    Aim
    Total hip arthroplasty can effectively improve patients' motility with end-stage osteoarthritis. This study aimed to: (1) compare gradual changes in utility values with total hip arthroplasty and estimated values without; (2) evaluate total hip arthroplasty cost-effectiveness; and (3) evaluate cost-effectiveness by age, diagnosis, and comorbidity.
    Methods
    Patients who underwent total hip arthroplasty between January 2008 and December 2009 were included. Patients completed the EuroQol preoperatively and at 1, 3, 5 and 7 years postoperatively. To derive the quality-adjusted life years gained, a utility score was obtained from the EuroQol item scores and combined with 7 years, and estimates were obtained by discounting the postoperative 1-year utility value at an annual rate of 2%–4%. Mixed-effects regression models were used to compare the estimated and the measured utility values.
    Results
    Mean total cost was 1,921,849 yen, and quality-adjusted life years gain score was 1.746 with per cost as 1,100,715 yen. Compared with actual measurements, the estimated values from 1 to 7 years post-surgery differed significantly, and interaction was observed. Regarding age, the older the patient, the higher the cost per quality-adjusted life years. Patients with lower preoperative physical function had higher quality-adjusted life years gains, while the cost per quality-adjusted life years was lower.
    Conclusions
    Total hip arthroplasty was cost-effective. Compared with actual measurements, the estimated utility values from 1 to 7 years post-surgery significantly differed. Even among older patients and those with impaired preoperative physical functions, its cost was lower than patients' willingness to pay in Japan.

    The contribution of physical activity (PA) to the prevention of metabolic abnormalities following liver transplantation (LT) has not been well documented. We aimed to assess PA in post‐LT patients and to quantify its relationships with the development of postoperative metabolic abnormalities and health‐related quality of life (HRQOL). We recruited 111 patients who had undergone LT ≥ 6 months previously. PA was measured by accelerometry, and HRQOL was evaluated using SF‐8. PA was quantified as the number of steps per day, and the time spent performing moderate‐to‐vigorous PA and light PA per week. The prevalence of hypertension, diabetes, and dyslipidemia increased more than twofold following LT. The proportion of the participants with a sedentary lifestyle (<5000 steps/day) was 36%. Logistic regression analysis showed that postoperative hypertension and obesity were associated with preoperative body mass index and the number of steps taken (in 2000 steps/day increments). Preoperative diabetes was associated with obesity, and PA was associated with physical function‐related HRQOL scores. Thus, increasing the number of steps taken per day has the potential to reduce hypertension and obesity, and PA could improve physical function‐related HRQOL in patients following LT.

    BACKGROUND:In non-Western countries including Japan, activities requiring deep flexion of the hip joint, such as seiza (kneeling with calves tucked under the thighs and buttocks resting on the heels) and using squat toilets are commonly practiced. The purpose of this study was to assess longitudinal changes in traditional health-related quality of life measures and measures of physical functions associated with lifestyle for Japanese patients pre-surgery and after total hip arthroplasty.
    METHODS:Consecutive primary total hip arthroplasty patients between July 2003 and November 2006 were eligible. Patients were measured preoperatively and at 6 weeks, 1 year and 3 years postoperatively. Patients completed the EuroQol 5D, the Western Ontario and McMaster Universities Osteoarthritis Index and items related to Japanese lifestyle activities such as squatting. Changes in these scale scores across the four time points were tested, and we examined predictive factors of EuroQol 5D score at 3-year follow-up using multiple linear regression.
    RESULTS:Of 1103 eligible patients, 576 completed questionnaires at all four time points. By 6 weeks post-surgery, reductions in pain and improvements in physical function and stiffness became highly significant, and improvements continued to 3 years postoperative. In contrast, improvements were far more limited for items related to Japanese lifestyle functions such as seiza and use of a Japanese squat toilet, even 3 years after surgery. Predictive factors of EQ5D at 3-year post THA were WOMAC pain and physical function, seiza, age and comorbidity measured at 3-year post THA.

    CONCLUSION:

    The rate of improvement in QOL requiring deep flexion of the hip joint was much slower than that in QOL related to Western lifestyle. Our study suggests a need for lifestyle modification for THA patients in the other countries where kneeling and squatting are commonly performed.

    Aim: Nurse-led lifestyle education may be particularly effective for promoting light-intensity physical activity (LPA), which is often included as part of patients' daily activity. We aimed to conduct a comprehensive review of associations between LPA and health-related outcomes among cancer survivors and clarify the effects of LPA. Methods: We searched four databases to identify relevant studies that involved participants aged ≥18 years, classified physical activity based on intensity, and measured LPA during the whole day by accelerometers or questionnaires. The quality of the included studies was assessed using quality assessment tools specific to LPA studies. The results were synthesized narratively. Results: We extracted 34 relevant studies. Eleven studies focused on multiple cancers, eleven on breast cancer, five on colorectal cancer, three on lung cancer, and one each on head and neck cancer, non-Hodgkin's lymphoma, prostate cancer, gynecological cancer, and colorectal and gastric cancer. Six studies reported that LPA was effective for improving fatigue. Although LPA also showed favorable associations with physical function and mortality, the number of studies was insufficient (four studies each). No association was found between LPA and adiposity (n = 5). Results for the associations between LPA and quality of life (QOL) (n = 12), depression (n = 4), and cognitive function (n = 3) were conflicting. Conclusion: This review confirmed that LPA is effective in improving several health-related outcomes. However, the results are integrated across cancer types, and further studies should investigate each outcome for each cancer type to clarify the effect of LPA.

    BACKGROUND: Health-related quality of life (HRQoL) following total hip and knee arthroplasty (THA/TKA) may decline more in Asian countries due to lifestyle and housing factors compared with patients in Western countries. AIMS: To evaluate the psychometric properties of the Japanese version of 15D and clarify the characteristics of HRQoL in Japanese patients who underwent THA/TKA compared with Finnish patients. METHODS: This longitudinal study included 182 patients between 2019 and 2021. Patients' HRQoL was measured using the 15D preoperatively and 3 months postoperatively. Student's t-tests were used to compare lifestyles between Japan and Finland. RESULTS: The Japanese version of the 15D showed good internal consistency, reliability, construct validity, and responsiveness. Patients with many steps inside their house had worse HRQoL in the dimensions of mobility, distress, and vitality in Japan. There were significant differences in mobility, distress, vitality, and sexual activity dimensions between Japan and Finland 3 months postoperatively. CONCLUSIONS: The lower HRQoL in Japan in the dimensions of mobility, distress, and vitality is likely due to Japan's traditional housing structure with many steps. Identifying differences in cultural and psychological characteristics, as well as by residential setting, will enable health care providers to help patients improve their HRQoL.

    Introduction: Depression prevention is crucial for maintaining both physical and mental health and enhancing individuals’ quality of life (QOL) post-transplantation. Despite the heightened susceptibility of post-transplant patients to COVID-19 infection, the relationship between depression and QOL during the pandemic remains unclear. This study aimed to (1) identify depression and QOL during the pandemic in kidney and liver transplant patients, and (2) explore the relationship between QOL and depression. Design: This cross-sectional study involved patients aged 20 years or older who were at least 6 months post-transplantation. Depression and QOL were assessed using the Patient Health Questionnaire-9 and the 8-item Short-Form Health Survey, respectively. Logistic regression analysis was conducted to examine the relationship between QOL and depression. The study included 204 participants comprising 68 postkidney transplant and 136 postliver transplant patients. Results: Among all participants, depressive symptoms were present 30% of the time, with 8.3% experiencing moderate or severe depression. Logistic regression analysis revealed that depression (mental component summary; OR = 2.419, P < .001, 95% CI: 1.862–2.915) and postoperative period (physical component summary; OR = 1.397, P = .008, 95% CI: 1.094–1.733) significantly influenced QOL. Conclusion: This study suggests that the prevention of depression in emerging infectious disease pandemics may contribute to maintaining the mental QOL of transplant recipients.

    Abstract

    Aim

    Advanced Practice Nurses are expected to provide lifesaving care to patients with complex acute illnesses in emergency and critical care settings. However, little is known about their competencies and barriers to practice in emergency and critical care settings. This review investigated these nurses' competencies to practice.

    Methods

    A scoping review was conducted in accordance with Arksey and O′Malley's framework. Extensive research searches were conducted using seven electronic databases: MEDLINE, CINAHL, Scopus, Web of Science, Ichushi Web, Mednar and GreyNet International. Definitions and explanations of Advanced Practice Nurse competencies were categorized into elements and grouped according to similarity.

    Results

    The database searches identified 2,483 studies, and data were extracted for 23 studies. Analysed studies were published between 2000 and 2021 and conducted in eight countries. Seven competencies were identified: performing advanced practice nursing, acute patient care, diagnostic assessment, interdisciplinary collaboration and consultation, leadership and system management, documenting patient care and supporting patient and family decision‐making.

    Conclusion

    This review identified competencies unique to Advanced Practice Nurses in emergency and critical care settings. Further research is required to facilitate understanding of the crucial roles of advanced care nurses among healthcare providers.

    Background: Available evidence on outcomes at 5 years and beyond for total hip arthroplasty (THA) is mainly limited to patient-reported outcome measures (PROMs). This study documented the trajectory of functional measurement using the Oxford hip score (OHS) and floor-sitting posture in Japan for up to 10 years post- THA and investigated predictors of dissatisfaction at 10 years with THA.Methods: Patients scheduled for primary THA at a university hospital in Japan between 2003 and 2006 were enrolled in this prospective study. Overall, 826 preoperative participants were eligible for follow-up, with response rates ranging from 93.6 to 69.4% at each postoperative survey point. The OHS and floor-sitting scores were calculated using a self-administered questionnaire six times up to 10 years postoperatively. Patient satisfaction, including general surgery, walking ability, and activities of daily living (ADL), was assessed in the 10-year survey.Results: The linear mixed-effects model demonstrated a postoperative improvement, peaking at 7 years for OHS and 5 years earlier for the floor-sitting score. The postoperative overall surgical dissatisfaction at 10 years with THA was very low (3.2%). No predictors of surgical dissatisfaction were identified in the logistic regression analyses. Predictors of walking ability dissatisfaction were older age, men, and worse OHS at 1 year postoperatively. Predictors of ADL dissatisfaction were poorer preoperative and 1-year postoperative floor-sitting scores and 1-year postoperative OHS.Conclusion: The floor-sitting score is a simple PROM suitable for the Japanese population; other populations would require a scale suitable for their lifestyles.Keywords: Oxford hip score; floor-sitting score; postoperative satisfaction; total hip arthroplasty.

    Abstract

    Aim

    This study aimed to validate the revised Short Questionnaire to Assess Health‐Enhancing Physical Activity (SQUASH) to measure sedentary activity in post‐liver‐transplant patients. The proposed scale could be useful for transplantation nurses to assess and modify sedentary lifestyles and increase physical activity.

    Methods

    The SQUASH was modified to include items on sitting time and light‐intensity physical activity (LPA‐SQUASH). A pilot study was conducted with 20 liver transplant patients, and an expert panel validated the scale contents. Then, post‐liver‐transplant outpatients at a Japanese university hospital participated in the main study (September–October 2020), in which questionnaires were mailed twice to assess test–retest reliability, and accelerometers used to establish criterion validity. Intra‐class correlation coefficients (ICC) were calculated for test–retest reliability. Spearman correlations and Bland–Altman plots were used to assess validity and measurement error.

    Results

    In total, 173 participants returned the questionnaires, and 106 and 71 completed the reliability and validation studies, respectively. The range of LPA‐SQUASH correlation coefficients for test–retest was .49–.58. ICCs ranged from .72 to .80 for items other than leisure. Accelerometer data and the LPA‐SQUASH total physical activity amount and light‐intensity physical activity correlated moderately.

    Conclusion

    We modified the SQUASH, which was developed to measure physical activity in healthy adults, to assess light‐intensity physical activity in post‐liver‐transplant patients. The LPA‐SQUASH showed acceptable validity and reliability. The questionnaire may be used by transplantation nurses to examine light‐intensity physical activity content/duration, deliver patient education considering patients' sedentary lifestyle, and facilitate goal setting for physical activity interventions to prevent metabolic syndrome.

    Purpose: Studies on physical activity (PA) after revision total hip arthroplasty (THA) are limited. It is important to assess PA levels as well as improvements in physical function and pain after revision THA. The purpose of the study was to compare accelerometer-measured PA and health-related quality of life (HR-QoL) in patients 1-3 years after revision or primary THA. We also clarified the relationship between measured PA and HR-QoL.

    Methods: This cross-sectional study included 64 patients who underwent revision THA and 188 who underwent primary THA 1-3 years earlier. Outcome measures were accelerometer-measured light PA, moderate-to-vigorous intensity PA, number of steps, questionnaire-based Oxford hip score, SF-8 physical and mental component scores. We conducted propensity-score matching for age, sex, comorbidities, body mass index, and postoperative follow-up duration and compared PA levels and HR-QoL scores between patients after revision THA and primary THA of each 50 patients.

    Results: Based on propensity scores in the revision THA (68.3 years) and primary THA (67.8 years) groups, light PA, number of steps, and HR-QoL scores 1-3 years after revision THA were significantly lower than those after primary THA (P < 0.05). Moderate-to-vigorous intensity PA did not different between groups (P = 0.204). Measured light PA, moderate-to-vigorous intensity PA, and number of steps after re-THA were moderately correlated with HR-QoL scores.

    Conclusion: Increasing the time and frequency spent on light PA to the levels after primary THA may lead to an increase in overall PA levels after revision THA. A detailed understanding of PA, including light PA, in daily life and an approach that promotes activity levels are necessary.

    <p>Objective: This study investigated and described the learning needs experienced by family caregivers in the Japanese working generation who care for community-dwelling people with dementia (PWD). Methods: This qualitative descriptive study conducted semi-structured in-depth interviews to explore the learning needs experienced by the family caregivers of PWD. 18 family caregivers were recruited via purposive sampling (age range 30-60 years) from two medical facilities, two self-help groups, and 10 prefectural-sponsored counseling facilities in Japan. The collected data were subjected to a qualitative content analysis. Then, the derived learning needs were independently reviewed by two nursing researchers with expertise in qualitative research and family nursing. The Standards for Reporting Qualitative Research (SRQR) recommendations were followed. Results: Four main categories and 20 subcategories were derived to clarify the learning needs of family caregivers in the Japanese working generation who care for community-dwelling PWD. The four main categories included (a) increasing long-term care literacy, (b) dispelling the negative images associated with dementia and long-term care, (c) working on long-term care as a unified team that handles issues concerning caregiving for dementia, and (d) promoting an active role as a member of society. Conclusions: This study clarified specific learning needs that family caregivers in the Japanese working generation experience when caring for community-dwelling PWD, thus, contributing to a more thorough understanding of the associated challenges. In addition, the structured list of learning needs should aid health professionals when conducting assessments and planning interventions, especially when framing these issues from the cultural perspective.</p>

    Abstract

    Aim

    Total hip arthroplasty can effectively improve patients' motility with end‐stage osteoarthritis. This study aimed to: (1) compare gradual changes in utility values with total hip arthroplasty and estimated values without; (2) evaluate total hip arthroplasty cost‐effectiveness; and (3) evaluate cost‐effectiveness by age, diagnosis, and comorbidity.

    Methods

    Patients who underwent total hip arthroplasty between January 2008 and December 2009 were included. Patients completed the EuroQol preoperatively and at 1, 3, 5 and 7 years postoperatively. To derive the quality‐adjusted life years gained, a utility score was obtained from the EuroQol item scores and combined with 7 years, and estimates were obtained by discounting the postoperative 1‐year utility value at an annual rate of 2%–4%. Mixed‐effects regression models were used to compare the estimated and the measured utility values.

    Results

    Mean total cost was 1,921,849 yen, and quality‐adjusted life years gain score was 1.746 with per cost as 1,100,715 yen. Compared with actual measurements, the estimated values from 1 to 7 years post‐surgery differed significantly, and interaction was observed. Regarding age, the older the patient, the higher the cost per quality‐adjusted life years. Patients with lower preoperative physical function had higher quality‐adjusted life years gains, while the cost per quality‐adjusted life years was lower.

    Conclusions

    Total hip arthroplasty was cost‐effective. Compared with actual measurements, the estimated utility values from 1 to 7 years post‐surgery significantly differed. Even among older patients and those with impaired preoperative physical functions, its cost was lower than patients' willingness to pay in Japan.

    Background / Regular physical activity (PA) is important for maintaining mental and physical health after liver transplantation (LT); however, the fluctuations in routine PA during COVID-19 and its putative impacts are currently unknown. This study examined the changes in PA during the COVID-19 pandemic and explored its association with fear and depression during the pandemic. / Material/Methods / This longitudinal study included 83 LT patients whose PA was measured using the short form of the International Physical Activity Questionnaire before and during COVID-19. Fear of COVID-19 was estimated based on previous studies, and depression was assessed using the Patient Health Questionnaire-9. Participants were also asked about important sources of information on COVID-19. PA was classified as inactive or active depending on the changes in PA, and logistic regression analyses with PA as a dependent variable were conducted to explore the associations among PA, depression, and fear of COVID-19. / Results / Moderate and high PA exhibited decreasing trends before and during the COVID-19 pandemic, especially in males. Fear of being infected with SARS-CoV-2, the virus that causes COVID-19, while shopping was significantly higher in females and was significantly independent of inactivity during the COVID-19 pandemic. Only 1 patient reported that their transplant center was their main source of information about COVID-19. Only 4.9% of the LT participants were depressed. / Conclusions / Our study results indicate the need to support the provision of accurate information about COVID-19 by health care professionals in transplant centers, especially for patients with low PA, to prevent PA decline in LT patients.

    This study aimed to review the validity and/or reliability of light-intensity physical activity (LPA) questionnaires and identify the most suitable questionnaires for measurement of LPA in adults. Following the PRISMA-P 2020 guidelines, we searched MEDLINE, PsycINFO, CINAHL, Scopus, Embase, and MedNar. Only studies that targeted adults ≥18 years old and used LPA measured by accelerometer and/or heart rate monitor as an objective criterion were included. The search resulted in 2748 article hits, from which we extracted 16 studies with 14 questionnaires. The 7-Day Sedentary and LPA Log and LPA Questionnaire were specifically designed for LPA measurement, and the Community Health Activities Model Program for Seniors physical activity self-report questionnaire scale has been revised for LPA measurement. These questionnaires had comparatively high reliability and validity in this review. Most studies contained methodological limitations such as test-retest period. In the future, more accurate reliability/validity studies should be conducted for each questionnaire.

    目的:糖尿病性腎症患者を対象に,6年間の後方視的調査を行い,チーム医療による個別的な予防指導と腎機能や血糖管理などの評価指標との関連について明らかにする.方法:腎症2期以降の患者について,診療記録を用いて個別予防指導群(介入群)と通常診療群(非介入群)で評価指標を比較し,HbA1c,eGFR,血圧を従属変数とした多重ロジスティック回帰分析を行った.結果:分析対象者は介入群224名,非介入群209名であった.ベースライン(開始時)において,HbA1cは介入群7.5(6.8-8.9)%,非介入群7.2(6.6-8.1)%で介入群の方が有意に高かったが(p<0.05),観察期間終了時,介入群では有意に改善を認めた(p<0.01).また,介入回数が多いことがHbA1cの維持・改善に有効で,eGFRはベースライン時腎症3期以降では悪化を認めた.結論:予防指導は腎症進行例では限界を示したが,血糖管理には有効であることが示唆された.(著者抄録)

    The aims of this study were to generate a written notification message using social marketing methods to encourage examinees with abnormal glucose levels to seek medical care and to evaluate the optimized message. A semi-structured interview was conducted among examinees with abnormal glucose levels who had never visited a medical clinic for diagnosis of hyperglycemia. Patients were divided into two segments. A tailored notification letter was developed according to these segments using a social marketing approach. Other examinees were prospectively enrolled as the control group. The tailored notification for Segment 1 included a web code to access a health care provider. Future loss of time and money was emphasized in the message for Segment 2. According to responses in a telephone interview, 654 examinees with abnormal fasting glucose levels in a health checkup were divided into Group 1 (n = 178), Group 2 (n = 24), and a control group (n = 452) and given a tailored or conventional notification letter. The proportion of examinees who actually visited a health care provider in Group 1 (20.8%) was significantly higher than that in the control group (11.1%); no examinees in Group 2 visited a health care provider. Receipt of a tailored notification was a significant factor associated with visiting a health care provider, independent of age, sex, or severity of glucose abnormality (odds ratio 1.77; p = 0.02). Our results showed that tailored notification developed using a social marketing approach prompted health checkup examinees with abnormal glucose levels to seek medical care.

    Aim: To evaluate the effects of an intensity display type accelerometer on diabetic patients' physical activity.

    Methods: This was a two-arm, non-randomized controlled study. Both groups received information about the recommendation of 150 min/week moderate-to-vigorous physical activity (MVPA). The intervention group used an intensity display type accelerometer to monitor their physical activity intensity for 10 days at baseline and 3 months later. We compared intervention and control groups after 3 and 6 months. Primary outcomes were MVPA and number of steps over 7 days. Secondary outcomes were glycosylated hemoglobin (HbA1c), body mass index, and self-management.

    Results: Of 62 participants, 30 and 32 were included in the intervention and control groups, respectively. Mean age in each group was 59.7 ± 10.8 and 58.8 ± 10.2 years, and mean HbA1c was 6.9 ± 0.9% and 6.9 ± 0.8%, respectively. There were no significant differences between the intervention and control groups at either time point, and no outcomes showed significant changes. In a subgroup analysis by physical activity intensity, MVPA of active individuals in the control group significantly decreased at 6 months from baseline. MVPA and number of steps among inactive individuals in the intervention group significantly increased at 6 months from baseline. Self-management of the intervention group showed a trend toward improvement, but HbA1c and body mass index showed no significant change.

    Conclusions: Monitoring physical activity intensity led to increased MVPA of inactive patients and maintained MVPA of active patients with diabetes mellitus. This straightforward intervention could be applied in clinical practice.

    The current study aimed 1) to describe changes in patient-reported outcomes and physical activity measured with an accelerometer preoperatively, 6 months and 2 years postoperatively in older patients undergoing total knee arthroplasty (TKA) for arthritis, and 2) to examine the predictors of the changes in physical activity (PA). This study included 58 patients (mean age 72.6 years, 84.5% women) who completed the Oxford Knee Score (OKS) and the 8-item Short Form Health Survey. Physical activity measured mean steps per day, duration of light physical activity and moderate-to-vigorous physical activity (MVPA) per week. All PA indicators and patient-reported outcomes improved 6 months postoperatively. After 6 months, knee-related pain and function gradually improved, and MVPA increased. The OKS was a sole predictor of improvement in PA during the 2-year study period, suggesting the importance of disease-specific quality of life.

    Aim: The hospital length of stay for orthopedic surgery has been decreasing during the last couple of decades. Therefore, this study was performed to explore the postoperative experiences of adult/older patients (age ≥20 years) with osteoarthritis who underwent total hip or knee arthroplasty, focusing on the first 6 weeks following discharge. Methods: A systematic literature search on qualitative studies was conducted using six databases, such as Medline, CINAHL and Mednar. Verbatim interview data and themes or subthemes related to the patients' experience after discharge were extracted. Content analysis was used to code interview data. Codes similar in meaning were grouped, and subcategories were formed. These subcategories were then grouped into categories. Results: Sixteen qualitative studies with 253 participants were analyzed. In total, 136 codes were generated and formed 29 subcategories. Six categories were generated: (a) postoperative pain and medication; (b) difficulty in performing activities of daily living; (c) appreciation for support and difficulties associated with receiving support; (d) variability in recovery process and information-seeking; (e) lack of patient-centered care; and (f) transportation problems and social isolation. Conclusion: Our review suggests that prospective patients and their caregivers need individually tailored presurgical education and advanced planning for postsurgical reduced mobility.

    Aim: The aims of this study were to: (a) describe the physical activity (PA) and quality of life (QOL) in living donor liver transplant (LDLT) recipients pre-operatively and at 3 months and 6 months post-operatively; (b) compare PA and QOL at 6 months post-operatively with a healthy control group; and (c) explore pre-operative factors that predict PA changes. Methods: Patients over 20 years of age who were undergoing LDLT were recruited. PA was measured based on the number of steps/day and time spent performing moderate-to-vigorous PA (MVPA) during 1 week using an accelerometer. QOL was assessed based on a physical (PCS) and mental (MCS) component summary of the eight-item Short-Form Health Survey. The LDLT and healthy control groups were matched for age (±3 years) and gender. Pre-operative factors predicting a change in PA were calculated using a generalized linear mixed model. Results: Twenty-four patients completed the study. By 6 months post-LDLT, the MCS and PCS were comparable to those in the control group. The number of steps (3,887 steps/day) and MVPA (29.3 min/week) showed significant improvement by 6 months post-operatively, but remained much lower compared with those in the control group. The multivariate analysis showed that younger age (p <.01, p =.04) and higher skeletal muscle mass (SMM; p <.01, p =.03) were predictors of improvement in number of steps and MVPA. Conclusion: This study suggests the need for pre-operative interventions by healthcare professionals that focus on outcomes such as improving low SMM to facilitate post-operative PA recovery.

    Aim: This study aimed to develop the empowerment scale for family caregivers of community-dwelling people with dementia (PWD) in Japan (EFCD) and to validate the scale. Methods: The questionnaires were mailed to 820 family caregivers of PWD. The first version of the EFCD based on interviews with family caregivers and elderly care specialists and content validity results was tested. Participants also completed the Revised Scale for Caregiving Self-Efficacy and the General Health Questionnaire. For the EFCD development procedure, construct validity was evaluated by item analysis and exploratory factor analysis. Criterion validity was tested using Spearman's correlations between scores of the three scales. Reliability was evaluated using Cronbach's alpha, intraclass correlation coefficients (ICC), and Bland and Altman analysis. The final model was verified by confirmatory factor analysis. Results: Data from 304 family caregivers were analyzed. Exploratory factor analysis identified a 16-item, four-factor structure for the final version of the EFCD, as follows: (a) Excellent Practice in Dementia Care; (b) Understanding the Essence of Dementia Care; (c) Caring for Oneself as well as for the Person with Dementia; and (d) Having Peers with Shared Support Activities. Reliability and validity of the scale was established using the methods described. Conclusions: The developed EFCD is a reliable and valid measure that provides a simple assessment of empowerment among family caregivers of PWD.

  • The Effect of Preventive Oral Care on Postoperative Infections After Head and Neck Cancer Surgery Reviewed

    Objective: This study aimed to investigate the incidence of postoperative pneumonia (PP) and surgical site infection (SSI) in head and neck cancer (HNC) patients and clarify the relationship between oral care and postoperative infection.
    Methods: We conducted a retrospective observation survey based on the medical records of 209 HNC surgery patients managed at a University Hospital in 2016-2018. The incidence of PP and SSI were assessed in patients who underwent operations of the nose and paranasal sinuses to the larynx. Factors associated with PP and SSI in a univariate analysis were included in a multiple logistic regression analysis. A Cox proportional hazards model was used analyze the incidence of PP according to time after surgery. The present study was approved by the ethical review board of our Institute.
    Results: The rates of PP and SSI in our study population were 20.5% and 23.0%. Operative time (P < 0.01), blood loss (P = 0.004), tracheostomy (P < 0.01), reconstruction (P < 0.01), and preoperative plaque control record (PCR) (P < 0.01) were significantly associated with PP. The PCR depicted the oral hygiene based on the percentage of plaque attached to the tooth neck. A multiple logistic regression analysis indicated that the incidence of PP was significantly higher in patients with PCR values of ≥50% after preoperative oral care (OR=10.174, 95% CI 2.14-48.32, P = 0.004). Tracheostomy (P < 0.01), reconstruction (P = 0.044), a lower preoperative albumin level (P = 0.019), and a lower preoperative hemoglobin level (P < 0.01) were significantly associated with SSI.
    Conclusions: The incidence of PP among patients who received oral care was high in those patients with high PCR values, indicating the importance of increasing compliance to preoperative oral care.

    Purpose: There is insufficient evidence regarding the precise levels of physical activity (PA) in older patients following total knee arthroplasty (TKA). The aims of this study were (1) to describe the changes in the amount and intensity of PA before and after TKA with an accelerometer in older patients, compared with age- and sex-matched healthy participants and (2) to assess the effect of TKA on PA depending on age. Methods: Sixty-six primary TKA patients aged 60 years or over (mean age, 73.3 years) wore an accelerometer (Lifecorder EX) for 10 consecutive days and completed the Oxford Knee Score (OKS) before and at 6 months after TKA. PA was evaluated by mean step count and time spent (min) engaged in PA per day. PA intensity was classified as light (1.5–3 metabolic equivalents (METs)), moderate to vigorous (≥3 METs), and total (≥1.5 METs). Sixty-four healthy participants completed a single assessment of PA. Results: Each of the PA measures and OKS increased significantly after TKA. Compared with healthy controls, light and total PA improved to 100% at 6 months after TKA in patients 75 years or older. By contrast, moderate-to-vigorous PA was 32% of that of the controls and rose to 78% after TKA. PA intensity in patients aged 60–74 years was 31–74% of the controls but did not reach the same level after TKA. Conclusion: TKA in older patients increases the amount of PA, with light and total PA suitable parameters for assessing PA.

    Little is known about the long-term changes in physical activity (PA) after total hip arthroplasty (THA). The purpose of this study is to describe the changes in the number of steps and intensity of PA as measured by accelerometers, patient-reported physical function, and health-related quality of life of THA patient preoperatively and at 1 and 3 years after THA.
    Methods
    This study included 153 patients (mean age, 61.4 years; 86.3% women) who wore an accelerometer for 10 consecutive days and completed the Oxford Hip Score (OHS) and the Short Form 8 at the 5-year follow-up after THA. PA was evaluated based on the mean steps per day and the time spent performing moderate-to-vigorous PA (MVPA) per week.
    Results
    All 5 indicators had significantly increased at 1 year after THA, although only MVPA and OHS had further increased significantly at the 3-year follow-up. The predictor of improvement in the number of steps from baseline to 1 year post-THA was younger age, while those of improvement in MVPA from baseline to 3-year post-THA were younger age, higher OHS, and better Short Form 8 mental component scores using mixed-model analysis. Participation in the PA measurements was high (69.9%) at the 3-year follow-up. No significant changes were observed for these outcomes in the remaining cohort beyond 3 years after THA.
    Conclusion
    In post-THA patients, MVPA continued to increase for 3 years postoperatively. Therefore, PA must be measured over the medium term to long term following THA.

    Background: Physical activity (PA) is beneficial for stroke prevention; in particular, moderate-to-vigorous physical activity (MVPA). However, few studies have investigated its relationship with recurrent ischemic stroke (RIS). Objectives: To clarify the relationship between MVPA and RIS and the burden of risk factors after a first-ever ischemic stroke. Methods: A total of 45 outpatients (mean age 67.1 ± 10.2 years) who had previously experienced a transient ischemic attack or a minor non-cardioembolic ischemic stroke at a single hospital in Japan (mean 6.4 ± 4.2 years previously), were enrolled between March and June 2016. All patients wore an accelerometer around their hips for 10 days, and their percentage body fat (%BF) and visceral fat level (VFL) were measured by bioelectrical impedance. Retrospective information about the history of RIS and risk factors (blood pressure, lipoprotein cholesterol and estimated glomerular filtration rate) were extracted from the clinical records. Binary logistic regression models were used to estimate the relevance of the RIS history to MVPA and potential risk factors such as sociodemographic and clinical variables (obesity, smoking and hypertension). Results: RIS occurred in 9 patients; they had significantly higher VFL (p = 0.007) and %BF (p = 0.007) values and lower MVPA (p = 0.011) values than patients without recurrence. A multivariate analysis of these factors indicated that age, VFL and MVPA were significant independent predictors of RIS. Conclusions: Patients with a history of mild ischemic stroke had low MVPA and high VFL values, which together may be a risk factor for RIS.

    BACKGROUND:
    The Asian lifestyle requires deep hip flexion, which increases the risk of dislocation. Hence, Asian total hip arthroplasty (THA) patients may have problems postoperatively. This study aimed to 1) document quality of life (QoL), Asian lifestyle-related items (five postures that require deep hip flexion), and perceived satisfaction in patients who underwent THA in Japan versus Korea, 2) assess the differences in QoL and Asian lifestyle-related items by patient lifestyle, and 3) identify the factors predicting QoL.
    METHODS:
    This cross-sectional study included 222 THA patients. Korean patients completed the questionnaires. Japanese patients were then matched with Korean patients by age, sex, and post-THA period. Assessed parameters included QoL measured by the EuroQoL, Oxford hip score (OHS), Asian lifestyle (measured by the Asian lifestyle-related items), and perceived satisfaction (measured by the five items of postoperative satisfaction). Multiple regression analysis was used to determine the predictors of QoL.
    RESULTS:
    Compared with Korean patients, Japanese patients had better QoL, but more difficulty performing postures requiring deep hip flexion. Greater QoL was associated with greater satisfaction. The predictors of decreased QoL after THA were lower OHS in both countries, worse squatting ability in Japan, and worse leg-crossing ability in Korea.
    CONCLUSIONS:
    In Japan and Korea, the OHS is an important predictor of QoL after THA.

    Background: With the increasing number of patients with knee osteoarthritis undergoing TKAs in China, there is a clear need for a valid, short, joint-specific patient-reported outcome measure such as the Oxford Knee Score (OKS). Questions/purposes: To test the translated and cross-culturally adapted Chinese (Mandarin) version of the Oxford Knee Score (OKS-CV) and its (1) reliability, (2) construct validity, (3) dimensionality, and (4) responsiveness. Methods: Three native Chinese bilingual translators (a professional English translator, an experienced orthopaedic surgeon, an advanced-practice nursing specialist) translated the English-language OKS into Mandarin Chinese. A consensus panel created a synthesis of those efforts, which then was back-translated by two bilingual nonmedical, professional English-language translators. The OKS-CV was developed according to the guidelines of copyright holders. Between March 2013 and March 2015, 253 patients underwent TKAs. Among them, 114 Mandarin-speaking patients with knee osteoarthritis underwent primary unilateral TKA (age, 67 ± 7 years; range, 55–84 years; female, 80%; preoperatively 54% had moderate to severe knee osteoarthritis), completed the preoperative questionnaires, and were followed up, with a mean postoperative followup of 2.7 years (SD, 0.5 years). Eligibility criteria were (1) patients with knee osteoarthritis who were scheduled to have a primary unilateral TKA, (2) patients who were fluent in Mandarin, and (3) consent to participate. The exclusion criteria were: (1) lack of understanding of Mandarin, and (2) inability to comprehend the questionnaires owing to cognitive impairment. To evaluate test-retest reliability, another group of 35 Mandarin-speaking outpatients with knee osteoarthritis (age, 61 ± 10 years; range, 44–84 years; female, 77%) was recruited to complete the OKS-CV twice at a 1-week interval. Reliability was tested using Cronbach’s alpha and intraclass correlation coefficient (ICC). Construct validity was evaluated using Spearman’s rank correlation coefficient to quantify the correlations between the OKS-CV and the WOMAC, Short Form-8 Health Survey (SF-8TM), and EuroQol Group 5-Dimension Self-Report Questionnaire (EQ-5D). Exploratory factor analysis was performed to clarify dimensionality. The eigenvalue indicates the importance of each factor obtained from factor analysis. Responsiveness was determined by standardized response mean (SRM) and effect size (ES) from preoperative and postoperative scores of the OKS-CV. Floor and ceiling effects also were analyzed. Results: The internal consistency (Cronbach’s alpha = 0.89) and test-retest reliability (ICC = 0.93; 95% CI, 0.87–0.97) proved good. Convergent construct validity was supported by moderate to strong correlations between the OKS-CV and the WOMAC (r = −0.80, p < 0.001), the SF-8TM physical component summary (r = 0.65, p < 0.001), and the EQ-5D usual activities (r = −0.41, p < 0.001) and mobility (r = −0.35, p < 0.001). There also were correlations between the OKS-CV and the SF-8TM mental component summary (r = 0.58, p < 0.001) and the EQ-5D anxiety/depression (r = −0.35, p < 0.001). The factor analysis yielded three factors with eigenvalues greater than 1. Responsiveness was excellent (SRM = 1.52; ES = 1.52). No floor or ceiling effect was observed. Conclusions: The OKS-CV showed good acceptability and psychometric properties for the intended population. Future studies are needed to evaluate the mental state of patients with knee osteoarthritis. Clinical Relevance: The OKS-CV appears to be a reliable, valid, and responsive instrument for Chinese patients with knee osteoarthritis. Based on these results we believe the OKS-CV can be used as a valuable tool for the assessment of patient-reported outcomes in Chinese patients with knee osteoarthritis before and after TKA.

    Aim: This concept analysis defined and described the components of empowerment of family caregivers who care for adults and the elderly. Methods: Rodgers evolutionary method of concept analysis was used. Data sources included Pub Med, CINAHL, Scopus, and Igaku Chuo Zasshi. Thirty articles published between 1995 and 2015 focusing on the empowerment of family caregivers providing care to adults and the elderly were selected. Result: The analysis reveals 6 attributes, 5 antecedents, and 5 consequences. In addition, the results reveal structured aspects of family caregivers, care receivers, and other people surrounding them, and their relationships. Empowerment of adults' and elders' family caregivers may be defined as “positive control of one's mind and body, cultivating a positive attitude, proactively attempting to understand one's role as a caregiver to improve caregiving capabilities, focusing on others as well as oneself, supporting the independence of the care receiver, and creating constructive relationships with other people surrounding them.”. Conclusion: The components of empowerment clarified in this research can be used for the measurement of and interventions aimed at improving empowerment among family caregivers. Furthermore, clarifying the definition of empowerment among family caregivers enables researchers to better distinguish empowerment from similar concepts in the future.

    We evaluated nocturnal sleep-wake rhythms and subjective sleep quality on the first postoperative night compared with the preoperative night in 34 patients who had total hip arthroplasty (mean age: 61.9 years; 82.4% female) under spinal anaesthesia. We also examined secondary factors related to sleep disturbances after surgery. Patients wore an accelerometer (actigraph) during the preoperative period and the first postoperative night to track sleep-wake rhythms. Secondary end-points were postsurgical pain and low back pain. Sleep parameters were measured objectively by actigraphy, and subjective sleep quality was measured by the Oguri-Shirakawa-Azumi Sleep Inventory, Middle-Aged and Aged Version and found to worsen significantly after surgery. Actigram data showed a reduction in actual sleep time from 353.7±121.2min preoperatively to 263.8±104.4min postoperatively. The data indicate that normalized continuous sleep time is important for patient healing and well-being and that postoperative sleep disturbance is related to low back pain more than to postsurgical pain.

    【目的】人工膝関節全置換術(以下TKA)の術前と術後6ヵ月の日常生活における一日当たりの歩数と健康関連QOLの変化を明らかにする.さらに歩数と健康関連QOLの関連を調査する.【方法】自宅で生活する患者に,手術前後の2回の調査にて歩数は加速度計で測定し,健康関連QOLのEuroQol(以下EQ-5D)とOxford KneeScore(以下OKS)を含む質問紙調査を行った.【結果】分析対象者は41名で,手術前3382歩/日から手術後4224歩/日と有意に増加した(p=0.000).EQ-5D(効用値)においては,手術前0.54から手術後0.73(p=0.000),OKSでは,手術前40.0から手術後25.0(p=0.000)と有意に改善した.術後の歩数とEQ-5D(効用値)で,正の相関(r=0.453,p=0.003)を認め,歩数とOKSでは,強い負の相関(r=-0.574,p=0.000)を認めた.歩数が増加することでEQ-5DとOKSは高い傾向が示された.【考察】TKA後は,歩行障害の改善に伴い,術前に比べ平均歩数が増加し,日常生活活動量が増えたことが示唆された.また,歩数は手術後の健康関連QOLとの関連が示され,今後は長期的な歩数の変化と共に活動内容の質的な把握が必要である.(著者抄録)

    研究目的】腰椎麻酔下で人工股関節全置換術を受けた患者の尿中メラトニンを調査し,術後の分泌動態を明らかにする。さらに,ストレス関連ホルモン・疼痛および環境要因との関連を評価する。【研究方法】尿中メラトニン量は手術当日の22時,2時,6時に測定し,術後1日目朝に血清中のコルチゾール・アドレナリン・ノルアドレナリン量を測定した。【結果】分析対象者は33名(平均年齢61.6歳,女性が81.8%)で,腰椎麻酔手術後に64%の患者が尿中メラトニン量の変動がない,あるいは早朝に上昇する等の異常なメラトニン分泌リズムを示した。異常なメラトニン分泌リズムを示した患者とストレス関連ホルモンが異常値を示した患者は一致せず,メラトニン分泌リズムはストレス関連ホルモンによって影響を受けていないことが示された。術後の腰背部痛は創部痛より高く,ストレス関連ホルモンとの関連があり,術後の疼痛管理が重要である。

    本研究はC型慢性肝炎患者の疲労感とQOLを明らかにし,身体活動量や関連要因の検討を行った。C型慢性肝炎患者75名を対象に,疲労感尺度(FIS),肝疾患特異的QOL尺度(CLDQ)の質問紙調査と身体活動量を測定した。疲労感が強い者は身体領域の疲労が強く,病態の進行によりQOLが低下した。FIS,CLDQはともに有意な相関を示し,身体活動量は平均歩数6,479歩,運動量160±112kcal/day,運動強度2.6±1.7 METs/dayであった。身体活動量は,3METs未満が80%と運動強度は多くが低値を示した。関連要因は,「65歳以上群」「就業なし群」「Alb値3.8未満群」「Plt値13.1万未満群」がFISは高値を示し,CLDQは低値を示した(p<.05)。C型慢性肝炎患者では,疲労や病態の進行を考慮しながら身体活動を高めることが重要であることが示唆された。

    人工関節の術後成績評価方法には,医療者サイドからみた関節機能評価と,患者サイドからみたQOL評価がある.今回,QOL評価であるOxford Knee ScoreとSF-8を用いて,人工膝関節術後患者のQOLを調査した.対象は両側同時TKA群56例112膝,片側TKA群32例32膝であった.両群において術前後でのKnee ScoreとFunction Score,Oxford Knee Score,SF-8を調査した.Knee Score,Function Scoreは両群ともに術前と比較すると,術後2ヵ月,4ヵ月,6ヵ月,1年で有意に改善した.またOxford Knee Scoreは術前,術後6ヵ月においてそれぞれ,両側同時TKA群37.6点,23.0点,片側TKA群31.4点,23.5点であり,両群とも有意に改善した.SF-8は,術前後で両側同時TKA群においてすべての項目で,片側TKA群において身体の痛みが有意に改善した.つまりTKA後のQOLは術後6ヵ月程度で改善することが示唆された.

    Purpose
    Total hip arthroplasty (THA) is a widely accepted orthopedic surgical procedure performed to
    relieve pain and improve physical functions. However, research on long-term quality of life
    among THA patient is limited. The purposes of this study were to 1) examine changes in the
    Euro-QOL (EQ5D) and depression in patients before THA and five years after THA and 2)
    assess post-operative satisfaction and anxiety evaluated by self-administered questionnaire.
    Design
    This study is a longitudinal, quantitative, and descriptive study.
    Methods
    Consecutive patients going through primary THA at Saga University Hospital, Japan,
    between July 2003 and November 2006 were eligible for the study. The preoperative
    questionnaires included baseline demographic variables. Surgical procedure, diagnosis and
    post-surgical complications requiring treatment were obtained from the medical record. The
    study was approved by the ethics committee of Saga University. The EQ5D and depression
    subscale of Arthritis Impact Measurement Scale(AIMS)2 were administered at the preoperative
    period and post-operative at five years. Participants were also asked to rate the
    original 10 items regarding post-operative satisfaction, such as “no pain of daily life” and
    working. Higher scores indicate better satisfaction. In addition, 3 items about anxiety
    regarding postoperative dislocation and loosening were asked for these participants. Higher
    scores indicate stronger anxiety. Responses of these items were based on a 4-point Likert
    scale.
    Results
    298
    During the study period, 1004 preoperative patients agreed to participate. Of these, 236
    participants did not return their questionnaires or were too many missing data after THA, and
    100 participants were excluded from analysis because of operation of the other hip within five
    years after THA. Sixty-hundred and sixty-eight participants completed both pre- and post-
    THA surveys. The valid-response rate was 66.5%. The improvement in the mean scores for
    the EQ5D and depression scales between the pre-operative and post-operative periods were
    highly significant (p<0.001). The most of the postoperative satisfaction items showed high
    mean scores. However, the “enjoy exercise” item had a low mean satisfaction score. In
    addition, mean score of the anxiety of postoperative loosening was higher than that of
    dislocation loosening.
    Conclusions
    Quality of life in participants who underwent surgery sustained improvement five years after
    THA, and the postoperative satisfaction remained high. However, participants reported much
    less satisfaction for exercise, and they were more concerned about loosening than dislocation.
    In order to assess long-term QOL in THA participants, health professionals need to pay
    attention to changes in patients’ concern over time.

    Purpose
    The purpose of this study was to determine objective changes in the physical activity and
    quality of life (QOL) of patients before, and 6 and 24 months after undergoing total knee
    arthroplasty (TKA). Furthermore, we compared the physical activity and QOL of the patients
    24 months after TKA with that of healthy persons.
    Design
    This was a longitudinal study.
    Methods
    Twenty-seven participants were selected among patients who were indicated to undergo
    primary TKA and patients who had undergone primary TKA 6 or 24 months prior. Physical
    activities within 1 week were measured using a small pedometer with an accelerometer
    (Lifecorder EX, Kenz, Japan) to determine the number of steps walked and the intensity of
    physical activity (<3 and ≥3 metabolic equivalents [METs] h/week). The QOL was assessed
    by conducting a self-reported questionnaire survey using the Short-Form 8-item (SF-8)
    questionnaire and Oxford knee score (OKS).
    Ethical considerations: The participants were informed that (1) study participation was
    voluntary, (2) study participation would not affect their treatment, and (3) only aggregated
    data would be presented. Written informed consent was obtained from all the participants.
    The study was approved by the ethics committee of Saga University of Medicine.

    Results
    This study included 3 men and 24 women, aged 73.6 ± 6.3 years (mean ± SD). The mean
    number of steps walked and the intensity of physical activity ≥ 3 METs differed significantly
    between before, and 6 and 24 months after TKA (p < 0.001). The number of steps walked
    before TKA was 3461 steps/day; 6 months after TKA, 4914 steps/day; and 24 months after
    TKA, 5133 steps/day. The intensity of physical activity ≥ 3 METs was 2.5, 5.9, and 6.9
    h/week, respectively. Among the healthy persons (age- and sex-matched with the TKA
    patients), the number of steps walked was 5948 steps/day, and the intensity of physical
    activity ≥ 3 METs was 8.1 h/week. No significant difference was observed between the TKA
    patients who had undergone TKA 24 months prior and the healthy persons. The preoperative
    SF-8 scores (Physical Component Summary [PCS] score, 38.2 points; Mental Component
    Summary [MCS] score, 49.3 points) greatly improved 24 months after TKA (PCS score, 45.3
    points; MCS score, 52.6 points).
    The total OKS improved 6 and 24 months after TKA compared with that before TKA.
    However, of the patients, 44.4% had moderate to extreme difficulty in kneeling down and
    getting up afterwards or they could not do it, and 29.6% answered the same for walking down
    the stairs.
    Conclusions
    The physical activity and QOL of the patients improved 6 and 24 months after TKA, and the
    patients had a relatively good recovery compared with healthy persons. However, at 24
    months after TKA, patients still had difficulty in kneeling down and getting up afterwards and
    walking down the stairs.

    Purpose
    Purpose: Bereavement, particularly of a spouse, is a stressful life event that has a large impact on the mind and body. This study explored the characteristics of the post-traumatic, psychological growth of widows and widowers in palliative care units who had lost spouses.
    Design
    This was a quantitative, cross-sectional study.
    Methods
    Methods: A questionnaire study was conducted among survivors in two palliative care units in Japan from April 2007 to March 2013. Participants were 550 people who were surveyed between 6 months and 6 years after bereavement. The survey incorporated a measure of demographic characteristics, the Post-traumatic Growth Inventory-Japanese (Taku, 2007), the Coping Scale (Ozeki, 1993), and the Duke Social Support Index-Japanese (Iwase et al, 2008).
    Results
    Results: From 550 participants, 244 (44.4%) were returned; 26 questionnaires were discounted because of problems. 108 spouses were made into the candidate among 218 persons. The average age of participants (SD) was 67.0 (9.9) years; 31 were widowers and 77 widows. The average age of the deceased spouses was the same. The mean palliative care unit length of stay was 1.5 months, for an average of two years six months during a bereavement period. High scores on the scale represented the amount of mental shock at the time of losing a spouse. However, family care has also been enhanced from the general units is a palliative care units, the spouse who was mental ready for inviting death was 62%. In addition, 85.2% of spouses accepted their spouse’s death, but some participants had still not accepted the death after six years. The post-traumatic growth (PTG) total scores for the sample were moderate, but higher than for parents of paediatric osteosarcoma patients (Yonemoto et al., 2012) and car accident survivors (Nishi et al., 2010). In addition, PTG scores were higher for widows than for widowers, and there was a significant difference between these scores (p=0.008). There were no significant differences in PTG on the following factors: separation-by-death period of less than one year and one year or more; palliative care unit hospitalization duration of one month or more, and less than one month; survivor’s age less than 65 years old, and aged 65 and over; spouses who were mentally ready for their spouse’s death, and spouses who were not; and spouses who did not accept death and spouses who did. PTG showed a positive correlation with emotion-focused coping, problem-focused coping, emotional support, and recognition evaluative support.
    Conclusions
    Conclusion: Spouses who lost partners through death in the palliative care unit had the highest average ages. For these spouses, existence of the deceased was large. The shock at the time of separation by death was also great. In addition, PTG was higher than in parents of paediatric osteosarcoma patients or car accident survivors. For bereaved individuals who have lost a spouse, nursing thing that can stress coping bereaved family and social support is important has been suggested.

    血液透析、腹膜透析、腎移植は、腎不全に対して行われる腎代替療法であり、患者の価値観や生活背景などから患者に見合った治療の選択と患者の満足度を高めるために、患者参加型の共同意思決定による療法選択が推奨される。しかし、日本の腎不全看護における患者報告型アウトカムとして、健康関連QOLやセルフケア等が評価される中、治療満足度を図る信頼性妥当性が検証されたものはない。より質の高い腎代替療法選択支援を行う上で必要な患者報告型アウトカムの一つである治療満足度の尺度開発を行い、腎代替療法選択支援におけるSDM実装モデルを構築する。

    仕事と認知症介護の両立は極めて困難で、介護離職による経済的困窮や高齢者虐待など甚大な問題に発展しやすい。申請者は先行研究で、介護の準備不足と介護者のメンタルヘルス不調が離職に繋がりやすい実態を明らかにし、働き盛り世代の認知症介護における学習課題を特定し学習支援確立の必要性を見出した。既存の支援は受動的な学習スタイルが主流で「備え」の意識や行動は定着していない。本研究は、働き盛り世代の「仕事と認知症介護の両立への備え」を促すVirtual Reality教材を用いた学習プログラムを開発し、実用化に向けて検証することである。

    肝移植後の長期生存が可能となった現在、糖尿病、高血圧、脂質異常症などの代謝性疾患(Post-Transplant Metabolic Syndrome:PTMS)が新たな健康課題となっている。PTMSの主な要因は免疫抑制剤であるが、患者が移植後に座位中心のライフスタイルとなる事もPTMSの要因と考えられている。本研究は、移植患者の座位時間と低強度活動を測定できる身体活動調査票を開発し、さらに座位中心のライフスタイルを改善するための患者教育プログラムを開発し、その有用性を明らかにする。本プログラムにより、移植後患者の主要な問題であるPTMS、不眠、QOLの改善が期待できる。

    高齢糖尿病患者の座位行動改善プログラムの開発と検証に向け、研究を3段階で実施する。
    第1段階のプログラム作成では、高齢糖尿病患者のための座位行動改善プログラムを作成することを目的に国内外の先行研究をレビューし、座位行動短縮の介入方法とその効果を検討する。
    第2段階の介入前調査では、高齢糖尿病患者の座位行動および身体活動を調査し疾患および精神的健康との関連を検討すること、高齢糖尿病患者のサルコペニアを把握し座位行動との関連を把握することを目的とする。
    第3段階の介入調査では座位行動改善のための介入プログラムを実施しその効果を検証する目的で行う。介入は腕時計型ウェアラブルデバイスを用いる。

    近年、直接作用型抗ウイルス薬(DAA)の登場によりHCV排除率は劇的に進展した。しかしその後の肝がん発症には課題が残っている。肝がんの発症要因の1つとしてサルコペニアがある。元々HCVに罹患していた患者は、ウイルス排除後も倦怠感があり、セデンタリーライフスタイル(座位中心の生活)であろうと予測される。しかし、日常生活における身体活動量の実態やサルコペニアとの関連は不明である。そこで本研究は、C型肝炎に対してDAA治療を行う患者を対象に、治療前後において、活動量計を用いて日常生活における身体活動量を調査し、身体活動量の変化およびサルコペニアとの関連を明らかにする。

    The purpose of this study was to investigate whether comprehensive health literacy(HL) in community-dwelling older adults is associated with objective oral hygiene and oral health-related quality of life (OHRQoL). Of the 118 participants who completed the survey, 18% recorded a rating of “unhealthy” for oral cleanliness in objective oral hygiene. As for the HL results, more than 60% of the respondents said that advice from family and friends in the field of disease prevention was effective, and more than half of them lived with their families. Regression analysis identified comprehensive HL as a related factor for both oral cleanliness and OHRQoL. These results indicated the need to assess HL during follow-up for comorbidities and take the opportunity to provide personalized oral health guidance and improve OHRQoL.

    This research project was conducted as follows: 1) A 10-item scale (SLPAQ) was developed to quantitatively assess low-intensity physical activity in individuals undergoing lower extremity arthroplasty, using an accelerometer to assess validity and adequate stability on retest. It is simple to answer and will be useful for inactive older adults; 2) an exercise self-efficacy scale (ESES) was developed and confirmed for adequate internal consistency, retest reliability, and validity; 3) self-monitoring of activity was recommended by having participants wear a wearable device with an activity alert function to prevent physical inactivity. In addition, changes in ESES were identified and support for decreasing sedentary time was discussed.

    本研究は、認知症ワーキングケアラーのエンパワメントを高める教育支援プログラムを構築することを目的に、認知症の人を介護する働き盛り世代家族介護者の学習課題と介護未経験者の学習課題の特徴を明らかにした。家族介護者18名から抽出された4つのカテゴリーと20のサブカテゴリーの学習課題から介護者の準備不足が明らかになり、介護開始前の学習支援確立の必要性が見いだされた。また、介護未経験者115名の学習課題に対する意識・行動に関する調査から、年齢や性別、役割による学習課題の特性とニーズを明らかにし、介護開始前の働き盛り世代を対象とした学習支援モデルを構築した。

    The purpose of this study was to verify the effects of self-monitoring of physical activity and sleep health guidance as interventions to increase physical activity in elderly people with diabetes. Regarding the relationship between sleep and HbA1c, those with higher HbA1c had a later bedtime and a shorter sleep latency. Before and after sleep health guidance, there was no difference in PSQI and sleep time, but sleep latency was shortened and bedtime and wakeup time were earlier. In the intervention group, who requested exercise self-monitoring, the number of steps increased 1 month and 3 months after the intervention compared to before the intervention. Moderate- and high-intensity physical activity time increased one month after the intervention compared to before the intervention, but it was not maintained three months after the intervention. It was considered that maintaining the improvement of physical activity for a long period of time is an issue.
