    This study investigated differences in auditory brainstem responses (ABRs) using the paired-click paradigm between young adults frequently exposed to loud sounds and those not. Young adults with normal hearing were divided into two groups: an exposed group (n = 23), who were frequently exposed to loud sounds through music practice, and a control group (n = 21). Sound exposure levels (measured via smartwatch), hearing thresholds (0.125–16 kHz), distortion product otoacoustic emission (DPOAE) magnitudes, word intelligibility, and ABRs to single and paired clicks were obtained. For paired-click ABRs, root mean square values for the post-wave I response delayed from the wave I peak (RMS <sub>post-w1</sub> ) were calculated for second-click responses. The exposed group exhibited higher sound exposure levels than the control group. Despite similar hearing thresholds, DPOAE magnitudes, wave I peak amplitudes and wave I/V amplitude ratio, the exposed group showed significantly lower RMS <sub>post-w1</sub> values for second-click responses than the control group. No significant group difference was observed in word intelligibility. The results showed that frequent loud sound exposure may induce peripheral auditory changes consistent with cochlear synaptopathy (CS). This suggests that the paired-click stimulation paradigm may offer a way of detecting noise-induced CS.

    Objective: This study aimed to evaluate whether auditory brainstem response (ABR) using a paired-click stimulation paradigm could serve as a tool for detecting cochlear synaptopathy (CS). / Methods: The ABRs to single-clicks and paired-clicks with various inter-click intervals (ICIs) and scores for word intelligibility in degraded listening conditions were obtained from 57 adults with normal hearing. The wave I peak amplitude and root mean square values for the post-wave I response within a range delayed from the wave I peak (referred to as the RMSpost-w1) were calculated for the single- and secondclick responses. / Results: The wave I peak amplitudes did not correlate with age except for the second-click responses at an ICI of 7 ms, and the word intelligibility scores. However, we found that the RMSpost-w1 values for the second-click responses significantly decreased with increasing age. Moreover, the RMSpost-w1 values for the second-click responses at an ICI of 5 ms correlated significantly with the scores for word intelligibility in degraded listening conditions. / Conclusions: The magnitude of the post-wave I response for the second-click response could serve as a tool for detecting CS in humans. / Significance: Our findings shed new light on the analytical methods of ABR for quantifying CS.

    Individuals with autism spectrum disorders (ASD) are reported to exhibit degraded performance in sound localization. This study investigated whether the sensitivity to the interaural level differences (ILDs) and interaural time differences (ITDs), major cues for horizontal sound localization, are affected in ASD. Thresholds for discriminating the ILD and ITD were measured for adults with ASD and age- and IQ-matched controls in a lateralization experiment. Results show that the ASD group exhibited higher ILD and ITD thresholds than the control group. Moreover, there was a significant diversity of ITD sensitivity in the ASD group, and it contained a larger proportion of participants with poor ITD sensitivity than the control group. The current study suggests that deficits in relatively low-level processes in the auditory pathway are implicated in degraded performance of sound localization in individuals with ASD. The results are consistent with the structural abnormalities and great variability in the morphology in the brainstem reported by neuroanatomical studies of ASD.

    Objective: To investigate possible differences in the auditory peripheral and brainstem functions between adults with autism spectrum disorder (ASD) and neurotypical (NT) adults. Methods: Click-evoked auditory brainstem responses (ABRs) were obtained from 17 high-functioning ASD adults (aged 21–38 years) and 20 NT adults (aged 22–36 years). A relatively large number of stimulus presentations (6000) were adopted, and ABRs by horizontal and vertical electrode montages were evaluated, in order to allow precise evaluations of early ABR components. Results: Waves I, II, III, and V were identified in the vertical electrode montage, and wave I and the summating potential (SP) in electrocochleograms were identified in the horizontal electrode montage. There were no significant group differences in the wave I, II, III, and V latencies or the interpeak latencies (IPLs) in the vertical electrode montage. In the horizontal montage, the ASD adults exhibited significantly shortened SP latencies compared with the NT adults, whereas there was no significant group difference in the wave I latency. Conclusion: The ASD adults may have the abnormalities of processing more in the peripheral auditory system than in the brainstem. Significance: The current study suggests that the peripheral abnormality is associated with ASD.

    Objective: To investigate the relationship between speech auditory brainstem responses (speech ABRs) and word intelligibility under reverberation in elderly adults. Methods: Word intelligibility for words under four reverberation times (RTs) of 0, 0.5, 1.0, 1.5. s, and speech ABRs to the speech syllable/da/ were obtained from 30 elderly listeners. Root mean square (RMS) amplitudes and discrete Fourier transform (DFT) amplitudes were calculated for ADD and SUB responses in the speech ABRs. Results: No significant correlations were found between the word intelligibility scores under reverberation and the ADD response components. However, in the SUB responses we found that the DFT amplitudes associated with H4-SUB, H5-SUB, H8-SUB, H9-SUB and H10-SUB significantly correlated with the word intelligibility scores for words under reverberation. With Bonferroni correction, the DFT amplitudes for H5-SUB and the intelligibility scores for words with the RT of 0.5s and 1.5s were significant. Conclusions: Word intelligibility under reverberation in elderly listeners is related to their ability to encode the temporal fine structure of speech. Significance: The results expand knowledge about subcortical responses of elderly listeners in daily-life listening situations. The SUB responses of speech ABR could be useful as an objective indicator to predict word intelligibility under reverberation.

    Attention to the relevant object and space is the brain's strategy to effectively process the information of interest in complex environments with limited neural resources. Numerous studies have documented how attention is allocated in the visual domain, whereas the nature of attention in the auditory domain has been much less explored. Here, we show that the pupillary light response can serve as a physiological index of auditory attentional shift and can be used to probe the relationship between space-based and object-based attention as well. Experiments demonstrated that the pupillary response corresponds to the luminance condition where the attended auditory object (e.g., spoken sentence) was located, regardless of whether attention was directed by a spatial (left or right) or nonspatial (e.g., the gender of the talker) cue and regardless of whether the sound was presented via headphones or loudspeakers. These effects on the pupillary light response could not be accounted for as a consequence of small (although observable) biases in gaze position drifting. The overall results imply a unified audiovisual representation of spatial attention. Auditory object-based attention contains the space representation of the attended auditory object, even when the object is oriented without explicit spatial guidance.

    In order to elucidate why many elderly listeners have difficulty understanding speech under reverberation, we investigated the relationship between word intelligibility and auditory brainstem responses (ABRs) in 28 elderly listeners. We hypothesized that the elderly listeners with low word intelligibility scores under reverberation would show degraded subcortical encoding information of reverberant speech as expressed in their ABRs towards a reverberant /da/ syllable. The participants were divided into two groups (top and bottom performance groups) according to their word intelligibility scores for anechoic and reverberant words, and ABR characteristics between groups were compared. We found that correlation coefficients between responses to anechoic and reverberant /da/ were lower in the bottom performance group than in the top performance group. This result suggests that degraded neural representation toward information of reverberant speech may account for lower intelligibility of reverberant speech in elderly listeners.

    世界的な高齢化に伴い難聴者は増加しており、認知症リスクや生活の質の低下、医療費増大を招く深刻な社会問題となっている。これを抑制するには早期発見と予防が不可欠であり、初期病態の把握が重要である。近年、難聴初期に蝸牛シナプス障害が生じることが示されている。申請者は、この障害を検出する手法として、対クリック刺激による聴性脳幹反応測定法を考案し、その指標が加齢変化を反映することを示した。本研究では、指標の感度・再現性を検証し、性別や音響暴露歴との関連を明らかにした上で、難聴の初期障害の程度を年齢換算で推定する「耳年齢」モデルの基盤構築と精度向上を目指す。

    音の時間変化を検知する能力、即ち時間分解能は音声知覚に必須の機能である。ヒト聴覚の時間分解能は鋭敏で聴覚末梢系の処理を反映すると考えられてきたが、近年矛盾する結果が出ており、新たな理論や神経基盤の解明が待たれている。時間分解能の低下は聴覚障害の一因となるが、耳鼻咽喉科の検査項目に時間分解能は含められておらず、診断や治療を妨げている。聴覚障害による死亡率の増加と莫大な経済損失は重大な社会的問題であり、診療用の時間分解能検査の確立と低下改善手法の開発が早急に望まれる。本研究では、基礎から臨床までの聴覚時間分解能の課題に対し、理論、脳機能計測、検査、聴覚訓練の研究を連携して取り組む。

    Hidden hearing loss refers to a condition in which individuals exhibit normal hearing thresholds but experience difficulty understanding speech in the presence of background noise and/or reverberation. The present study aimed to elucidate the mechanisms underlying hidden hearing loss, with a particular focus on cochlear synaptopathy-a disorder affecting the synapses between inner hair cells and cochlear nerve fibers in the cochlea-which is considered a major contributing factor. Currently, there is no established method for detecting cochlear synaptopathy in humans. Therefore, considering the properties of cochlear nerve fibers affected by cochlear synaptopathy, we proposed auditory brainstem responses (ABRs) using a paired-click stimulation paradigm as an index that could reflect cochlear synaptopathy. Our findings demonstrate a significant association between the ABR indices and word intelligibility scores under noisy and reverberant listening conditions.
