INTRODUCTION: Postoperative irradiation is commonly used in patients with oral cancer who are at high risk of recurrence. However, the need for contralateral neck irradiation remains controversial. This study evaluated the efficacy of unilateral neck irradiation and identified risk factors for recurrence in the contralateral neck. METHODS: This retrospective study included 85 patients with oral cancer (27-88 years of age; median, 65 years) who underwent postoperative unilateral neck irradiation at a single institution between 2010 and 2023. Recurrence patterns and late adverse events were analyzed. The ipsilateral neck control rate (INCR), neck control rate (NCR), progression-free survival (PFS), and overall survival (OS) were estimated using the Kaplan-Meier method. The two-year contralateral neck control rate (CNCR) was evaluated using log-rank testing and Cox proportional hazards regression. RESULTS: The median duration of follow-up was 45 months. At two years, the rates of INCR, NCR, PFS, and OS were 85%, 77%, 69%, and 82%, respectively. Recurrence was observed in 31 (36.5%) patients, including ipsilateral recurrence in 15 (17.6%), contralateral recurrence in 11 (12.9%), and distant recurrence in 16 (18.8%), with 6 (7.1%) showing contralateral-only recurrence. No grade >=3 late adverse events were observed. Five or more metastatic lymph nodes, >=T2 tumors, and newly diagnosed status were significantly associated with poorer CNCR. CONCLUSION: Unilateral neck irradiation may be an effective strategy for selected patients with oral cancer, particularly those with fewer metastatic lymph nodes, lower T stage, or recurrent cases.

INTRODUCTION: This study evaluated dosimetric values of cone beam computed tomography (CBCT)-guided online adaptive radiotherapy (oART) in patients with gastric mucosa-associated lymphoid tissue (MALT) lymphoma, accounting for interfractional and intrafractional motion. METHODS: Four patients with stage I gastric MALT lymphoma received CBCT-guided oART. For each of the 60 treatment sessions, scheduled (SCH) and adapted (ADP) plans were generated. Dosimetric evaluation focused on clinical target volume (CTV) and organs at risk (OARs). Metrics included CTV D98% and D95%, mean dose to the liver and left and right kidneys, maximum dose to the spinal cord, and V5Gy for bilateral kidneys. Adaptive planning CBCT-based contours were propagated to synthetic CTs of SCH and ADP plans to assess interfractional motion. Post-treatment CBCT-based contours were propagated to synthetic CTs of the ADP plan to evaluate intrafractional motion. RESULTS: ADP plans significantly improved CTV coverage: mean D98% increased from 94.7% in the SCH plan to 98.6%, and D95% from 97.3% to 99.2% (p < 0.001). Most OAR doses were reduced in the ADP plans, including bilateral kidney V5Gy (11.3% vs. 8.3%, p < 0.001) and spinal cord Dmax (9.8 Gy vs. 7.9 Gy, p < 0.001). Liver Dmean was slightly higher in the ADP plan (11.4 Gy vs. 11.1 Gy, p = 0.002). No significant differences were observed in CTV and OAR dosimetric parameters between adaptive planning and post-treatment CBCTs (e.g., CTV D98%: 98.6% vs. 98.5%, p = 0.629). CONCLUSION: CBCT-guided oART improved target coverage and maintained post-treatment dosimetric stability in gastric MALT lymphoma, supporting clinical feasibility.

Background/Objective: The recommended post-operative radiotherapy dose of approximately 60 Gy may be reduced during treatment planning, and the optimal irradiation method and dose remain unclear. We aimed to clarify the usefulness and safety of post-operative radiotherapy for soft tissue sarcomas. Methods: Forty-five patients with soft-tissue sarcomas who underwent adjuvant radiotherapy at two institutions from June 2014 to August 2020 were included. Patients with a high risk of recurrence underwent post-operative irradiation, with doses of 60-70 Gy and 50 Gy used in patients with positive and negative resection margins, respectively. Results: The median patient age was 72 (21-88) years. The most common histological types of sarcomas were myxofibrosarcoma (n=12) and dedifferentiated liposarcoma (n=11), followed by other sarcomas. Thirty patients were newly diagnosed, and 15 underwent surgery for localized recurrent disease. Thirty-two and 13 patients underwent wide and marginal resection, respectively. Surgical margins were negative in 12 patients and positive in 33 patients. Chemotherapy was administered before or after radiotherapy in 16 patients. The 2-year local control, progression-free survival, and overall survival rates after post-operative radiotherapy were 88%, 78%, and 93%, respectively. In patients with positive resection margins, doses of >=60 Gy contributed to local control (p=0.0002, log-rank test) and progression-free survival (p=0.0033). Late grade 3 adverse events were observed in 7% of the patients. Conclusion: Post-operative radiotherapy for soft tissue sarcomas is safe and effective, with high doses (>=60 Gy) contributing to reduced recurrence among patients with positive resection margins.

Purpose: This study evaluated the efficacy and safety of transvaginal approach combined intracavitary and interstitial brachytherapy (IC/IS BT) assisted by transrectal ultrasound (TRUS) for treatment of locally advanced cervical cancer (LACC). Materials and Methods: A total of 30 patients of LACC treated with external beam radiotherapy and IC/IS BT via transvaginal approach assisted by transrectal ultrasound were observed retrospectively. The 2-year local control (LC), progression-free survival (PFS), and overall survival (OS) were analyzed using the Kaplan–Meier method. Late adverse events were also evaluated to assess the safety of IC/IS BT. Results: The median follow-up period was 22 months. The 2-year LC, PFS, and OS were 90%, 61%, and 82%, respectively. We observed no critical complications related to the IC/IS BT technique. Late adverse events of grade 3 or more included one case of grade 4 colon perforation. Conclusion: Our patient series demonstrated that radiotherapy combined with transvaginal approach, TRUS-assisted IC/IS BT achieves favorable local control and safety for LACC.

Although methods for sequencing library preparation from double-stranded DNA are well established, those from single-stranded DNA (ssDNA) have not been well studied. Further, the existing methods have limitations in efficiency and yield. Therefore, we developed a highly efficient procedure for sequencing library preparation from ssDNA. In this method, the first adaptor tagging of ssDNA is performed using terminal deoxyribonucleotidyl transferase (TdT)-assisted adenylate connector-mediated ssDNA (TACS) ligation, which we reported recently. After complementary strand synthesis using the adaptor-tagged ssDNA, second adaptor tagging via Vaccinia virus topoisomerase I (VTopoI or TOPO)-based adaptor ligation is performed. With additional steps for degradation, repression, and removal of the adaptor dimer, the proposed TACS-TOPO scheme realizes adaptor dimer-free sequencing library preparation from ssDNA samples of 24 pg. The TACS-TOPO scheme was successfully applied to cell-free DNA analysis with amplification-free library preparation from 50 µL of human serum. A modified TACS-TOPO scheme was also applied to DNA extracted from ancient human bones, bringing two to eight times more library yields than those using a conventional library preparation protocol. The procedures for preparing VTopoI and its complex with a double-stranded oligonucleotide adaptor are also described. Overall, the proposed TACS-TOPO scheme can facilitate practical and sensitive sequencing analysis of ssDNA.

BACKGROUND: Cell-free DNA (cfDNA), which is extracellular DNA present in the circulating plasma and other body fluids, is currently investigated as a minimally invasive, highly informative biomarker. While nucleosome-sized cfDNA fragments have been investigated intensively, shorter DNA fragments in the plasma have not been studied due to several technical limitations. RESULTS: We aimed to investigate the existence of shorter cfDNA fragments in the blood. Using an improved cfDNA purification protocol and a 3'-end-labeling method, we found DNA fragments of approximately 50 nucleotides in length in the human plasma, present at a molar concentration comparable to that of nucleosome-sized fragments. Unfortunately, these short fragments cannot be recovered by widely used cfDNA isolation methods. In addition, they are composed of single-stranded DNA (ssDNA), thus escaping detection in previous studies. Therefore, we established a library-preparation protocol based on our unique ssDNA ligation technique and applied it to the isolated cfDNA. Deep sequencing of these libraries revealed that the short fragments are derived from hundreds of thousands of genomic sites in open chromatin regions and enriched with transcription factor-binding sites. Remarkably, antisense strands of putative G-quadruplex motifs occupy as much as one-third of the peaks by these short fragments. CONCLUSIONS: We propose a new class of plasma cfDNA composed of short single-stranded fragments that potentially form non-canonical DNA structures.

Whole-genome bisulfite sequencing (WGBS) is the current gold standard of methylome analysis. Post-bisulfite adaptor tagging (PBAT) is an increasingly popular WGBS protocol because of high sensitivity and low bias. PBAT originally relied on two rounds of random priming for adaptor-tagging of single-stranded DNA (ssDNA) to attain high efficiency but at a cost of library insert length. To overcome this limitation, we developed terminal deoxyribonucleotidyl transferase (TdT)-assisted adenylate connector-mediated ssDNA (TACS) ligation as an alternative to random priming. In this method, TdT attaches adenylates to the 3'-end of input ssDNA, which are then utilized by RNA ligase as an efficient connector to the ssDNA adaptor. A protocol that uses TACS ligation instead of the second random priming step substantially increased the lengths of PBAT library fragments. Moreover, we devised a dual-library strategy that splits the input DNA to prepare two libraries with reciprocal adaptor polarity, combining them prior to sequencing. This strategy ensured an ideal base-color balance to eliminate the need for DNA spike-in for color compensation, further improving the throughput and quality of WGBS. Adopting the above strategies to the HiSeq X Ten and NovaSeq 6000 platforms, we established a cost-effective, high-quality WGBS, which should accelerate various methylome analyses.

BACKGROUND: Endoscopic submucosal dissection (ESD) can be used as a less invasive treatment option for superficial esophageal cancer involving the muscularis mucosae (T1a-MM) or upper third of the submucosa (T1b-SM1). Additional treatment after ESD is needed to prevent lymph node metastasis. However, the efficacy of radiotherapy following ESD has not been well evaluated. Moreover, the clinical outcomes of patients with large mucosal defects of the esophagus who received radiotherapy after ESD have not been reported. This study aimed to clarify the efficacy of additional radiotherapy following ESD for esophageal squamous cell cancer involving T1a-MM or T1b-SM1. METHODS: We analyzed twenty-seven patients with pathologically confirmed T1a-MM or T1b-SM1 esophageal squamous cell cancer treated by ESD. Thirteen patients received additional radiotherapy (RT group), and the remaining patients did not (non-RT group). Locoregional control (LRC), overall survival, cause-specific survival, and adverse events including treatment-related esophageal strictures were evaluated. RESULTS: The three-year LRC was significantly better for the RT than the non-RT group (100% vs. 57.8%, respectively; p = 0.022). Chemotherapy following ESD did not improve LRC. Multivariate analysis showed that radiotherapy was an independent prognostic factor for better LRC (p = 0.0022). Contrary to the results in LRC, overall and cause-specific survival were not significantly different between the RT and non-RT groups. A subgroup analysis of patients with mucosal defects involving >= 3/4 of the esophageal circumference after ESD showed that LRC of the RT group was better than that of the non-RT group (p = 0.049). Treatment-related esophageal strictures were observed in 2 of 6 patients in the RT group with large mucosal defects after ESD. No patients with mucosal defects involving less than 3/4 of the circumference after ESD developed treatment-related strictures. CONCLUSIONS: Radiotherapy after ESD contributed to better LRC in esophageal squamous cell cancer involving pT1a-MM and pT1b-SM1. Esophageal strictures were observed in some patients with large mucosal defects after ESD. Despite leading to better LRC, radiotherapy after ESD should be undertaken after careful consideration for patients with large mucosal defects after ESD.
