    Although evidence from basic studies indicates that C5a induces hyperalgesia, knowledge from studies of humans is limited. This comparative analysis of the peripheral blood C5a concentration of women diagnosed with fibromyalgia (FM) who exhibited widespread pain and that of female healthy controls (HCs) was done to assess possible correlations of C5a concentration with pain threshold. The data of 30 patients with FM and 29 HCs were included in the analysis. C5a concentration in the peripheral blood was quantified by ELISA, and the cold pain threshold (CPT) was assessed. The correlation between C5a concentration and CPT was analyzed using the Spearman correlation coefficient, and the peripheral blood C5a concentrations of FM and HC were compared by t-test. The mean (standard deviation) peripheral blood C5a concentrations of FM and HC were 12.7 (6.48) ng/ml and 8.82 (4.79) ng/ml, respectively (p = 0.0114). Although no significant relation was observed between CPT and C5a concentration in FM (R = - 0.12, p = 0.52), a significant correlation was found for HC (R = 0.41, p = 0.03). The results suggest that C5a would be a potential biomarker for the pain sensitivity of women and give new insights into the pathophysiology of FM.

    BACKGROUND: Modern-Type Depression (MTD) is a category of depression that has been studied mainly in Japan; however, no study has attempted to determine its relation to chronic pain. AIM: To determine possible associations between psychological traits related to MTD and the chronic pain of patients at psychiatric clinics. METHOD: Two hundred and twenty-one first time patients who visited the psychiatric clinic at a Japanese university medical center or an associated clinic were enrolled. The Hamilton Depression Rating Scale was used to measure depressive symptoms. The 22-item Tarumi's Modern-Type Depression Trait Scale (TACS-22), Achievement Motive, and 20-item Toronto Alexithymia Scale were used to assess psychological traits related to depression and chronic pain. The clinical diagnosis of each patient was confirmed by use of the Structured Clinical Interview for DSM-IV Axis I Disorders, administered by experienced specialists. The medians of the psychological traits identified were compared between patients with or without chronic pain. Analysis was also done of patients with Major Depressive Disorder (MDD). RESULT: Of the 221 patients, 139 had chronic pain. Patients with chronic pain had more severe depressive symptoms, Alexithymia, and high scores for the complaint trait of MTD. Seventy-three of the 221 patients met the criteria for MDD (53 had chronic pain). Patients with MDD comorbid with chronic pain had a higher competitive achievement score, severe depression, and difficulty identifying feelings. CONCLUSION: Complaint and competitive traits were shown to be related to chronic pain in psychiatric settings. Further study will allow us to design multidimensional approach for patients suffering from depression.

  • Vitamin D Deficiency Osteomalacia Related to Hypophosphatemia in a Patient with Anorexia Nervosa : A Case Report Reviewed

    <p><b>Introduction</b> : Although osteoporosis is a well-known complication of anorexia nervosa (AN), osteomalacia is presented in a few reports. We herein describe a case of vitamin D deficiency osteomalacia related to hypophosphatemia in a patient with AN.</p><p><b>Case report</b> : A woman in her 50s had a history of weight loss, with her body mass index (BMI) at one time decreasing to 10 kg/m<sup>2</sup>. She was diagnosed with AN and underwent treatment as an outpatient. She began to withdraw from society and isolated herself at home. Her serum phosphorus level decreased to 0.5 mg/d<i>l</i>, and she was admitted to our department on an emergency basis. She was diagnosed with vitamin D deficiency osteomalacia related to hypophosphatemia. She had high bone ALP level (bone specific alkaline phosphatase〔BAP〕, 29.5 µg/<i>l</i>), decreased bone density (lumbar spine young adult mean〔YAM〕, 47%), increased bone scintigraphic uptake (ribs and spine), vitamin D deficiency (25 [OH] D, 5.2 ng/m<i>l</i>), normal fibroblast growth factor 23 (FGF 23) level (15.0 ng/m<i>l</i>), and normal urine. After regaining weight through nutritional therapy, supplemental vitamin D therapy, and sunbathing, the serum phosphorus and ALP levels normalized ; subsequently, she was discharged from the hospital.</p><p><b>Discussion</b> : We speculate that the patient’s long-term low nutritional status and home isolation caused vitamin D deficiency osteomalacia. No serious symptoms developed because the hypophosphatemia allowed the diagnosis of the disease at an early stage. Because patients with AN are at high risk for vitamin D deficiency, the risk of osteomalacia as well as osteoporosis, should be considered.</p>

    【背景】「慢性疼痛のアクセプタンス」は慢性疼痛の理解や治療のための重要な概念とされている。その測定にはChronic Pain Acceptance Questionnaireが最も使用されている。本研究の目的はCPAQ日本語版(CPAQ-J)の作成,その妥当性の検討であった。【方法】慢性疼痛患者243人を対象とし,CPAQ-Jおよび痛みの強度,機能障害,不安,抑うつ,痛みの破局化,生活の質,生活満足度につき回答してもらった。【結果】CPAQ-Jはスクリープロット,主因子法により英語原版と同様の2因子構造を示し,十分な内的整合性(Cronbach α係数0.83)と再検査信頼性(級内相関係数0.88)が示された。また,痛みの強度,機能障害,不安,抑うつ,痛みの破局化,生活の質,生活満足度と有意な関連を認めた。【結論】本研究でCPAQ-Jは我が国の慢性疼痛患者において,十分な信頼性と妥当性が示された。(著者抄録)

    We investigated the impact of medical collaboration between the Chiba Eating Disorder (ED) Treatment Support Center (CSC) in Chiba Prefecture and the ED treatment center at Kohnodai Hospital. We calculated the number of consultations performed by the CSC and referral rate to other medical facilities from October 2017 to March 2020, as well as the clinical characteristics of the patients treated at our hospital from April 2016 to March 2020. Data on duration of hospitalization and increase in body mass index (BMI) during hospitalization were recorded. Patients were divided into all of the Japan and Chiba Prefecture groups. Data were evaluated by Poisson's regression analysis or one-way analysis of variance. A p-value < 0.05 was considered significant. The 2019 data served as reference values. Our data demonstrated that while the number of CSC consultations increased (2017:201, 2018:547, 2019:552) annually, the number of hospitalizations for EDs decreased (197, 194, 134, respectively). In comparison, the number of outpatient consultations for EDs across all of Japan peaked in 2018 and decreased significantly thereafter (2016:110, 2017:139, 2018:193, 2019:142). After the CSC was established, the number of patients treated in our department decreased as expected. Patients with anorexia nervosa (AN) who were treated on an outpatient basis across all of Japan were younger in 2019 (27.0 ± 1.2) than in 2018 (31.9 ± 1.2). Severe cases had better outcomes, and there was a significant increase in BMI of inpatients with AN in both groups. Overall, the activities of such ED treatment support center promoted successful treatment of severe ED cases.

    背景:うつ病と慢性疼痛の関連は以前より示唆されているが、新型/現代型うつ(以下MTD)と慢性疼痛との関連を検討した研究はない。目的:MTDに関連する心理特性と精神科受診者の慢性疼痛との関連性を検討した論文(FujimotoらFront Psychiatry 2021)を紹介する。方法:大学病院または関連病院の精神科外来を受診した初診患者221名を対象とした。抑うつ症状の測定にはハミルトンうつ病評価尺度を用いた。心理的特性は22項目版現代うつ気質尺度(TACS-22)と20項目版トロント・アレキサイミア尺度(TAS-20)で評価した。測定項目の中央値を慢性疼痛の有無で比較した。結果:221名の患者のうち139名が慢性疼痛を有していた。慢性疼痛を有する患者は、抑うつ症状、感情同定困難(TAS-20下位項目)、そしてMTDの不平不満傾向(TACS-22下位項目)が高得点であった。結論:精神科臨床での慢性疼痛には、MTDに関連する心理特性である不平不満傾向が関係していることが示唆された。(著者抄録)

    器質的疾患を指摘できない慢性疼痛において、近年その生物学的基盤が明らかになりつつある。特に注目を浴びているのが、神経炎症である。視神経脊髄炎スペクトラム障害などの脱髄疾患を中心として神経炎症との関連が明らかになるにつれ、病態に基づいた治療が実臨床に導入されつつある。慢性疼痛においても炎症メディエーターやミクログリアの関与が知られているが、近年グリア細胞の活性をin vivoで評価できる18kDa-translocator protein(TSPO)をリガンドとして用いたPET検査が行われるようになり、病態の解明が進んでいる。また、統合失調症や自閉症スペクトラム障害での関与が疑われているシナプス刈り込みも慢性疼痛の病態形成に関与している可能性がある。遺伝子ビッグデータを用いた研究においても、抑うつ、PTSDや自己免疫性疾患との関連が確認された。近い将来、慢性疼痛の生物学的基盤の理解がさらに進み、臨床的場面で有用なバイオマーカーの開発につながることを期待する。(著者抄録)
