19. 血流制限下低強度レジスタンス運動中の筋内エネルギー代謝
磁気共鳴分光法(31P-MRS)を用いて、血流制限下低強度レジスタンス運動中の筋内エネルギー代謝を明らかにし、自然血流下の低強度および高強度レジスタンス運動中と比較検討した。明らかな疾患をもたない健常男性15例を対象とした。被験者の右下腿部下に31P-MRS専用の表面コイルを装着し、右下腿三頭筋の最大筋腹部位を分析対象として行った。低強度負荷条件として、最大挙上重量(1RM)の20%を用いた低強度条件(L)と、血流制限を併用した低強度血流制限条件(LI)、および1RMの65%を用いた高強度条件(H)の3条件による動的足底屈運動を2分間(毎分30回)行い、筋内エネルギー代謝を測定した。LIのクレアチンリン酸および筋内pHの変化は、Lに比較して有意に大きかったが、LIのこれらの変化は、Hに比較して有意に小さかった。.

62. 酸化ストレスと心不全患者における骨格筋異常
【背景・目的】心不全患者における運動能力低下は筋内エネルギー代謝異常および筋細胞内脂肪蓄積(IMCL)に密接に関連している。しかしながら、これらの異常の機序は明らかではない。我々は酸化ストレスの亢進が骨格筋異常に関連していると仮説を立てた。【方法・結果】拡張型心筋症患者(DCM;10名、左室駆出率<40%、NYHA I~III)および同年齢の健常者(9名)を対象とした。最高酸素摂取量はDCMで有意に低値であった。31P-磁気共鳴分光法(MRS)により、20%1RM(最大挙上重量)の定常負荷を用いた仰臥位右足関節底屈運動中のクレアチンリン酸低下(PCr loss)はDCMで有意に低下した。1H-MRSを用いて測定したIMCLはDCMで2.1倍に増加した。DCMにおいて、血漿SOD活性は減少し、チオバルビツール酸反応性物質(TBARS)は増加した。SOD活性およびTBARSは最高酸素摂取量、PCr loss、およびIMCLと有意に相関した。【結論】酸化ストレスは骨格筋代謝障害およびIMCL増加に重要な役割を果たしており、心不全患者の運動能力を低下させる可能性が示唆された。(著者抄録).

64. 血流制限を併用した筋力トレーニングの効果 女性における至適方法の検討へ
女性および男性における血流制限を併用したレジスタンス運動の効果を単回運動時の筋内代謝的ストレスから評価し、その差異について検討した。仰臥位における右足関節底屈運動を毎分30回のペースで、総計60回(2分間)施行した。女性の最大挙上重(1-RM)は男性に比べて有意に低かったため、各条件における絶対的負荷量は、女性において低値となった。また、収縮期血圧も女性において低値であったため血流制限圧も低くなった。安静から運動時における骨格筋内クレアチンリン酸は、各条件・時間において、男女間の有意差は見られなかった。65%1-RM条件において、女性における筋細胞内pHの低下が小さいように見えるが、クレアチンリン酸同様に各条件・時間においても男女間の有意差はみられなかった。男性、女性同様に血流制限により有意にクレアチンリン酸、筋細胞内pHの低下は増強したが、高強度負荷には及んでいなかった。.

【目的】メタボリック症候群(MetS)患者では有酸素運動能が低下しており、運動療法の障害となっている。ピオグリタゾンは、骨格筋ミトコンドリア機能や脂肪酸酸化に関わる遺伝子発現を誘導することが知られている。MetS患者においてピオグリタゾンが、有酸素運動能を改善させるかどうかを検討した。【方法】運動習慣の無い14人のMetS患者にピオグリタゾン15mg/dayを3ヵ月間投与した。投与前後で、有酸素運動能測定、31P磁気共鳴分光法を用いた足関節底屈運動中(最大挙上重量の20%)の下腿骨格筋高エネルギーリン酸代謝および1H磁気共鳴分光法を用いた筋細胞内脂肪測定を行った。【結果】ピオグリタゾン投与後に、最高酸素摂取量と嫌気代謝閾値は改善した。骨格筋における運動中のクレアチンリン酸低下と筋細胞内脂肪はピオグリタゾン投与後に減少した。【結論】ピオグリタゾンはインスリン抵抗性患者の有酸素運動能を高める有効な治療法であると考えられた。(著者抄録).

【目的】一酸化窒素(NO)合成酵素阻害剤であるL-NAME(LN)を用いて運動誘発性の運動能力と骨格筋ミトコンドリア機能の増加におよぼすNOの役割について検討した。【方法】雄性C57BL/6JマウスをSED(対照群)群、SED+LN群、EX群およびEX+LN群の4群に分け5週間飼育した。運動は水槽内での水泳運動とし、1日90分、週5日の頻度で実施した。LNは飲水(1mg/mL)にて投与した。【結果】小動物用トレッドミルにより評価した仕事量は、SED群に比較してET群で有意に増加したが、EX+LN群では有意に低下した(p<0.05)。また、同時に測定した最高酸素摂取量、骨格筋ミトコンドリアにおけるクエン酸回路・電子伝達系にかかわる因子の酵素活性・遺伝子発現も、SED群に比較しEX群において有意に増加したが(p<0.05)、EX+LN群で有意に減少した。【結論】NOは、運動誘発性の運動能力およびミトコンドリア機能増加における重要な調節因子であることが示唆される。(著者抄録).

115. 沖田 孝一, 高田 真吾, 門口 智泰, 森田 憲輝, 堀内 雅弘, 絹川 真太郎, 筒井 裕之, 血流制限を併用したレジスタンス運動が血管機能へ影響, 北翔大学北方圏生涯スポーツ研究センター年報, Vol.6, pp.15-17, 2015.10, 従来の高強度負荷を用いるレジスタンス運動では、血管機能を悪化させるとの報告が散見される。近年、血流制限を併用したレジスタンス運動が、低強度負荷にもかかわらず、顕著な筋肥大および筋力増加をもたらすことが報告された。しかしながら、同運動が血管機能にどのような影響を及ぼすのかは明確にされていない。本研究では、血流制限を併用した低強度レジスタンス運動が筋量・筋力および血管機能に与える影響について検討した。健常男性14名にて、右下腿を対象に、最大挙上重量(1RM)の20%を用いた血流制限下仰臥位足関節底屈運動を、60収縮(30回/分、2分間)を2セット、1日2回、週3回、4週間実施した。血流制限は、右大腿部に空気圧式カフを装着し、安静時収縮期血圧の1.3倍にて施行した。トレーニング前後、血管機能の指標として、CAVI(心臓足首血管指数)、四肢血圧をそれぞれ、フクダ電子社製脈波検査装置(VaSera:VS-1000)により、また、1RMおよび最大筋横断面積(MCA)を測定した。トレーニング後、1RMおよびMCAは有意に増加した(p<0.05)。血圧値は対象肢の収縮期血圧においてのみ有意に低下した(p<0.05)。CAVI値はいずれの側においても変化が認められなかった。血流制限を併用した低強度レジスタンス運動は、筋量および筋力を増加させるとともに、血管機能を改善する可能性が示唆された。(著者抄録).
116. 精神及び身体機能高度低下例における複合的音楽療法の可能性
"Exercise is medicine"に象徴されるように、運動療法を主軸とした心臓リハビリテーションは、疾患治療ツールとして発展してきた。しかしながら、超高齢化が進む臨床現場では、歩行すら困難な患者が増加しており、運動療法の限界が懸念される。身体負荷が少ない音楽療法などの芸術療法は、特にうつ状態や認知機能の改善を目的として民間療法的に施行されているが、ターゲットとする精神・神経機能への効果を客観的に評価することが難しく、科学エビデンスに乏しいため、医療には組み込まれていない。一方、精神・神経機能は、心疾患者の予後に影響することはよく知られている。脳由来神経栄養因子(BDNF)は、神経細胞の寿命、成長、維持に中心的な役割をしており、その血中レベルが、学習、記憶、認知機能やうつ、さらには狭心症患者の予後に関連することが報告されている。そこで我々は、身体機能が著しく低下した高齢者において、血中BDNFを精神・神経機能のバイオマーカーとして用い、歌唱、簡易楽器の演奏からなる複合的な音楽療法の効果を検討し、良好な結果を得たので、心リハ領域における音楽療法の潜在的有用性について報告する。(著者抄録).
