径がクリティカルサイズ(10 mm)を超えると治療しても瘢痕化しやすいことから、骨再生を誘導する各種細胞の足場となる再生医用材料の応用が注目されてきた。一方で、炎症制御が再生誘導に不可欠であることが知られており、局所の状態を炎症・免疫応答が亢進しているステージから組織再生可能なステージへと移行させなければ再生医用材料を欠損部局所に埋入しても組織は再生されない。本研究の最終目標は、再生医用材料を適確に機能させるため、M2マクロファージを誘導するフィブリンゲルを用いて炎症優位な状態から組織再生可能な状態に局所環境を転換する方法の確立である。
